Cold can produce lumps in the fatty layer under the skin, but “cold-induced fatty growth” is not a single medical diagnosis. Doctors first separate cold panniculitis—an inflammatory cold injury that usually improves after exposure stops—from paradoxical adipose hyperplasia (PAH), an uncommon delayed complication in which fat enlarges after cryolipolysis (“fat freezing”). Their timing, appearance, expected course and treatment are different.
A new, hard, enlarging, painful or persistent lump deserves an in-person examination, especially if it appeared after a cosmetic procedure. Appearance alone cannot reliably identify the cause, and another type of mass may need to be ruled out.

What doctors mean by a cold-related fatty lump
The phrase can describe two distinct problems. Both involve tissue beneath the skin, yet one is an injury caused by cold exposure and the other is a delayed adverse response to a body-contouring procedure.

These categories should not be conflated. A post-procedure lump is not automatically PAH, and a cold-induced rash is not evidence that fat has permanently grown.

How the diagnostic visit works
1. The clinician establishes the timeline and trigger
Expect questions about exactly when the lump or rash began, how long each cold exposure lasted, whether exposure was repeated, and whether a cryolipolysis applicator was used at that site. Bring the procedure date, treated body area, applicator information if available, and photographs showing changes over time.
Timing is a major clue. Cold panniculitis generally appears during or shortly after cold injury. PAH is characteristically delayed, with the U.S. Food and Drug Administration (FDA) describing reported cases that developed two to five months after cryolipolysis.
2. The skin and underlying tissue are examined
For suspected cold panniculitis, a dermatologist looks for tiny hard bumps, raised scaly plaques, redness, altered color and deeper tender or itchy lumps in exposed or treated areas. The American Academy of Dermatology (AAD) describes this pattern in people whose skin has been injured by cold, including repeated whole-body cryotherapy.

For suspected PAH, the examination focuses on a well-defined, firm enlargement in the treatment footprint. A rectangular or applicator-shaped contour is a useful clue, but it is not a diagnosis by itself. The clinician also checks for tenderness, skin injury, a hernia, infection and other causes of a subcutaneous mass.

3. Additional tests are selected for the individual case
The reviewed clinical guidance does not establish one universal imaging or biopsy protocol for every suspected PAH case. A clinician may choose imaging when the size, depth or diagnosis is uncertain, or when another mass must be excluded. Published case literature has documented adipose tissue in the treated area on imaging, but that finding does not mean every patient needs the same test.
Biopsy is likewise not automatic. The decision depends on the examination and differential diagnosis. Do not delay an assessment while trying to identify the condition from photographs or by pressing on the lump.
Cold panniculitis: diagnosis, treatment and recovery
What causes it
Cold panniculitis occurs when cold damages the fatty layer beneath the skin and triggers inflammation. It may follow prolonged or repeated exposure, including whole-body cryotherapy, or localized cold applied to an area. The affected skin can itch, hurt, swell or develop bumps and plaques.
Why stopping exposure is the treatment
The central treatment is to stop the exposure that is injuring the skin. The AAD’s patient guidance emphasizes that the rash will not clear while the injury continues. Avoid further whole-body cryotherapy, ice-based treatment or other deliberate cooling of the affected area until a qualified clinician has evaluated it.

In the AAD case description, the skin healed on its own over several weeks after exposure stopped. Darker or lighter patches could remain for months before gradually clearing. Persistent color change does not necessarily mean that the underlying fat has enlarged.
When symptoms need prompt review
Arrange medical care for a lump that is rapidly enlarging, very painful, blistered, numb, draining, associated with fever, or not improving after cold exposure ends. Those features can indicate a deeper injury, infection or a different condition requiring treatment.
PAH after cryolipolysis: recognition and management
Typical presentation
PAH is a delayed increase in fat at a site treated with cryolipolysis—the opposite of the intended reduction. The FDA describes a raised, very firm bulge, often rectangular and corresponding to the applicator. It may become noticeable months after an initially uncomplicated treatment.

Why it may not resolve without intervention
Unlike cold panniculitis, PAH is not expected to reliably disappear simply because the treatment has ended. FDA guidance says the enlarged fatty bulge may not go away on its own and may require surgery. Do not assume that repeating cryolipolysis will correct it; additional cold-based treatment over a suspected complication should wait until a qualified clinician has assessed the area.
Surgical consultation
Correction is individualized. A surgeon considers the bulge’s size, location, skin laxity, contour goals, scars, recovery time and the patient’s general health. Published experience has included liposuction alone and liposuction combined with abdominoplasty. In one small, retrospective single-center series, three patients treated with liposuction required a second procedure. The same series reported high satisfaction among its treated patients, but its size and design do not establish one universally best operation.
Ask a clinician experienced with PAH to explain:
- which procedure fits the amount and distribution of enlarged fat;
- whether loose skin would make abdominoplasty or another skin procedure relevant;
- expected contour improvement and scar placement;
- normal activity restrictions and recovery milestones;
- the possibility of staged or repeat surgery; and
- procedure-specific risks, including anesthesia, bleeding, infection, contour irregularity and recurrence.
How common is PAH?
An often-cited figure is a 0.0051% incidence estimate reported by authors of a 2014 JAMA Dermatology case report. It is a historical, study-specific estimate—not a current universal risk for every device, provider or patient. That article also cited more than 650,000 cryolipolysis treatment cycles from manufacturer data; this was manufacturer-derived volume reported in that publication, not an independently measured current total.
The same 2014 authors said no single unifying risk factor had been identified. Proposed mechanisms and demographic associations remain hypotheses rather than proven predictors. A clinician cannot currently calculate an individual’s reliable personal risk from those suggestions alone.
Other cryolipolysis effects patients should know
FDA consumer guidance lists redness, bruising, swelling, pain or discomfort and skin discoloration as common effects that may resolve without intervention. Less common problems can be more consequential:
- numbness that lasts several weeks;
- temporary palpable fat nodules;
- freeze burns;
- certain nerve-related sensations; and
- throat or nerve symptoms when treatment is performed under the jaw or chin.
PAH and reported hernias are among more serious complications that can require surgery. FDA advises avoiding treatment in or near a pre-existing hernia or another structurally weak area. Tell the treating provider about any hernia, abdominal wall weakness, prior surgery or unexplained lump before treatment.
What to do while waiting for an appointment
- Stop deliberate cold exposure. Do not use whole-body cryotherapy, ice packs or another fat-freezing session on the area until it has been assessed.
- Document the change. Photograph the area in consistent lighting and record measurements, pain, itching, numbness and the date each symptom changed.
- Gather treatment details. Note the cryolipolysis date, body site, number of applications and any immediate skin reaction.
- Seek urgent care for warning signs. Rapid growth, severe pain, spreading redness, blistering, fever, drainage, marked numbness or abdominal symptoms should not wait for a routine appointment.
Do not massage aggressively, attempt to freeze or heat the lump, or rely on a consumer device claiming to diagnose or dissolve it. The available clinical guidance supports stopping the injurious exposure and obtaining clinician-led assessment, not self-treatment.

- What changes should I monitor while we observe it?
- If surgery is appropriate, which options fit my tissue and contour goals?

The key decision is not whether a lump merely “looks like fat.” It is whether the history, examination and—when needed—testing support a cold injury, a delayed post-procedure enlargement or another diagnosis that requires different care.
