How Drug Sales, Patent Expirations, and New Product Launches Affect Pharmaceutical Earnings

Pharmaceutical earnings are shaped by a moving portfolio rather than a single sales number. Existing products generate revenue according to prescription volume, net price and product mix; patent or regulatory-protection losses can expose those products to lower-priced generic or biosimilar competition; and new launches can replace lost revenue only if uptake, supply, reimbursement, pricing and margins develop fast enough to cover the costs of bringing them to market.

How established drug sales become earnings

A medicine’s reported sales are the starting point, not the final profit result. Revenue changes with the number of prescriptions or units sold, the price realized after discounts and rebates, and the mix of products and countries in the period. A company may report strong sales growth because it sold more units, achieved a higher net price, shifted toward higher-value products, or experienced a combination of those factors.

The revenue bridge

Driver What changes Why it matters for earnings
Volume Prescriptions, treatment starts, refill rates and units supplied Additional units usually add contribution profit when manufacturing capacity and other variable costs are controlled.
Realized price List price adjusted for rebates, discounts, government programs and payer contracts A higher net price can lift gross profit, but resistance from payers may reduce access or volume.
Product and geographic mix The proportion of sales from products, indications and countries with different prices and margins Mix can raise or lower gross margin even when total revenue grows at the same rate.
Foreign-exchange translation Conversion of overseas sales into the reporting currency Reported growth can differ from growth at constant exchange rates.

Revenue does not translate one-for-one into net income. Companies also pay for research and development, manufacturing, sales forces, medical education, advertising, distribution, administration, interest, taxes, collaborations, impairments, restructuring and other special items. A launch can therefore increase revenue while reducing near-term earnings if commercialization spending rises faster than gross profit.

Lilly’s 2024 example

Eli Lilly and Company reported 2024 revenue of $45,042.7 million, up from $34,124.1 million in 2023, a 32% increase. The company attributed the increase primarily to Mounjaro, Zepbound and Verzenio, with higher volume and, to a lesser extent, higher realized prices contributing to growth. Net income rose to $10,590.0 million from $5,240.4 million, and diluted earnings per share increased to $11.71 from $5.80.

The larger percentage increase in net income and diluted EPS was not simply a mathematical consequence of the 32% sales increase. Lilly said higher gross margin was the primary driver, partly offset by increased research and development, marketing, selling and administrative expenses, as well as special charges. Gross margin was 81.3% of revenue in 2024 compared with 79.2% in 2023, mainly because of favorable product mix and higher realized prices.

Which earnings measure is being used?

When comparing companies, keep the denominator and accounting basis consistent. Diluted EPS includes the effect of potential share dilution. Some companies emphasize reported earnings, while others also present adjusted or “core” earnings that exclude selected items. AstraZeneca’s 2024 annual report, for example, reported 18% growth in both total revenue and reported EPS, with total revenue of $54.1 billion and reported EPS of $4.54; it also presented core EPS separately. Reported and adjusted figures can tell different stories because the adjustments may include acquisition costs, restructuring, impairments or other items.

What a patent or exclusivity loss does to a product

Protection loss changes the competitive environment around a medicine. Generic manufacturers generally offer the same active ingredient, while biosimilars are highly similar biological products. Because competing products commonly enter at lower prices, the original product can lose both price and volume, sometimes quickly. Pfizer states in its 2025 Form 10-K that generic and biosimilar competition after patent or regulatory-exclusivity loss can substantially reduce affected product revenue in a short period.

Patent rights and regulatory exclusivity are different

A patent and regulatory exclusivity are separate legal protections. The U.S. Food and Drug Administration explains that a patent may be issued or expire independently of drug approval, while exclusivity attaches when a product is approved if statutory requirements are met. The protections can overlap, expire at different times and cover different aspects of a medicine, such as its active ingredient, formulation, method of use or a period during which an application cannot be approved.

U.S. protection example Period described by the FDA Qualification
New chemical entity exclusivity Five years Applies when statutory requirements for a qualifying new chemical entity are met.
Orphan-drug exclusivity Seven years Applies to an approved orphan indication that satisfies the applicable requirements.
New clinical investigation exclusivity Three years Linked to qualifying new clinical investigations supporting an approval or change.
Pediatric exclusivity Six months added to existing protection Granted when the required pediatric studies and statutory conditions are completed.
New U.S. patent term Generally 20 years from filing The FDA notes that adjustments and other factors affect the effective duration.

These periods are legal context, not a universal timetable for generic or biosimilar competition. A company analyzing an individual product must examine every relevant patent, listed use, formulation, litigation outcome, regulatory exclusivity and jurisdiction.

Why the expiry date is not the competition date

Patent expiration can make entry legally possible, but it does not by itself establish when a rival will launch. A generic or biosimilar application may be delayed, challenged in court, settled, held for regulatory reasons or launched in only selected countries. Conversely, entry can occur on a date different from the headline patent or exclusivity expiry. Pfizer specifically cautions investors about this distinction and says it expects significant revenue reductions from patent-based or regulatory-exclusivity expiries during 2026 through 2030. That is Pfizer’s company-specific forward-looking outlook, not an industry-wide forecast.

How erosion reaches the income statement

Before entry, an originator may have substantial sales and a high gross margin. After entry, the originator can face a lower net price, reduced formulary preference, fewer prescriptions and higher contracting costs to defend share. Manufacturing costs may not fall as quickly as revenue, so gross margin can compress. Sales and marketing resources may be redeployed, and a company may record impairment or restructuring charges if it concludes that future cash flows are lower than previously expected.

The scale of the impact depends on the product’s contribution to company revenue and profit, the number and aggressiveness of competitors, the pace of substitution, the durability of clinical differentiation, and the countries in which competition begins. A product that is already declining may create less incremental damage than a fast-growing blockbuster, while a product with several protected indications may lose revenue in stages.

How a new launch affects revenue and profit

A launch adds earnings through a sequence of operational milestones rather than on approval day. The product must be manufactured at commercial scale, distributed, recognized by physicians and patients, covered by payers, and retained by users long enough to generate repeat demand. Additional indications or countries can extend the growth curve, but each expansion requires evidence, regulatory work and commercial investment.

The launch path

  1. Approval and labeling: The approved indication determines which patients can be promoted to and which reimbursement rules apply.
  2. Supply availability: Manufacturing capacity, raw materials, quality release and distribution determine whether demand can become recorded sales.
  3. Market access: Payer negotiations, formularies, government programs and patient affordability influence the net price and the number of eligible users.
  4. Clinical adoption: Physicians, health systems and patients need evidence, education and practical access before prescriptions scale.
  5. Commercial expansion: New indications, geographic launches and additional formulations can broaden demand but also add regulatory and selling costs.

Zepbound illustrates the ramp, not a benchmark

Lilly reported U.S. Zepbound revenue of $4.93 billion in 2024, compared with $175.8 million in 2023. The company said the product launched in the United States in November 2023 for adults with obesity or overweight with weight-related comorbidities. The figures show how quickly a product can add sales after launch, but they are a company-reported example, not a typical launch rate or a promise that other products will follow the same trajectory.

That launch also carried costs. Lilly said marketing, selling and administrative expenses rose 16% in 2024, primarily because of promotional efforts supporting ongoing and future launches. A launch can therefore be accretive to earnings, dilutive, or neutral in its first periods depending on gross margin, promotional intensity, manufacturing investment and the speed at which repeat demand develops.

Management targets are not realized earnings

AstraZeneca’s chief executive Pascal Soriot stated in the company’s 2024 annual report: “By 2030, we aim to launch at least 20 new medicines and achieve $80 billion in Total Revenue, with sustained growth thereafter.” This is management’s forward-looking ambition. It should be tested against development success, approval timing, competitive entry, pricing, access and the cost of building each product, rather than treated as an achieved result or an independent forecast.

Can launches offset a patent cliff?

The right question is not whether a company has launches; it is whether the launches can replace the specific profit exposed to competition in the same period and markets. A portfolio may look healthy in aggregate while still facing a temporary earnings decline if an older product loses revenue before newer products reach scale.

Question Evidence to examine Interpretation
How large is the exposed product? Annual sales, gross profit contribution and share of company revenue A small product loss is easier to absorb than a product that funds a large portion of operating costs.
When can competition begin? Patent and exclusivity records, litigation, settlement terms and country-by-country estimates Use a range of dates; an expiry date alone is insufficient.
How fast can the launch scale? Supply capacity, prescriptions, refill behavior, payer coverage and geographic rollout Early revenue can be volatile and may not represent a steady run rate.
What margin will remain? Net price, manufacturing cost, royalties, collaboration economics and promotional spending High sales growth can produce modest earnings growth if costs or revenue sharing are high.
Are the measures comparable? Reported versus adjusted/core earnings, diluted share counts and currency basis Different definitions can make replacement appear larger or smaller than it is.

Protection dates and competitor-entry forecasts contain legal and commercial uncertainty. Analysts should model several timing cases, such as delayed entry, one competitor entering first, and broader substitution after multiple competitors arrive. The purpose is not to predict a single precise date but to understand how sensitive earnings are to each outcome.

How to read a pharmaceutical earnings report

Start with product-level sales

Identify which products drove the period’s change and whether management attributes it to volume, price, mix, acquisitions or currency. Check whether growth came from the same products that face future protection loss. A company can report impressive total growth while its mature portfolio is already contracting.

Separate gross profit from operating spending

Compare gross margin with research, marketing, selling and administrative expenses. A launch may have attractive product economics but require years of clinical, manufacturing and promotional spending. Look for impairments, restructuring, collaboration payments and other special items that affect reported net income but may not recur.

Check geography and indication

Sales and protection status differ by country. A product may face competition in one market while remaining protected elsewhere. Likewise, an indication may lose exclusivity before another. Use the company’s geographic and product tables rather than applying a single worldwide date.

Track cash and capacity, not only EPS

Working-capital changes, inventory build, capital expenditure and manufacturing expansion can consume cash before a launch reaches scale. EPS can also benefit from share repurchases or change because of dilution, even when operating performance is unchanged.

Common mistakes when assessing pharma earnings

  • Equating sales growth with profit growth: Higher revenue can be outweighed by research, launch, manufacturing or administrative costs.
  • Treating patent expiry as an automatic first-day collapse: Entry timing, litigation, settlements and substitution vary by product and country.
  • Assuming every launch replaces an expiring blockbuster: Most launches have different markets, margins, adoption curves and commercial requirements.
  • Using a company target as a forecast: Pipeline and revenue ambitions are forward-looking statements with execution risk.
  • Mixing reported and adjusted figures: The difference can be material when special charges or acquisition accounting are present.
  • Ignoring net price: List-price increases do not necessarily produce higher realized revenue after rebates and payer concessions.
  • Generalizing from one product: Lilly’s Zepbound growth demonstrates one launch outcome, not an industry-wide success rate or standard earnings contribution.

A practical framework for estimating the earnings effect

For each major product, build a timeline with sales, margin and protection status by market. Mark the earliest plausible competitor-entry date and a later case that reflects litigation or regulatory delay. Then estimate how quickly price and volume could change after entry, without assuming a universal erosion percentage.

For each launch, map approval, supply availability, reimbursement, physician adoption and indication expansion. Estimate revenue and gross profit separately from research and commercial spending. Finally, compare the launch’s expected contribution with the profit—not merely the revenue—at risk from the older product during the same quarters.

This framework highlights mismatches that headline growth can hide: a launch may be large but low-margin; a patent cliff may arrive before supply is ready; or a product may remain protected in the largest market while losing exclusivity in smaller ones.

What to monitor each quarter

  • Product volume, net price and mix explanations in the earnings release.
  • Gross-margin movement and the stated reasons for change.
  • Research, selling, marketing and administrative spending tied to launches or portfolio changes.
  • Updates to patent litigation, regulatory exclusivity and expected competitor-entry dates.
  • Launch prescriptions, supply constraints, payer coverage and geographic expansion.
  • Reported, diluted and adjusted/core EPS on a consistent basis.
  • Management’s revised guidance, identifying which statements are historical results and which are forward-looking expectations.

Drug sales provide the inflow, protection determines how durable that inflow is, and launches determine whether a company can replace what is lost. Pharmaceutical earnings improve when new or expanding products deliver enough high-quality, net-priced revenue and margin to exceed the costs of development and commercialization before older products face meaningful competition.
