How Doctors Monitor Side Effects During Radiopharmaceutical Treatment

Doctors monitor radiopharmaceutical treatment with a plan tailored to the drug, cancer being treated, dose schedule, the patient’s health and the treatment center’s protocol. There is no single testing calendar for every radiopharmaceutical. Depending on the treatment, monitoring can include blood counts, kidney or liver tests, questions about symptoms and separate tests that measure whether the cancer is responding. Ask your nuclear-medicine and oncology teams which checks apply to your treatment and when they are due.

What monitoring is meant to catch

A radiopharmaceutical combines a radioactive atom with a molecule that targets cancer cells. Different medicines target different cancers and may expose different healthy tissues, so the risks and follow-up are not interchangeable. The Society of Nuclear Medicine and Molecular Imaging (SNMMI) provides background on radiopharmaceutical therapy and prostate cancer; the National Cancer Institute lists approved examples across several diseases, including Pluvicto, Lutathera, Xofigo, Azedra and iodine-131. Those examples do not share one side-effect profile or monitoring schedule.

Monitoring has two related but distinct jobs. Safety checks look for treatment effects on blood cells, organs or how a person feels. Response checks look for signs that the cancer or a related disease marker is changing. A marker used to assess response is not, by itself, a test for treatment toxicity.

That difference can help make sense of a clinic visit that includes several kinds of tests. A CBC or kidney-function test helps the team assess safety; a cancer marker such as PSA may help assess response. A result can be reassuring for one purpose without answering the other. Ask what each planned test is intended to show, and how its result will be used in decisions about treatment.

What tests may be scheduled, and when?

The timing depends on the specific medicine. The following US examples show why it is important to confirm the name of the treatment when discussing a schedule; the timing for one drug should not be applied to another.

Treatment example Documented blood and organ checks Other follow-up described
Lu-177 PSMA therapy (Pluvicto) The FDA’s 2026 US prescribing information directs clinicians to check complete blood counts and kidney-function laboratory tests before and during treatment. Kidney testing includes serum creatinine and calculated creatinine clearance. SNMMI’s patient guidance says tests several days before each treatment include red cells, white cells, platelets, liver function and kidney function. SNMMI describes PSA follow-up during treatment. The FDA label also gives treatment-specific hydration and radiation-safety precautions.
Radium-223 (Xofigo) SNMMI says blood counts are checked before treatment begins and no later than 10 days before every therapy cycle; further blood work may be obtained as needed. A specified kidney- or liver-test schedule is not stated on that patient page. SNMMI describes PSA and alkaline phosphatase as markers used to track prostate cancer and bone disease. The page does not establish a radiation-safety schedule here.

Sources: FDA 2026 Pluvicto prescribing information, SNMMI Pluvicto patient guidance and SNMMI Radium-223 patient guidance. SNMMI pages are patient education, not an individualized protocol; a treating center may give instructions for a particular patient.

The wording of the Pluvicto sources is worth noting: the FDA label calls for CBC and kidney tests before and during treatment, while SNMMI describes tests several days before each dose. Those statements do not create an exact calendar for every patient. Your care team can tell you the dates needed for your treatment and whether a result must be available before a dose. The Xofigo timing above is specific to radium-223 and should not be used to plan tests for Pluvicto or another medicine.

How blood counts help clinicians assess safety

A complete blood count (CBC) measures components of blood that can be affected by treatment. Red blood cells carry oxygen; white blood cells help the body fight infection; platelets help blood clot. Clinicians use the results and their change over time, alongside symptoms and the patient’s overall condition, to decide whether it is appropriate to give the next dose as planned.

Looking at more than one blood component matters. Hemoglobin is a measure related to red blood cells, while leukocytes and neutrophils are types of white blood cells; platelets are involved in clotting. A clinician interprets each result in context rather than treating every count as the same problem. The team may also compare current results with earlier results to understand whether counts are changing during treatment.

For Pluvicto, the FDA’s 2026 label warns of potentially severe myelosuppression and directs CBC checks before and during treatment. In the VISION study, grade 3 or 4 decreased hemoglobin was reported in 15% of Pluvicto-treated patients, decreased platelets in 9%, decreased leukocytes in 7% and decreased neutrophils in 4.5%. Grade 3 or higher pancytopenia was reported in 1.1%, including two fatal events. These are observations from VISION, not an individual risk prediction or a rate for every radiopharmaceutical.

When toxicity is significant, the label says clinicians may withhold, reduce or permanently discontinue Pluvicto according to the severity of myelosuppression. An abnormal result does not mean that every patient automatically stops treatment: the clinical team interprets the finding in context and follows the medicine’s prescribing information. Depending on what is found, a treatment change may be a delay or dose reduction rather than permanent discontinuation.

How kidney and other organ checks fit in

For Pluvicto, the FDA label specifically directs kidney-function laboratory tests, including serum creatinine and calculated creatinine clearance, before and during treatment. These checks help clinicians assess renal toxicity and determine whether a treatment change is needed. The label bases withholding, dose reduction or discontinuation on the severity of renal toxicity, rather than saying that any single abnormal value requires the same action for everyone.

In the VISION study, grade 3 or 4 acute kidney injury occurred in 3.4% of Pluvicto-treated patients. The corresponding reported rates were 1.3% in PSMAfore and 1.6% in PSMAddition. These are study-specific results from the 2026 FDA label. The label cautions that adverse-event rates from different trials cannot be directly compared because trial conditions differ.

SNMMI’s patient guidance for Lu-177 PSMA also lists liver-function tests among the checks several days before each treatment. That patient overview does not provide a universal liver-test calendar for every radiopharmaceutical. For another medicine, ask the treating team which organ tests are relevant and how their timing relates to each dose.

For radium-223, the SNMMI patient page specifically describes blood-count checks; it does not provide a kidney- or liver-test schedule. That is a limit of what the page establishes, not a reason to assume those tests are or are not needed for an individual. Ask the team what it plans for your therapy and why.

What symptom checks and response tests can tell you

Laboratory results are only one part of follow-up. Clinicians also ask about new or worsening symptoms and consider them alongside examination findings and test results. SNMMI identifies fatigue and dry mouth as main side effects described for Lu-177 PSMA therapy and notes that pain may occur as tumors respond. These examples are specific to that patient guidance; they should not be read as a complete list of possible effects for all radiopharmaceuticals.

Tell your team about symptoms that are new or getting worse, even if you are unsure whether they are related to treatment. It can help to describe when a symptom began and whether it has changed since the last visit. The care team can decide whether it needs assessment and whether it affects the treatment plan; do not use another medicine’s symptom list as a substitute for its advice.

Some follow-up tests measure disease response rather than harm from treatment. During Lu-177 PSMA therapy, SNMMI describes following PSA, a prostate-cancer marker; a decrease may indicate response. PSA does not show on its own whether blood counts or kidney function have been affected. For radium-223, SNMMI describes PSA and alkaline phosphatase as blood markers used to track prostate cancer and bone disease. Which markers are useful depends on the cancer and treatment.

The distinction matters when interpreting a test result: a response marker answers a different question from a safety test. Your clinician can explain what a particular result is intended to track and whether it changes the treatment plan. A PSA result, for example, should not be mistaken for a blood-count or kidney-safety check.

Radiation-safety instructions are separate from toxicity monitoring

Precautions after administration address possible radiation exposure to people nearby. They are not blood tests or a way to detect treatment side effects. The FDA’s 2026 US Pluvicto label gives the following specific instructions after each administration:

  • Limit close contact, defined in the label as less than 3 feet, with other people for 2 days, and with children and pregnant women for 7 days.
  • Avoid sexual activity for 7 days.
  • Sleep separately for 3 days from other people, 7 days from children, or 15 days from pregnant women.
  • Maintain hydration and urinate frequently before and after administration, as advised in the label.

These periods are Pluvicto-specific instructions in current US labeling, not universal rules for radiopharmaceutical therapy. SNMMI says the nuclear-medicine physician or radiologist will tell patients which precautions apply and for how long. Follow the written directions from your own treatment center rather than using another drug’s instructions. If you share a household with children or a pregnant person, make sure you understand which contact and sleeping instructions apply to your treatment.

How to prepare for appointments and between-dose follow-up

Confirm the plan with the clinicians who know the medicine, dose schedule and your health history. Before a treatment or follow-up appointment, it can help to write down questions and note symptoms that have changed since the previous visit. Do not assume a test schedule from another patient, another medicine or an online example applies to you.

  • Ask which blood counts and organ-function tests are planned, and how many days before or after each dose they should be done.
  • Ask which symptoms to report between appointments, whom to contact and how to reach the team.
  • Ask which results assess safety and which are used to track the cancer’s response.
  • Ask what radiation-safety instructions apply at home after your particular treatment, including how long they last.

You can also confirm how you will learn that test results are ready and whom to contact if a planned test has not been arranged or a result is delayed. If you are unsure whether a result is current for the next dose, ask the treatment team rather than assuming the dose can proceed. The clinicians can clarify whether the result is needed before treatment and whether it affects timing.

Report new or worsening symptoms to your care team and follow its directions about when to seek help. These sources do not establish one urgent-care rule that applies to everyone receiving radiopharmaceutical treatment. The appropriate next step depends on the symptom and the individual treatment plan.

Why your own schedule may differ from an example

Even when a label or patient page describes routine checks, it does not determine every appointment for every patient. The medicine and indication, dose timing, prior results, health status and local treatment protocol all matter. In the United States, the FDA label is the prescribing source for Pluvicto-specific requirements; patient-facing SNMMI pages help explain examples but are not substitutes for clinician instructions. Detailed schedules described for Pluvicto and radium-223 should not be generalized to Lutathera, Azedra, iodine-131 or other therapies.

If a planned test has not been arranged, a result is delayed, or you are unsure whether a result is current for the next dose, contact the treating team before assuming the dose can proceed. Your nuclear-medicine and oncology clinicians can clarify which tests are required for your specific treatment and whether a result affects its timing.

Frequently Asked Questions

What blood tests might I need before each radiopharmaceutical treatment?

It depends on the medicine. For Pluvicto, the FDA’s 2026 prescribing information calls for complete blood counts and kidney-function tests before and during treatment; SNMMI describes checks several days before each dose that include red cells, white cells, platelets, liver function and kidney function. For radium-223, SNMMI says blood counts are checked before treatment starts and no later than 10 days before each cycle. Confirm your own dates and tests with your treatment team.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How will my doctor know if treatment is affecting my kidneys or blood counts?

For Pluvicto, clinicians use CBC results to monitor blood cells and kidney-function tests that include serum creatinine and calculated creatinine clearance. They interpret results alongside symptoms, prior results and your overall condition. The FDA label allows treatment to be withheld, reduced or discontinued based on the severity of toxicity; an abnormal result does not automatically mean treatment must stop.

Does a falling PSA mean I have no treatment side effects?

No. SNMMI describes PSA follow-up during Lu-177 PSMA therapy, and a decrease may indicate a cancer response. PSA is a response marker, not a test of blood counts or kidney function. Safety monitoring and response monitoring answer different questions.

Are radiation-safety precautions the same for every radiopharmaceutical?

No. The FDA’s 2026 Pluvicto label gives specific post-treatment instructions, but those time periods should not be applied to other medicines. Ask your nuclear-medicine team for the written precautions for your treatment and household.

The Bottom Line

Monitoring is specific to the radiopharmaceutical: blood counts, organ-function tests, symptom checks and cancer-response markers each serve different purposes. Confirm your testing dates, symptoms to report and household radiation precautions with your nuclear-medicine and oncology teams.
