How Fitness Trackers Estimate Steps, Sleep, and Heart Rate

Fitness trackers estimate steps, sleep, and heart rate from sensor signals; they do not directly count every footfall, read brain activity, or measure blood flow like clinical instruments. The results depend on the device, how it is worn, what you are doing, and how its software interprets the data. Step counts can be useful for tracking patterns, wrist heart-rate readings are sensitive to movement and fit, and sleep stages are best treated as estimates rather than a diagnosis.

How a tracker turns sensor signals into numbers

A wearable collects signals from sensors such as an accelerometer and optical heart-rate sensor, then applies software to classify them. The sensor data are not the same thing as the final metric: a step count, pulse reading, or sleep-stage label is an algorithmic estimate. Manufacturers do not all use identical methods, and the details can vary by product and generation.

That distinction explains why two trackers can disagree without either showing an obvious malfunction. A meaningful comparison needs to match the metric, the reference method, the activity, and the conditions in which the devices were worn.

How fitness trackers estimate steps

Wrist trackers detect patterns of movement and classify some of them as walking or running. Fitbit says its algorithm looks for motion and intensity patterns associated with those activities; that is an example of one company’s approach, not a disclosure of every brand’s algorithm. Wrist placement creates predictable sources of error. A person pushing a stroller may walk while keeping the tracker arm relatively still, so steps can be missed. Conversely, arm movements during cooking or desk work may resemble walking and add steps. Fitbit also says wrist preference affects sensitivity. Its distance estimate uses height to estimate stride length, so distance is not simply a direct measurement of each step.

What studies say about step-count accuracy

A 2020 systematic review of 158 publications covering nine wearable brands found that validity differed among brands. In controlled studies, Apple and Samsung had higher step-count validity and less variability than several other brands; comparisons were affected by the number of studies available and device wear location. Those historical findings do not establish a ranking for current models.

Everyday performance was less consistent in the review’s free-living comparisons: 29 of 69 comparisons with calculable group error, or 42%, were within ±10% of the reference count. The pooled results slightly overestimated steps overall, with a mean error of 5% and median error of 6%. These are summaries of the studies reviewed, not a forecast for any particular tracker or wearer. Fuller et al., JMIR mHealth and uHealth, 2020

How wrist trackers estimate heart rate

Many wrist devices use photoplethysmography, usually abbreviated PPG. LEDs shine light into the tissue, and an optical sensor detects changes in reflected light associated with the pulse-related changes in blood volume near the skin. Software processes that signal to estimate heart rate. It is an optical estimate, not a direct measurement of the heart’s electrical activity.

Movement can make the pulse signal harder to separate from other changes. A Garmin-focused review identifies movement, sweat, skin temperature, fit or compression, and individual factors as influences on readings. A loose or shifting watch and vigorous arm movement can therefore make a reading less dependable. Review of Validity and Reliability of Garmin Activity Trackers, 2020

Heart-rate accuracy depends on the setting

In the controlled-setting comparisons summarized by Fuller and colleagues, 100 of 177 comparisons (56.5%) were within ±3% measurement error. The median error across comparisons was −1%. The review’s historical brand analysis found Apple Watch within ±3% in 71% of comparisons, Fitbit in 51%, and Garmin in 49%; differences among models and study conditions mean these figures should not be read as a current brand leaderboard. The Garmin-focused review also found heart-rate validity generally better at rest than during activity, with performance degrading as arm movement and intensity rose.

For a useful comparison, check whether the study measured resting pulse or exercise heart rate, what activity and intensity were involved, and which device generation was tested. A result from a controlled setting does not guarantee the same performance during an ordinary workout.

How trackers estimate sleep and sleep stages

Consumer wearables infer sleep from movement and physiological signals rather than measuring brain activity as a sleep laboratory does. The Garmin review describes devices using accelerometer and heart-rate data, with newer models also using heart-rate variability. Its review found only two Garmin sleep-validity studies, and neither used polysomnography, the laboratory reference method, as its criterion. The evidence was sparse and variable.

Quiet wakefulness can be mistaken for sleep, which may lead trackers to overestimate total sleep time or sleep efficiency and underestimate wake after sleep onset. The review describes this as a broader activity-tracker limitation, not an inevitable result for every current product. Sleep-stage labels are also algorithmic classifications, not clinical findings. The available evidence does not establish a validated 2026 comparison of sleep-stage accuracy across brands.

How to interpret disagreements between devices

When two trackers or a tracker and your own count differ, first check whether they were worn in the same position and used under comparable conditions. Then consider what each metric represents: wrist motion classification for steps, an optical pulse estimate for heart rate, or an inference from movement and physiological signals for sleep.

  • Steps: Compare against a manual count over a defined walk, and note whether your arms were moving normally. A stroller, shopping cart, or arm-intensive task may change the result.
  • Heart rate: Compare readings at rest or during the same type of activity, with the device fitted consistently. A comparison during vigorous movement may reflect signal limitations as well as device differences.
  • Sleep: Treat duration and stage labels as estimates. When comparing devices, note whether the reference was a sleep diary or polysomnography; those methods do not provide equivalent validation.

The 2020 multi-brand review does not establish one best brand across all metrics. A tracker that performs well for steps need not perform equally well for heart rate or sleep, so comparisons should be metric-specific and relevant to the conditions in which you plan to use the device.

Are fitness-tracker readings medical measurements?

No. Garmin says its wearables are “intended to be tools to provide you with information to encourage an active and healthy lifestyle.” It also states that its metrics are close estimates that may not be precisely accurate and are not intended for medical purposes. A general consumer tracker’s reading should not be treated as a clinical diagnosis or substitute for medical advice. Garmin: Activity Tracking and Fitness Metric Accuracy
