It measures selected biological features and runs them through a model designed to estimate a particular target—such as chronological age, a health-related profile, or the pace of aging. The result can be informative, but it is not a diagnosis or a precise forecast of how long you will live.
What a biological-age test measures
“Biological age” is not a single directly observable quantity with an agreed gold-standard measurement. A test measures particular features and applies an algorithm to estimate a target. That target—and the data used to calculate it—vary between tests.

DNA-methylation clocks
Many tests use DNA methylation: chemical marks measured at selected locations on DNA called CpG sites. A model combines those measurements using assigned weights. Depending on how the model was trained, its output may estimate chronological age, a health-related phenotype, or another outcome. It is an estimate from selected signals, not a direct reading of a universal biological-age value. The 2025 review From Population Science to the Clinic? Limits of Epigenetic Clocks as Personal Biomarkers discusses these distinctions and the limits of applying such measures to individuals.

Other approaches
Not every biological-age test uses DNA. Some approaches combine physiological measurements or biomarkers. Even when two products both use the phrase “biological age,” they may measure different inputs and use models built for different purposes.
#1 Best Overall
- Estimates Your Dog's Age And Birthday From DNA: Uses DNA methylation, a biological clock in your dog's genes, to give you an estimated age and birthday. 77% of dogs have a true birthday within 12 months of their estimated birthday
- Built For Rescue And Unknown-History Dogs: If you adopted a dog with no papers and no known birthday, this is made for you. Best for dogs past the puppy stage, when age is hardest to judge by looks alone
- Includes Life-Stage Appropriate Care Information: Your results come with life stage information developed by veterinarians. A dog's social, behavioral, and health needs change with age, and knowing their estimated life stage helps you keep up
- Simple At-Home Cheek Swab With 2-Way Shipping: Swab your dog's cheek, register online, and mail it back with the prepaid label in the box. No vet visit or blood draw needed. Results arrive 4-5 weeks after the lab receives your sample
- From Embark, The Most Recommended Dog DNA Test: This test estimates age only. For breed and ancestry see Embark Breed ID, for breed plus over 270 genetic health risks see Breed & Health
Why two tests can give different results
Two scores are not necessarily competing readings of the same thing. One model may be trained to estimate chronological age; another may be trained on health-related outcomes; a third may estimate the rate of aging. The name of the clock and the outcome it was trained to predict matter more than the broad label on the report.
Age in years versus pace of aging
DunedinPACE is a pace measure, not an age estimate in years. Its authors describe it as “a DNA-methylation estimate of the Pace of Aging, the ongoing rate of decline in system integrity.” The 2022 study developed the measure by modeling within-person changes in organ-system integrity over time and distilling those changes into a single-time-point methylation measure: DunedinPACE, a DNA methylation biomarker of the pace of aging. A pace score and an age-in-years score answer different questions and should not be treated as interchangeable.

Sample and processing differences
Blood, saliva, and cheek-cell samples do not have identical methylation profiles, and some clocks are tissue-specific. Results can also be affected by cell composition, sample collection and storage, laboratory processing, data preprocessing, assay platform, and biological fluctuation. A model’s fit to a particular tissue, population, and assay matters when interpreting its output. The 2025 review Cross-tissue comparison of epigenetic aging clocks in humans examines why tissue source is an important consideration.
How accurate are biological-age tests?
There is no single accuracy figure that applies to every test. Accuracy depends on what the model was built to estimate, the sample and measurement method, and the population in which it was evaluated. The 2025 review reports median absolute errors of 3.6 years or higher for first-generation epigenetic age estimates in studies it cites. That finding is not a universal error range for every clock or consumer test.

Technical repeatability is also different from clinical usefulness. In the original DunedinPACE study, test-retest reliability was ICC 0.96 (95% CI 0.93–0.98) in one replicate dataset. The study also reported ICC 0.97 (0.94–0.98) in an EPIC-array replicate dataset and 0.87 (0.82–0.90) when comparing 450K and EPIC arrays. These are repeatability results for that measure and those setups; they do not show that every consumer test performs similarly or that a score is useful for making medical decisions.
For individual use, the 2025 review identifies three barriers: unreliable single-timepoint readings, difficulty interpreting change over time, and limited specificity about what a measurement means. A vendor’s precision claim is not, by itself, independent evidence of clinical accuracy.

What a score can—and can’t—tell you
What it may tell you
A score can summarize the measurements included in a particular model, and some clocks have been associated with later health outcomes in research populations. DunedinPACE, for example, was associated with morbidity, disability, and mortality across datasets in its study. Such findings may make a measure useful for population-level research; they do not establish that it can diagnose a condition or determine an individual’s health trajectory.

What it cannot establish on its own
- A diagnosis: A biological-age result alone does not establish that you have a disease or require treatment.
- A precise lifespan forecast: An association between a clock and mortality in a study does not calculate how long a particular person will live.
- The cause of a score: A result does not necessarily explain which underlying biological process produced it or why it differs from another test.
- That changing the score will improve health: An intervention-related score change does not by itself show improved health or longer life. The DunedinPACE authors say that establishing the measure as a surrogate endpoint ultimately requires evidence connecting intervention-induced changes to healthy-lifespan outcomes.
- A treatment threshold: There is no accepted universal gold-standard value for biological age and no universal clinical cutoff at which an epigenetic clock score calls for treatment.
The National Institute on Aging’s 2023 workshop summary says epigenetic age alone “does not provide any information about underlying biological mechanisms” and recommends using it “in conjunction with other biomarkers.” See the National Institute on Aging workshop summary. A test result should not be used to diagnose disease, change prescribed treatment, or replace ordinary clinical evaluation.

How to assess a test report
Before interpreting a number, look for the details that explain what it represents. If the report does not answer these questions, ask the provider rather than assuming scores with similar labels are comparable.
- Which clock or model was used? Find out what it was trained to predict: chronological age, a health-related outcome, or pace of aging.
- What does the output mean? Check whether it is an age estimate in years, a pace measure, or another kind of score, and what reference group is used for comparison.
- What sample was tested? Note whether it came from blood, saliva, cheek cells, or another tissue, and whether the model was validated for that sample type.
- What validation supports it? Look for evidence of repeatability and validation in a relevant population, tissue, and assay. A favorable result on one measure does not establish superiority on all others.
- What can the provider reasonably claim? Distinguish population-level research associations from evidence that a test guides individual diagnosis or treatment.
- How are your sample and data handled? Review the provider’s privacy and data-handling terms.
A 2026 review of consumer-facing clocks and tests, From the lab to lifestyle: epigenetic clocks in personalized aging and health, provides additional context for evaluating these products and their claims.
If you repeat a test
A later score is easiest to compare with an earlier one when the same clock, sample type, laboratory, and platform are used. Even then, a difference may reflect measurement variation or biological fluctuation rather than a meaningful change. Interpreting change also requires enough time and context; the reviewed literature does not establish a universal retesting interval. There is no clinical cutoff that turns a change into a treatment instruction.
