How cancer vaccines train the immune system to recognize tumors

Cancer treatment vaccines train immune cells to notice molecular features on a tumor and attack cells carrying those features. Most do this by delivering a tumor antigen—or instructions for making one—to antigen-presenting cells such as dendritic cells. Those cells display the antigen to T cells, which can then recognize the same target on cancer cells. This is a targeted immunotherapy strategy, not a universal cancer cure: the antigen, vaccine platform, cancer type, disease stage and treatment setting all determine whether it can help.

What a cancer treatment vaccine is designed to do

Cancer cells can look sufficiently similar to healthy cells that the immune system does not eliminate them. A treatment vaccine supplies a clearer target. It exposes immune cells to a tumor-associated antigen—a substance found on cancer cells but absent from, or present at lower levels in, normal tissue—or provides genetic instructions for cells to make that antigen.

Dendritic cells are central to the training process. They take up vaccine material, process it and display antigen fragments to T cells. As Karine Breckpot, Ph.D., of Vrije Universiteit Brussel, explained to the National Cancer Institute (NCI), “Dendritic cells act as teachers, educating T cells so that they can search for and kill cancer cells or virus-infected cells,” depending on the antigen. If activated T cells later encounter the target on a tumor cell, they may destroy that cell.

The result depends on biology that varies from one tumor and patient to another. A measurable immune response does not automatically mean a tumor will shrink or that survival will improve. Tumors can lack the selected antigen, contain only a fraction of cells with it, or resist immune attack through other mechanisms. For that reason, a vaccine candidate must be judged in its specific clinical setting rather than treated as interchangeable with every other cancer vaccine.

NCI’s overview describes the goal this way: “Treatment vaccines can help the immune system learn to recognize and react to antigens and destroy cancer cells that contain them.”

How the immune-training process works

  1. Select a cancer-associated target. Developers choose an antigen shared by a cancer type, material from the patient’s own tumor, or a mutation that creates a patient-specific neoantigen. The target must be present on enough tumor cells to be useful while offering a workable distinction from healthy tissue.
  2. Deliver the target or its instructions. A vaccine may contain tumor material, use a patient’s dendritic cells, deliver an antigen directly, or carry messenger RNA (mRNA) that instructs cells to produce a selected protein. In a personalized mRNA approach, sequencing of tumor tissue is used to identify mutations that could generate neoantigens.
  3. Let antigen-presenting cells process it. Dendritic cells and other antigen-presenting cells take up the material. With mRNA, cells that absorb the instructions make the encoded protein; the resulting antigen can then be processed for presentation to T cells.
  4. Activate T cells. Dendritic cells display the antigen in a context that can activate antigen-specific T cells. The aim is to expand or strengthen T cells able to recognize the target rather than stimulate an indiscriminate response against all tissue.
  5. Find tumor cells carrying the target. Activated T cells circulate and inspect cells. When their receptors recognize the vaccine-trained antigen on a cancer cell, they can release toxic signals that kill that cell.
  6. Assess the actual clinical effect. Doctors must look beyond an immune marker. Imaging, symptoms, laboratory findings and longer-term outcomes determine whether the treatment is helping the cancer. The presence of antigen-specific T cells alone is not proof of tumor control.

Treatment vaccines are not the same as HPV or hepatitis B vaccines

The word “cancer vaccine” covers two fundamentally different purposes. A treatment vaccine is given to a person who already has cancer and is designed to direct immunity against antigens on cancer cells. A prevention vaccine protects against an infection that can cause cancer years later.

Type What it targets When it is given What it does not do
Treatment vaccine Antigens or mutations associated with an existing tumor After cancer has been diagnosed, in a defined disease setting It is not guaranteed to shrink every tumor or work for every cancer type
HPV vaccine Human papillomavirus types that can cause cancer Before infection, as a preventive measure It does not treat an established HPV infection or an existing HPV-related tumor; see the NCI HPV vaccine fact sheet
Hepatitis B vaccine Hepatitis B virus, which can contribute to liver-cancer risk Before infection, as prevention It is not a therapeutic vaccine for liver cancer

NCI explains the treatment-versus-prevention distinction in its cancer treatment vaccine overview. Calling an HPV vaccine a treatment for an HPV-related cancer confuses prevention of a cancer-causing infection with treatment of a tumor that already exists.

Major treatment-vaccine platforms and their clinical status

Platforms differ in what they deliver, how they stimulate immunity and how much human evidence exists. The following examples should not be read as a ranking.

Approach Antigen source and delivery Clinical position described by NCI Key limitation
Patient’s own tumor cells Material made from an individual’s tumor One category of treatment-vaccine approach Manufacturing and target selection are patient-specific; broad efficacy is not established
Shared tumor-associated antigens An antigen found across some tumors of the same type Many such approaches remain experimental The target may be absent or expressed unevenly among patients and tumor cells
Dendritic-cell vaccine: sipuleucel-T A patient’s immune cells are prepared outside the body to present a prostate-cancer-related antigen and then returned Approved for some men with advanced prostate cancer that has spread, causes few or no symptoms and no longer responds to hormone treatment Its indication is narrow; it is not a general vaccine for all cancers
Oncolytic-virus therapy: talimogene laherparepvec (T-VEC, Imlygic) A modified herpes simplex virus type 1 is injected into a tumor Used for some people with melanoma that returns after surgery and cannot be removed by further surgery It requires direct tumor injection and has specific disease and eligibility limits
Personalized mRNA vaccine mRNA encodes neoantigens selected from a patient’s tumor mutations Investigational; NCI’s 2022 account described individualized manufacturing taking roughly one to two months after tissue collection Early studies do not establish a broadly effective or approved treatment

How sipuleucel-T illustrates dendritic-cell training

Sipuleucel-T is made for an individual patient rather than supplied as a one-size-fits-all injection. Immune cells are collected, prepared to present a prostate-cancer antigen and infused back into the patient. NCI lists it for a specific group of men with metastatic, minimally symptomatic disease that has stopped responding to hormone treatment. That approved use should not be generalized to earlier-stage prostate cancer or to other tumors.

How T-VEC adds an in-tumor immune trigger

T-VEC is based on herpes simplex virus type 1 and is injected directly into a tumor. The virus replicates in tumor cells, causing them to burst. The dying cells release viral particles and other substances that can alert the immune system to antigens beyond the injected lesion. NCI describes its use for some people with melanoma that has returned after surgery and cannot be surgically removed. It is sometimes called a treatment vaccine because it combines direct tumor destruction with immune stimulation, although it is an oncolytic-virus therapy rather than a conventional antigen shot.

What mRNA cancer vaccines add

mRNA is an instruction molecule, not the antigen itself. An mRNA vaccine carries a temporary code for a selected protein. Cells that take up the code make that protein, and antigen-presenting cells can display the resulting antigen to T cells. This lets developers encode several targets or rapidly redesign a vaccine when a patient’s tumor mutations are known.

Personalized candidates focus on neoantigens: abnormal proteins created by mutations unique to a tumor. Julie Bauman, M.D., deputy director of the University of Arizona Cancer Center, told NCI that “Personalized cancer vaccines may teach the immune system how cancer cells are different from the rest of the body.” The practical challenge is speed and precision. In an NCI 2022 account of an investigational approach, manufacturing took roughly one to two months after tumor tissue was collected.

That account described an early head-and-neck cancer study in which two of the first 10 participants had complete responses and five had tumor shrinkage. The cohort was very small, and those figures are an early observation—not a response rate that can be applied to other patients, cancers or current products.

An NCI report dated April 26, 2023 compared three experimental mRNA vaccines aimed at the HPV-16 E7 protein in a mouse model. A single dose eradicated tumors in that model, and all three vaccines stimulated antigen-specific CD8-positive T cells. The report also noted that approaches successful in mice have not always succeeded in later human trials, and said the mRNA vaccines discussed then were not FDA-approved. Mouse tumor eradication is therefore evidence for a biological concept, not a demonstrated human outcome.

How to judge evidence without overinterpreting it

Evidence you may see What it can establish What it cannot establish by itself
Antigen-specific T cells in blood or tissue The vaccine engaged part of the intended immune pathway That tumors will shrink or survival will improve
Animal tumor eradication That a platform can work in a particular controlled model Safety, dosing or efficacy in people
Responses in a small early human cohort That the approach merits further study and may help some participants A reliable response rate for the wider population
Randomized clinical outcomes in the indicated cancer setting Whether benefits and risks are established for that use That the same vaccine works in a different cancer, stage or treatment line

John Schiller, Ph.D., of NCI’s Center for Cancer Research, cautioned about the mouse findings: “Looking at immune responses to mRNA vaccines did not necessarily predict how well the vaccines worked to get rid of the tumor.” Researchers also did not fully understand every component of the immune response associated with tumor eradication in that model. This is why platforms should not be ranked from an animal comparison or a small, uncontrolled cohort alone.

Side effects and who can receive a vaccine

Safety depends on the platform, dose, injection site, cancer and the patient’s other treatments. NCI lists flu-like symptoms among possible effects of treatment vaccines. Sipuleucel-T carries a risk of stroke. T-VEC can cause tumor lysis syndrome and herpesvirus-related complications, in addition to local and systemic effects. These risks are not interchangeable: a side-effect profile for one platform cannot be used to predict another’s.

Eligibility also depends on disease extent, symptoms, prior treatment, organ function, immune status and whether the selected antigen is present. An oncology team must match the therapy to its approved indication or, for an investigational vaccine, to the clinical-trial protocol. Anyone considering a trial should verify current recruitment and eligibility in an official trial registry and with the treating team, because listings change.

Where the field is heading

Minutes from an NCI Clinical Trials and Translational Research Advisory Committee meeting dated July 15, 2026 describe a proposed National Cancer Vaccine Taskforce Initiative. The planning framework includes evaluating vaccine platforms, discovering antigens, monitoring immune responses, building basket trials that can test a strategy across cancers and creating shared infrastructure. Proposed exploration of adjuvant vaccine approaches includes pancreatic, liver, colorectal and fusion-positive pediatric cancers, among other indications.

These minutes describe national research planning and demonstration studies, not completed clinical efficacy findings or treatments available outside an appropriate indication or trial. The proposed framework reflects a move toward coordinated, biomarker-informed studies rather than a claim that one vaccine will work across all tumors.

Questions to discuss with an oncology team

  • What antigen is this vaccine targeting, and is it present in my tumor?
  • Is the treatment approved for my exact cancer type, stage and treatment history, or is it investigational?
  • What evidence comes from clinical outcomes in people, rather than immune markers or animal models?
  • How is the vaccine made, and how long does preparation take after tissue collection?
  • What side effects require urgent attention, and how will they be monitored?
  • Could participation affect other treatments or eligibility for another trial?

The central idea is straightforward but narrow: cancer treatment vaccines make selected tumor features more visible to the immune system so T cells can recognize cells carrying those features. Whether that training translates into meaningful tumor control must be demonstrated separately for each vaccine, cancer and clinical setting.
