How Doctors Investigate Hives That Keep Coming Back

Doctors usually investigate recurring hives by reconstructing the pattern first, then examining the skin and looking for clues that justify a specific test. Hives that recur or persist for more than six weeks fit the chronic urticaria timeframe, but that label does not automatically mean a food allergy or another hidden disease. For many people, the most useful evaluation is a careful history, review of systems and physical examination rather than a broad testing panel.

This article explains what clinicians ask, when allergy or blood testing makes sense, which symptoms can point to a different diagnosis, and how to prepare for the appointment. It is general information, not a personal diagnosis or treatment plan.

What “recurring hives” means medically

Urticaria is the medical term for hives. Episodes lasting less than six weeks are classified as acute urticaria. Hives that recur or continue beyond six weeks fall into the chronic urticaria category.

Chronic urticaria can be:

  • Chronic spontaneous urticaria: wheals appear without a consistent external trigger. This form is often not caused by an allergy.
  • Chronic inducible urticaria: lesions are repeatedly provoked by a physical stimulus such as pressure, heat, cold, scratching, exercise or sweating.

The overall condition may last months or years, while an individual wheal commonly fades within 24 hours. New spots can appear as older ones disappear, making the rash seem continuous. Deeper swelling, called angioedema, may accompany hives or occur by itself and can last longer than a surface wheal. Swelling without hives can involve a different mechanism and deserves medical assessment.

Difficulty breathing or swelling of the tongue or throat is an emergency. Seek immediate medical attention rather than waiting for a routine allergy appointment.

Step 1: Reconstruct the timeline

The first diagnostic tool is a precise chronology. A clinician is trying to separate a short-lived episode from a chronic pattern and to determine whether each lesion behaves like a typical wheal.

Questions about onset and frequency

  • When did the first episode begin?
  • How often do new episodes occur?
  • Are there symptom-free intervals?
  • How long does one individual spot remain in exactly the same place?
  • Do old lesions disappear completely, or leave discoloration, bruising or another mark?

The six-week threshold applies to the duration or recurrence of the condition, not to the life of one spot. A single lesion that remains fixed for much longer than a day, especially if it hurts or leaves a mark, may prompt the clinician to reconsider whether it is ordinary urticaria.

Why dates matter

People often remember the worst flare but not the sequence around it. Dates can reveal whether symptoms began after a new medicine, infection, travel, repeated pressure or another event. They can also show whether an apparent trigger occurs every time or was present only once by coincidence.

Step 2: Describe the rash and other symptoms

“Hives” describes a pattern, not one single appearance. The clinician will ask what the lesions feel like, where they occur and what happens elsewhere in the body.

Features of each lesion

  • Is it itchy, burning, painful or mostly unnoticed?
  • Is it raised and pale or pink in the center, with surrounding redness?
  • Does it move or change shape as it fades?
  • Does it bruise, scale, blister or leave persistent discoloration?

Associated swelling and systemic symptoms

Report swelling of the lips, face, eyelids, tongue, throat, hands, feet or abdomen, including whether it occurs with visible hives. Also mention fever, joint pain, breathing trouble, abdominal symptoms, vomiting, diarrhea or faintness. These details do not prove a particular cause, but they can change which conditions the clinician considers and whether urgent care is needed.

Angioedema can last longer than a wheal. Swelling that repeatedly occurs without hives is especially important to describe because it may lead the clinician toward a different evaluation than the one used for typical itchy wheals.

Step 3: Review plausible triggers, medicines and exposures

Doctors look for a repeatable time relationship, not merely something that happened before a flare. Bring a complete list of prescription medicines, over-the-counter products, supplements and recent dose changes. Include medicines taken only occasionally, such as pain relievers or antibiotics.

Exposures a clinician may ask about

  • Foods and drinks, including whether symptoms begin consistently soon after one specific item.
  • New or changed medicines.
  • Insect bites or stings.
  • Recent infections.
  • Heat, cold, pressure from clothing or straps, scratching, exercise or sweating.

Do not treat an unverified association as proof of causation. Chronic spontaneous hives are usually not due to a food allergy, pet exposure or stress. A food or medication allergy becomes more plausible when the same exposure is followed by a similar reaction repeatedly and within a medically credible time window. Do not start broad food elimination on your own; discuss suspected foods with a clinician.

Step 4: Examination and review of the wider health picture

The current US supplementary guideline describes history, review of systems and physical examination as the core routine evaluation. During the examination, the clinician may look at active lesions, examine areas where swelling has occurred and check whether marks or other skin changes remain.

The guideline states: “No laboratory testing is required if there are no suspected occult diseases based on history, review of systems, and physical examination.” This is a conditional recommendation, not a rule that testing is never appropriate. It means a normal, typical presentation does not automatically require a large screening panel.

If the rash is absent during the visit, photographs and a symptom record can help the clinician assess whether the reported pattern fits urticaria.

When testing is useful—and when broad panels can mislead

Testing should answer a clinical question raised by the history or examination. Untargeted panels can produce false-positive results in people who do not have the disease being tested for. That can lead to anxiety, unnecessary dietary restrictions, extra appointments and more investigations without explaining the hives.

History-led evaluation versus untargeted testing

Approach What prompts it Potential benefit Potential downside
History, review of systems and examination A typical pattern without clues to another disease Identifies the chronic pattern and avoids tests that are unlikely to change care May not identify one external cause, because chronic spontaneous urticaria often has no single obvious trigger
Targeted laboratory or allergy evaluation A specific symptom, exposure, physical trigger or risk factor Investigates a defined possibility Results must be interpreted in context; an isolated positive result may not explain the rash
Broad screening panels No particular clinical clue Usually little added information for typical chronic urticaria Higher chance of incidental findings, cost and follow-up work

Allergy testing

Routine allergy panels are not usually helpful for chronic spontaneous hives. Targeted testing may be considered when a particular food, medicine, insect exposure or other allergen has a consistent, plausible timing relationship to symptoms. A positive test alone does not prove that the exposure caused every flare, and a negative panel does not replace a careful history.

Other targeted tests

The 2026 US supplementary guideline notes that testing for parasitic infection may be appropriate when regional prevalence, symptoms or exposure risk support that question. The same principle applies to other investigations: the clinician chooses them because a feature makes a specific disease worth checking, not because every person with hives needs the same battery of tests.

Step 5: Recognize patterns that may be a mimic

Persistent itchy rashes are sometimes mistaken for hives. The clinician may reconsider the diagnosis when lesions stay fixed, hurt, bruise, blister, leave marks or occur with prominent systemic symptoms.

Clues that can change the workup

Observed pattern Why it matters
Individual lesions last substantially longer than a day or leave bruising or discoloration May suggest a process other than ordinary transient wheals, such as urticarial vasculitis
Fever, joint pain or recurrent inflammation with the rash Can prompt consideration of autoinflammatory syndromes or another systemic illness
Grouped itchy bumps after outdoor or insect exposure May fit papular urticaria rather than spontaneous hives
Deep swelling without hives Can point toward bradykinin-mediated angioedema or another non-histamine mechanism
Blisters, scaling or a different persistent skin eruption May lead to consideration of bullous pemphigoid, dermatitis or another skin disease

These are possibilities for a clinician to assess, not conditions to diagnose from a checklist. Atypical findings may justify referral to allergy/immunology or dermatology, a skin biopsy or targeted laboratory work. They do not mean every person with recurring hives needs those procedures.

What to bring to the appointment

A short, dated record is more useful than trying to remember months of flares from memory. ASCIA says, “Keeping a diary of symptoms with a photographic record of hives can be very helpful for this.” A phone note or calendar is sufficient; a special notebook is optional.

Record these details

  • Date and approximate time each flare began.
  • How long an individual spot lasted and whether it left a mark.
  • Where the rash appeared and whether it moved.
  • Possible exposures in the preceding hours, including food, medicine, infection, heat, cold, pressure, exercise or scratching.
  • Swelling of the face, lips, tongue, throat, hands, feet or abdomen.
  • Fever, pain, joint symptoms, breathing or gastrointestinal symptoms.
  • What you took or did for relief and how the symptoms changed.

Take clear photographs in good light, including a date if your phone allows it. Photograph the whole area and a closer view, while preserving privacy. Images are particularly valuable when the skin is clear on the day of the appointment.

How to interpret the result of the evaluation

A typical history and examination may support chronic spontaneous urticaria without identifying one external cause. That conclusion does not mean the symptoms are imaginary, and it does not imply that a hidden food allergy was missed. It means the available pattern does not point to a specific allergy or occult disease that testing could reliably uncover.

There is also a difference between tests intended to look for another disease and tests proposed to predict how someone will respond to a treatment. The 2026 supplementary document treats predictive testing as a separate question; a routine blood panel should not be presented as a guaranteed way to find the cause or forecast an individual response.

If symptoms continue, change character or fail to follow the expected course, update the clinician rather than repeating random panels. New information—especially fixed lesions, bruising, isolated swelling or systemic symptoms—can change the diagnostic question.

When to seek urgent care

Call emergency services or seek immediate emergency assessment for trouble breathing, throat tightness, or swelling of the tongue or throat. Those symptoms can progress quickly. For recurring hives without emergency features, arrange a clinical evaluation and bring the timeline, photographs and medication list.
