How APOE4 Is Inherited and What Genetic Testing Can Tell You

APOE ε4 is a version of the APOE gene associated with a higher risk of late-onset Alzheimer’s disease. You inherit one APOE copy from each biological parent, so your result is a combination of two alleles. An APOE test can identify that combination, but it cannot diagnose Alzheimer’s, establish that you will develop it, or reliably predict when symptoms would begin.

The most important distinction is between inheriting a risk factor and inheriting a disease-causing mutation. APOE ε4 changes statistical risk; it is not the rare, autosomal-dominant cause of some early-onset familial Alzheimer’s cases.

What APOE ε4 is—and what it is not

APOE is a gene involved in transporting fats and cholesterol. Its common alleles are called ε2, ε3 and ε4. Everyone has two APOE alleles, although the two copies may be the same or different.

APOE ε4 is associated with increased susceptibility to late-onset Alzheimer’s disease. One ε4 allele is generally associated with more risk than some other genotypes, and two ε4 alleles are associated with a greater risk than one. Those are population-level associations, not a diagnosis. Some people with ε4 never develop Alzheimer’s, and some people diagnosed with Alzheimer’s do not carry ε4.

MedlinePlus Genetics summarizes the distinction this way: “people with the APOE e4 allele inherit an increased risk of developing Alzheimer’s disease, not the disease itself.” The inheritance pattern of late-onset Alzheimer’s remains uncertain because many genes, health conditions and environmental factors contribute to it.

How APOE4 is inherited from your parents

You receive one APOE allele from each biological parent

During reproduction, each biological parent passes one of their two APOE copies to a child. The child’s genotype is the resulting pair. A parent with one ε4 allele can pass either that allele or the parent’s other allele. A parent with two ε4 alleles has ε4 available in both copies, so either transmitted copy is ε4.

Whether a child inherits ε4 is therefore a matter of which allele is transmitted—not a direct prediction from whether a parent has Alzheimer’s. A parent can carry ε4 without developing the disease, and a parent without ε4 can still have Alzheimer’s for other reasons.

Common genotype combinations

APOE genotype What the genotype means What it does not establish
ε2/ε2, ε2/ε3 or ε2/ε4 Contains one or two ε2 alleles, with the exact combination affecting the associated population risk estimate. It does not rule out Alzheimer’s or predict an individual outcome.
ε3/ε3 Two ε3 alleles; often used as a reference genotype in studies. It is not proof of immunity or a guarantee of normal aging.
ε3/ε4 One ε4 allele and one ε3 allele; associated with higher risk than the reference genotype in many studies. It does not mean Alzheimer’s is inevitable or indicate an age of onset.
ε4/ε4 Two ε4 alleles; associated with a greater risk than one ε4 allele in many study populations. It is still a susceptibility result, not a diagnosis or certainty.

The table describes allele combinations, not a complete family-probability calculator. The chance of a particular child receiving a particular combination depends on both parents’ genotypes, and a child’s eventual health also depends on factors beyond APOE.

Is APOE4 inherited from your mother or father?

It can come from either biological parent. A laboratory result normally cannot tell whether an ε4 allele came from the maternal or paternal side unless additional family testing or phased genetic analysis is performed. If a parent has one ε4 allele, that parent may transmit either of their two APOE copies.

If one parent has APOE4, will you have it?

Not necessarily. A parent with one ε4 allele has another APOE allele that may be passed instead. Even when a child inherits ε4, the result indicates susceptibility rather than a certain future diagnosis. Testing a parent’s status is not a substitute for testing the individual, and a family history of Alzheimer’s does not reveal the child’s genotype by itself.

APOE4 is different from rare familial Alzheimer’s mutations

Some early-onset familial Alzheimer’s disease is caused by rare disease-causing variants in APP, PSEN1 or PSEN2. When a biological parent carries one of those pathogenic variants, NIH states that each child has a 50% chance of inheriting it. That figure describes transmission of a specific rare variant in an autosomal-dominant condition.

Do not apply that 50% figure to APOE ε4. APOE ε4 is a common risk allele, not an autosomal-dominant Alzheimer’s mutation. Nor does carrying an APP, PSEN1 or PSEN2 variant provide a universal age-of-onset schedule; clinical outcomes and timing still require specialist interpretation.

If several relatives developed dementia unusually early, or if a known pathogenic variant is present in the family, ask a medical geneticist or genetic counselor about a familial-Alzheimer’s evaluation. An APOE-only test may not address that question.

What an APOE genetic test actually examines

The two variants used to assign ε2, ε3 and ε4

The FDA’s 2019 review describes an APOE assay that tests two single-nucleotide polymorphisms (SNPs), rs429358 and rs7412. The combination of bases at those positions is used to determine whether a person has ε2, ε3 or ε4 and whether the genotype is, for example, ε3/ε3, ε3/ε4 or ε4/ε4.

A report may list the raw SNP calls, the inferred APOE genotype and a category such as lower, reference or increased relative risk. Read the laboratory’s interpretation alongside its reference population and test limitations. Different services may report the same biology in different formats.

Genotype is not diagnosis

An APOE result does not show whether amyloid plaques or other Alzheimer’s-related changes are present. It does not replace a cognitive evaluation, neurological examination, brain imaging or disease-specific biomarker testing when those are clinically indicated. It also cannot reliably tell whether symptoms will occur or the age at which they might start.

How to interpret APOE risk numbers

Risk figures are easy to overread because studies may report relative risk, odds ratios or likelihood ratios rather than an individual’s absolute lifetime probability.

In the FDA review, cited studies of people of European descent reported likelihood ratios of 1.9 for ε3/ε4 and 8.2 for ε4/ε4, using the study’s reference genotype and design. These are study-derived likelihood ratios, not universal multipliers and not the probability that a particular person will develop Alzheimer’s. The cited estimates differ for other ancestry groupings.

To translate any relative measure into an absolute probability, a clinician would need an appropriate baseline incidence plus factors such as age, sex, family history, ancestry, medical history and the specific outcome being estimated. Without those inputs, multiplying a personal risk by 1.9 or 8.2 produces a misleading number.

Number What it refers to How to use it
50% NIH’s figure for a child inheriting a rare disease-causing APP, PSEN1 or PSEN2 variant from a biological parent who carries it. Do not use it as an APOE ε4 transmission or Alzheimer’s-risk figure.
1.9 FDA-reviewed, study-derived likelihood ratio for ε3/ε4 in cited European-descent data. It is not an individual absolute probability; ancestry and reference assumptions matter.
8.2 FDA-reviewed, study-derived likelihood ratio for ε4/ε4 in cited European-descent data. It is not a guarantee, diagnosis or universal risk multiplier.

Why ancestry and test design affect interpretation

The size of APOE’s association with Alzheimer’s differs among ancestry groups in published studies. A result interpreted with estimates from one population may not have the same predictive performance in another. Ask which population supplied the laboratory’s reference data and whether the provider explains uncertainty for your ancestry.

Many direct-to-consumer tests examine only selected markers. They may identify APOE status without testing every variant associated with Alzheimer’s, without assessing rare familial mutations and without incorporating medical or environmental information. Raw genotype files can also be difficult to interpret: a third-party report may misclassify variants, use an unsuitable reference population or present relative risk as if it were a personal forecast.

Should you get an APOE test?

Testing is a personal decision. Consider what you would do with an increased-risk, average-risk or uncertain result before ordering it. A result can be useful for discussing family history, planning medical conversations or deciding whether to pursue formal evaluation, but it can also cause anxiety or confusion when no clear action follows.

Reasons someone may consider testing

  • You want your clinician to know your APOE status when discussing Alzheimer’s risk or eligibility for a research or clinical program.
  • A family history has prompted questions that require professional genetic interpretation.
  • You understand that the result changes susceptibility estimates but does not predict your individual future.

Reasons to pause

  • You are expecting a definitive answer about whether you will develop Alzheimer’s or your age of onset.
  • You would make major medical or financial decisions from a single genotype result.
  • You have not reviewed how the company stores, uses or shares genetic data.

Use counseling before and after testing

NIH recommends genetic counseling around APOE testing, and MedlinePlus says people whose direct-to-consumer results indicate increased risk may benefit from speaking with a genetic counselor or another healthcare professional. Counseling can clarify what the assay tested, place the result in your family and medical context, discuss emotional effects and identify whether a broader evaluation is appropriate.

A practical testing and results checklist

  1. Define the question. Decide whether you are asking about common late-onset susceptibility, a suspected early-onset familial condition or something else.
  2. Review the assay. Confirm that the provider explains rs429358 and rs7412, the APOE genotype it reports, its reference population and its analytical limitations.
  3. Check professional oversight. Find out whether a qualified clinician or genetic counselor is available to explain the result.
  4. Read privacy terms before submitting DNA. Look for data retention, research use, law-enforcement disclosure, deletion options and whether raw data can be downloaded or shared.
  5. Interpret the result in context. Bring the report, family history and relevant medical information to a healthcare professional rather than relying on an isolated online score.
  6. Do not act on a third-party interpretation alone. If you upload raw data elsewhere, verify the provider’s qualifications and understand that an automated interpretation may omit important variants or context.

Other factors that shape Alzheimer’s risk

APOE is only one part of risk. NIH and MedlinePlus identify genetic, lifestyle and environmental contributors, including physical inactivity, an unhealthy diet, alcohol misuse, smoking, poor sleep and social isolation. These factors are not a promise that lifestyle changes can erase inherited susceptibility, but they are relevant health issues to discuss with a clinician regardless of genotype.

An increased-risk APOE result should not lead to unproven supplements, devices or drastic restrictions. A reduced-risk result should not be treated as permission to ignore symptoms or routine preventive care. Memory or thinking changes warrant medical evaluation whether or not APOE ε4 is present.

What to remember about an APOE4 result

  • Each biological parent contributes one APOE allele, and either parent can be the source of ε4.
  • One ε4 allele is associated with increased late-onset Alzheimer’s risk; two are associated with greater risk in many study populations.
  • APOE ε4 is a susceptibility allele, unlike rare autosomal-dominant disease-causing variants in APP, PSEN1 or PSEN2.
  • A test reports genotype and statistical association, not a diagnosis or a dependable prediction of whether or when Alzheimer’s will occur.
  • Risk estimates depend on ancestry, reference genotype, study design and other personal factors.
  • Professional counseling and careful privacy review are as important as the laboratory result itself.
