AI-powered ECG screening is a clinical decision-support test. Machine-learning software examines patterns in an electrocardiogram (ECG) and estimates whether they are associated with a defined target, such as a reduced left-ventricular ejection fraction (LVEF). A positive result can prompt an echocardiogram or other evaluation; it is not, by itself, a diagnosis of heart failure.
The clearest regulatory example is prescription software intended to screen adults who are already at risk for heart failure. It uses ECG data to flag a possible LVEF of 40% or less. The result must be interpreted with a clinician’s assessment, and a negative result does not eliminate the need for further testing when the person’s history or symptoms remain concerning.

What AI ECG screening is
An ECG records the heart’s electrical activity through electrodes placed on the skin. Conventional ECG interpretation looks for visible electrical patterns, while an AI-ECG system applies a machine-learning model to the digital signal and returns a screening output for a specified condition.
The model does not directly measure the heart’s ejection fraction. Instead, it identifies signal features that, in the population used to develop and validate the software, were associated with reduced pumping function. The FDA classifies this type of product as notification software that uses non-invasive physiological inputs and machine learning to suggest the likelihood of reduced ejection fraction and support referral or diagnostic follow-up (FDA product classification).

Public regulatory documents do not disclose a complete signal-processing pipeline, model architecture, or training data for the cited product. It is therefore not accurate to claim that every AI-ECG uses a particular kind of neural network or that the software calculates LVEF directly.
How does AI ECG screening work?
1. A clinician obtains an ECG
The cited low-EF use case accepts ECG data, typically from a clinical 12-lead recording. The electrodes capture voltage changes over a short recording, and the ECG device produces a digital tracing that can be analyzed by compatible clinical software. The reviewed indication is for prescription use; it does not establish that a general-purpose home monitor can run the algorithm.
2. The software checks the signal and analyzes patterns
After receiving the ECG, the software processes the signal and applies its trained model. The computation is intended to find combinations of waveform, timing, and other signal characteristics that correlate with the target condition. The software’s output is a likelihood or screening classification, not an image of the heart and not a replacement for an echocardiographic measurement.
3. The result appears in a clinical workflow
A clinician may see a positive or negative screening result alongside the routine ECG. In a primary-care study, AI results were made available as decision support during ordinary care. That design tests whether the information changes referral and diagnosis behavior; it does not show that software can replace a physical examination, echocardiography, or cardiology review.
4. The clinician decides what happens next
A positive result can lead to a focused history and examination, laboratory work, an echocardiogram, or another non-invasive test. The appropriate next step depends on symptoms, previous disease, medications, examination findings, and local protocols. The algorithm is one input to that decision rather than an autonomous diagnosis.
What can an AI ECG detect?
The best-supported target: possible low ejection fraction
The FDA-labeled example screens adults at risk for heart failure for a left-ventricular ejection fraction (LVEF) of 40% or less. LVEF is the percentage of blood the left ventricle ejects with each contraction. A low value can occur in heart failure with reduced ejection fraction and in several other cardiac conditions, but an ECG screening result cannot establish the value on its own.

The same labeling says the software is not intended for ECGs with a paced rhythm. A paced tracing may not match the data characteristics for which the model was evaluated, so a result from that type of ECG should not be treated as covered by the indication. Read the complete FDA indications-for-use document for the product’s wording and restrictions (FDA K232699 indications for use).
What the evidence does not establish
The available evidence supports a defined low-EF screening use, not a universal “AI heart detector.” It does not justify saying that one system detects every arrhythmia, heart attack, valve disorder, cardiomyopathy, or structural abnormality. Different algorithms can have different targets, ECG lead requirements, patient exclusions, thresholds, and validation results.
For example, an algorithm trained to flag low LVEF should not automatically be described as a stroke-risk, coronary-blockage, or rhythm-diagnosis tool. Those would require separate intended-use statements and clinical evidence.
Can an ECG detect a weak heart?
An ordinary ECG can provide clues about cardiac disease, but it does not measure pumping strength the way an echocardiogram does. AI screening extends the ECG’s use by looking for subtle signal patterns associated with low LVEF. The result is still indirect: it identifies people who may merit confirmatory evaluation.
Pay attention to the threshold. The FDA indication for the cited software uses LVEF less than or equal to 40%. A major randomized trial used a different endpoint: a new diagnosis of LVEF less than or equal to 50% within 90 days. That trial threshold was chosen for the study and should not be presented as the threshold for every AI-ECG product.

What does a positive AI ECG mean?
A positive result means the software found an ECG pattern associated with its target and that clinical evaluation may be warranted. It does not mean that the patient definitely has heart failure or that LVEF has been measured at a particular value.
The FDA labeling explicitly says the software is not a stand-alone diagnostic device for cardiac conditions and should not be used for patient monitoring. In the label’s words: “Anumana Low Ejection Fraction AI-ECG Algorithm is not intended to be a stand-alone diagnostic device for cardiac conditions, should not be used for monitoring of patients, and should not be used on ECGs with a paced rhythm.”
A clinician normally combines the output with symptoms such as breathlessness or swelling, medical history, examination, prior imaging, and risk factors. If the combined assessment suggests possible dysfunction, an echocardiogram can measure LVEF and assess valves, chambers, wall motion, and other findings.
Can a normal or negative AI ECG rule out low ejection fraction?
No. The FDA instructions state that a negative result should not rule out additional non-invasive evaluation in a patient who remains at high risk. A negative screen lowers concern only within the limits of the tested software, its input quality, and the patient population in which it was evaluated.
Persistent symptoms, a known cardiomyopathy, prior myocardial infarction, exposure to cardiotoxic therapy, or an abnormal examination can justify further work-up even when the algorithm does not flag the ECG. Clinical judgment takes precedence over a single automated output.
What did the randomized clinical trial find?
A pragmatic, cluster-randomized study published in Nature Medicine in 2021 examined whether showing an AI-ECG result to primary-care clinicians affected the rate of newly diagnosed low EF. It enrolled 22,641 adults without prior heart failure across 45 clinics or hospitals.

| Trial measure | Finding | How to interpret it |
|---|---|---|
| Primary endpoint | New diagnosis of LVEF ≤50% within 90 days of an ECG | A study definition, not the FDA threshold for every product |
| New diagnosis with AI decision support | 2.1% | Patients whose clinicians could see the AI result |
| New diagnosis in control care | 1.6% | Routine care without the intervention result |
| Effect estimate | Odds ratio 1.32; 95% confidence interval 1.01–1.61; P=0.007 | More diagnoses were recorded in the intervention workflow |
| Among AI-positive participants | 19.5% versus 14.5% | Diagnosis rates in the study’s positive-result comparison, not sensitivity or an individual’s disease probability |
The trial shows how an AI result may help clinicians identify people who otherwise might not receive timely imaging. It does not prove that the software replaces echocardiography, nor does it provide a universal accuracy figure for all AI-ECG systems. Diagnosis rates also depend on clinician behavior, referral access, follow-up completion, and the population enrolled.

How a clinician should use the result
- Confirm the input. Check that the ECG is technically adequate, uses the required lead configuration, and is not a paced-rhythm tracing excluded by the indication.
- Review the screening output in context. Treat a positive flag as a reason to consider evaluation, not as proof of a low LVEF.
- Assess symptoms and risk. Review heart-failure symptoms, cardiovascular history, medications, examination findings, and relevant comorbidities.
- Choose confirmatory testing. When indicated, arrange echocardiography or another appropriate non-invasive assessment rather than relying on the AI classification.
- Act on the confirmed diagnosis. If imaging or specialist assessment confirms reduced function, manage the underlying condition according to current clinical guidance.
- Do not dismiss ongoing concern after a negative result. Continue evaluation when the clinical picture indicates meaningful risk.
Regulatory status: what FDA clearance does and does not mean
FDA records a 510(k) substantial-equivalence decision dated July 28, 2025, for Anumana’s ECG-AI Low Ejection Fraction (LEF) 12-Lead algorithm (1010) (FDA 510(k) K250652). A 510(k) decision means the device met the applicable premarket requirements for its intended use and was found substantially equivalent to a legally marketed predicate. It is not a claim that the FDA endorses the product, guarantees performance in every setting, or authorizes consumer self-screening.
The FDA describes oversight of AI-enabled medical devices as risk-based, taking intended use and device characteristics into account. Its public resources explain the regulatory framework and list devices that have met applicable requirements (FDA AI-enabled medical-device overview; FDA AI-enabled device list). Authorization is tied to the specific product, version, indication, inputs, and jurisdiction; it should not be generalized to every app marketed as an AI ECG.
How to compare AI-ECG systems responsibly
When a hospital or clinician evaluates products or studies, these questions are more informative than a generic accuracy claim:
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →| Comparison point | What to verify |
|---|---|
| Target and threshold | Does it screen for LVEF ≤40%, ≤50%, an arrhythmia, or another defined endpoint? |
| Population | Which ages, risk groups, care settings, and exclusions were represented? |
| Input | Is a 12-lead ECG required, and are paced rhythms or poor-quality recordings excluded? |
| Validation | Was performance tested in a population and workflow similar to the intended deployment? |
| Performance reporting | Are sensitivity, specificity, calibration, confidence intervals, and the operating threshold reported? |
| Regulatory status | What indication and jurisdiction are covered, and what exactly was cleared or authorized? |
| Follow-up pathway | What test, referral, staffing, and time frame follow a positive result? |
A product with impressive performance in one hospital may not behave identically after a change in ECG hardware, patient mix, prevalence, or workflow. Deployment therefore requires local governance, quality checks, clinician training, and a plan for confirmatory care.

Common pitfalls and safer alternatives
Assuming a flag is a diagnosis
Problem: A positive output is treated as proof of heart failure. Safer approach: Use it to trigger clinical review and confirmatory imaging.
Using a negative result to end the work-up
Problem: A high-risk patient with symptoms is reassured solely because the screen is negative. Safer approach: Continue evaluation when history, examination, or symptoms warrant it.
Ignoring the intended population
Problem: The algorithm is applied to a paced rhythm or to a population outside its labeling. Safer approach: Check the current instructions for use and document exclusions before deployment.
Free tools Windows power users keep installed
One-click scans. No signup required.
Confusing diagnosis rates with accuracy
Problem: The 2.1% versus 1.6% trial result is quoted as sensitivity or as an individual probability. Safer approach: Describe it as the difference in new diagnoses within that randomized workflow.
Linking consumer hardware to clinical software without evidence
Problem: A smartwatch or home ECG is assumed to run a prescription algorithm. Safer approach: Verify the exact device compatibility, lead configuration, software version, prescription requirements, and clinical support pathway.
What patients should ask
- What condition is this algorithm designed to screen for, and what threshold does it use?
- Was my ECG technically suitable, and does any exclusion such as paced rhythm apply?
- If the result is positive, what confirmatory test will I receive and how soon?
- If the result is negative, do my symptoms or risk factors still call for an echocardiogram?
- Who will review the result and explain what it means for my care?
AI-ECG screening is most useful when it closes a gap between routine ECG collection and timely diagnostic evaluation. Its value comes from helping clinicians decide whom to investigate—not from replacing the clinical assessment and imaging that establish what the heart is actually doing.
