How Antidepressants Can Interact With Cancer Drugs and Other Medications

Yes. Some antidepressants can change how cancer medicines are processed, while combinations with other medicines can increase risks such as sedation or heart-rhythm effects. If you have cancer or take tamoxifen, give your oncology team or pharmacist a complete list of everything you use—including over-the-counter medicines, patches, vitamins, herbs, and supplements—before starting or changing a medicine. Do not stop or switch an antidepressant on your own.

Can antidepressants interact with cancer drugs?

They can. An interaction may change the amount of a medicine in the body, affect how well a cancer drug works, or add to side effects. The interaction depends on the particular medicines and the person taking them; it does not mean that every antidepressant is incompatible with cancer treatment.

The National Cancer Institute (NCI) warns that some anticancer drugs may not mix safely with certain antidepressants, foods, herbs, or nutritional supplements. Its patient guidance advises people to tell their health care providers about all medicines and supplements they take, including patches. That matters because a cancer regimen may include several medicines, and a product that seems unrelated to cancer treatment can still affect medication metabolism or side effects.

There are two useful ways to think about these combinations. A metabolism interaction can alter how quickly the body processes a medicine, potentially changing its concentration or active metabolites. A combined-effect interaction occurs when medicines have overlapping effects, such as increasing sedation or contributing to heart-rhythm risk. A medication review should consider both, rather than looking only for drugs with the same purpose.

Interactions are specific to a person’s entire regimen. The same antidepressant may raise different questions depending on which cancer medicine is used, other prescriptions, and health conditions. Even foods, herbs, or supplements can be relevant, so mention them rather than assuming that only prescription drugs matter.

How can an antidepressant affect tamoxifen?

Tamoxifen is partly converted by the liver enzyme CYP2D6 into endoxifen, one of its active metabolites. Some antidepressants inhibit CYP2D6, which can reduce endoxifen exposure. This is a reason to review the combination with the oncology team—not a reason to stop either medicine without medical advice.

The NCI’s clinician guidance describes a prospective trial in which taking paroxetine with tamoxifen reduced endoxifen concentrations. It also summarizes a prospective observational study of 80 women starting adjuvant tamoxifen: CYP2D6 gene variants and concurrent use of CYP2D6-inhibiting selective serotonin reuptake inhibitors (SSRIs) were associated with lower endoxifen levels. These findings concern drug levels; they do not, by themselves, predict what will happen to an individual patient.

NCI also summarizes studies that examined cancer outcomes. One study followed more than 1,300 women for a median of 6.3 years and reported worse recurrence- and survival-related outcomes among women with lower CYP2D6 activity. A retrospective Ontario cohort of more than 2,400 women reported an increased risk of death associated with overlapping paroxetine and tamoxifen use, with the reported risk related to the duration of overlap. These are observational findings, not proof that an interaction will cause recurrence or death in every person. NCI says the clinical implications have not been fully elucidated and that further study is needed.

When discussing tamoxifen, make sure the team knows the exact antidepressant, dose if known, and dates of use. NCI’s patient information lists sertraline, citalopram, and venlafaxine among medicines a doctor may recommend discussing for people taking tamoxifen. That list is a prompt for an individual review—not a guarantee that any one option is suitable or safe for every patient. NCI’s clinician PDQ identifies potent CYP2D6 inhibitors, including paroxetine and duloxetine, as concerns with tamoxifen.

It may help to ask the team to review not only the antidepressant but also any other medicines that could affect tamoxifen or CYP2D6. NCI’s breast cancer hormone therapy guidance names quinidine, diphenhydramine, and cimetidine as medicines that could interfere with tamoxifen. Share both current and recently changed medicines, and ask whether the length of any overlap matters to your individual plan.

Which medication effects should the cancer team review?

Antidepressants differ in their effects on drug-metabolizing enzymes, heart rhythm, and alertness. Even medicines in the same class are not interchangeable from an interaction standpoint. The examples below describe concerns identified in NCI guidance; they are not prescribing recommendations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Medicine or concern What NCI guidance says What to raise with the care team
Paroxetine A significant CYP2D6 inhibitor; coadministration has been shown to reduce endoxifen concentrations. Tell the prescriber and oncology team if you take it with tamoxifen, including how long the medicines have overlapped.
Fluoxetine A significant CYP2D6 inhibitor. Ask whether CYP2D6 effects matter for your specific cancer medicines.
Duloxetine Listed by NCI’s clinician PDQ as a potent CYP2D6 inhibitor of concern with tamoxifen. Include it in a tamoxifen medication review; do not change it without the prescriber.
Sertraline CYP2D6 inhibition is dose-dependent; NCI’s patient page lists it among medicines a doctor may recommend discussing with tamoxifen. Share the dose and ask the team to assess it in the context of the whole regimen.
Fluvoxamine A significant CYP1A2 and CYP3A4 inhibitor. Ask whether those enzyme pathways are relevant to any of your current cancer or other medicines.
Citalopram May prolong the QTc interval, an electrical measure of heart rhythm; NCI’s patient page lists it among medicines to discuss with tamoxifen. Raise cardiac conditions and other medicines that may affect heart-rhythm risk.
Mirtazapine Can be highly sedating; NCI cautions that it may be undesirable with sedating medicines such as opioids or benzodiazepines. Tell the clinician about opioid, benzodiazepine, or other sedating medicine use and any concerns about alertness.

Enzyme effects are not limited to the tamoxifen example. NCI identifies fluvoxamine as a significant CYP1A2 and CYP3A4 inhibitor, while fluoxetine and paroxetine are significant CYP2D6 inhibitors; sertraline’s CYP2D6 inhibition is dose-dependent. These differences are one reason a broad label such as “SSRI” is not enough for a medication check. Ask whether a pathway matters for the specific cancer drug and other medicines you take.

Other effects may matter even when metabolism is not the main concern. NCI notes that citalopram can prolong QTc, so heart conditions and other medicines that may affect cardiac risk are worth mentioning. A sedating antidepressant such as mirtazapine may be undesirable alongside opioids or benzodiazepines. Tell the reviewer about drowsiness, falls, or difficulty staying alert, especially if several medicines can cause sedation.

NCI’s breast cancer hormone therapy information also names quinidine, diphenhydramine, and cimetidine as medicines that could interfere with tamoxifen. Mention these, if applicable, along with other prescription and nonprescription products. The NCI page was updated September 23, 2026; the examples still need to be checked against your own medication list and treatment plan.

Why do cancer type and treatment matter?

The same antidepressant combination may not mean the same thing for every patient. NCI notes that the cancer and its treatment can affect absorption or metabolism. Cancers involving the gastrointestinal tract, liver, or kidneys can also influence antidepressant selection. The medicines used to treat the cancer, other health conditions, and the person’s prior response and tolerability all contribute to the decision.

This is why a general list of “safe” antidepressants cannot replace a review of the full regimen. The right questions include whether an enzyme pathway is relevant to a particular cancer medicine, whether two drugs add to a side effect, and whether an option addresses the patient’s symptoms while remaining suitable for their overall treatment. The available NCI guidance does not establish one best antidepressant for everyone with cancer.

If the team is comparing options, the discussion can include the likely interaction pathway—especially CYP2D6 for tamoxifen—other enzyme effects relevant to the regimen, QTc or cardiovascular concerns, and sedation with medicines such as opioids or benzodiazepines. Symptom goals also matter: a medicine may be considered for depression, hot flashes, neuropathy, or more than one concern. A person’s previous benefit, side effects, and ability to tolerate treatment are part of the comparison too. These factors call for individualized discussion, not a self-selected switch based on one feature.

What medicines should I tell my cancer team about?

Make one current list and include products even if you take them only occasionally. NCI specifically advises sharing all medicines and supplements, including patches. If you can, record the strength or dose and how often you use each item; bring the packaging or a photo if you are unsure of a name.

  • Prescription medicines, including antidepressants and medicines prescribed by clinicians outside the oncology team.
  • Over-the-counter medicines, including products for pain, allergies, sleep, or stomach symptoms.
  • Skin patches and other non-pill medicines.
  • Vitamins, herbal products, and nutritional supplements.
  • Medicines used only as needed, along with how often you actually take them.

Record the names of the clinicians who prescribe your cancer and mental-health medicines. A medication list notebook can help keep the information together, but it cannot identify or prevent an interaction; the care team or pharmacist must review the combination. A phone list or a current photo of medicine labels can also help you keep details available when you speak with the team.

How to get a medication review

  1. Assemble the list. Include every prescription, nonprescription medicine, patch, vitamin, herb, and supplement, with doses and frequency when available.
  2. Contact the oncology team or a pharmacist. Ask them to review the full list alongside the current cancer treatment, rather than checking the antidepressant in isolation.
  3. Name the specific concern. If you take tamoxifen, say so explicitly and ask whether any medicine on the list could affect CYP2D6 or tamoxifen. Mention paroxetine, fluoxetine, duloxetine, sertraline, and any other antidepressant by its exact name.
  4. Discuss overlapping effects. Tell the reviewer about heart conditions and medicines that may affect cardiac risk if citalopram is involved, and about opioids or benzodiazepines if a sedating antidepressant such as mirtazapine is involved.
  5. Agree on a plan before making a change. Ask which clinician will coordinate any adjustment and what to do if a new medicine is proposed. Do not stop, start, or switch an antidepressant on your own.

Review the list again when a cancer medicine or antidepressant is started, stopped, or changed, and when another clinician adds a medicine. NCI advises discussing current medicines before starting tamoxifen. Keeping the oncology team and mental-health prescriber informed helps them assess the same combination rather than making decisions from incomplete lists.

What the evidence does—and does not—tell you

A measured change in endoxifen concentration is not the same as a proven outcome for an individual. The outcome studies summarized by NCI raise a concern about tamoxifen and CYP2D6 activity, but observational results cannot establish that every overlapping antidepressant causes a worse cancer outcome. NCI says the clinical implications remain incompletely understood. For a patient, the practical response is a clinician-led review of the actual medicines, not a self-directed change based on a single study or a broad online list.

Frequently Asked Questions

Can antidepressants interact with cancer drugs?

Yes. Some antidepressants can affect the enzymes that process cancer medicines, and combinations can also add to effects such as sedation or heart-rhythm risk. The significance depends on your specific medicines and health situation, so ask your oncology team or pharmacist to review your full medication and supplement list.

Can an antidepressant affect tamoxifen?

Some antidepressants inhibit CYP2D6, an enzyme that helps convert tamoxifen into the active metabolite endoxifen. NCI identifies paroxetine and duloxetine as potent CYP2D6 inhibitors of concern and also notes other medicines that may warrant discussion. Do not stop or change either medicine without speaking with your prescriber and oncology team.

What medicines should I tell my cancer team about?

Tell them about all prescription and over-the-counter medicines, patches, vitamins, herbs, and supplements—including medicines you use only occasionally. Include doses and frequency if you know them, and name the clinicians who prescribe your cancer and mental-health medicines.

Should I stop my antidepressant if I take tamoxifen?

No—do not stop, start, or switch an antidepressant on your own. Drug-level and observational outcome findings do not determine what will happen to an individual. Contact your oncology team, prescribing clinician, or pharmacist to review the combination and agree on a plan.

Is there one antidepressant that is safest for everyone receiving cancer treatment?

No single best option is established for everyone. A clinician may consider enzyme effects, the particular cancer regimen, heart-rhythm and sedation concerns, symptom goals, prior response, tolerability, and health conditions when reviewing options.

The Bottom Line

Some antidepressants can affect cancer-drug metabolism, while other combinations may increase sedation or heart-rhythm concerns. If you take tamoxifen, make sure the team knows about every antidepressant and other medicine that could affect CYP2D6 or tamoxifen. Keep a complete list of prescriptions, nonprescription products, patches, vitamins, herbs, and supplements, and review it with your oncology team or pharmacist before a medicine is started or changed. Never change an antidepressant on your own.
