AI medical scribes turn visit information into a draft clinical document. Depending on the system, they listen to the patient–clinician conversation, accept a clinician’s dictation, or process an uploaded summary. Speech recognition converts spoken language to text; language models then organize relevant details into a configured note or letter. A clinician must review, correct, and approve the result before it is treated as documentation.
The important distinction is that an AI scribe is a documentation aid, not an autonomous clinician. Capture methods, note formats, EHR connections, recording practices, and regulatory status differ by product and jurisdiction.

What an AI medical scribe does
An AI medical scribe supports documentation after or during a healthcare encounter. It receives conversation or other clinician-provided input, transcribes and organizes that material, and produces a draft such as a progress note, consultation letter, or another structured document. The draft is intended for professional review; it does not replace the clinician’s examination, reasoning, orders, or sign-off.

“Ambient” describes one capture method: the application listens in the background while the patient and clinician talk. Ambient does not mean that every AI scribe records continuously, and “AI scribe” is not a single standardized product category. Other systems rely on dictation or an uploaded written summary.

The five-stage workflow
- Capture or input. The clinician starts an ambient session, dictates after or during the visit, or uploads a summary. Whether a tool records the whole encounter, selected segments, or only a clinician’s voice is product-specific.
- Transcription and organization. Automatic speech recognition converts audio to text. Some services attempt to separate patient and clinician voices, identify speaker roles, recognize clinical terms, and classify portions of the dialogue. Those capabilities should be verified rather than assumed.
- Draft generation. A generative model turns the transcript or supplied text into an output configured for a care setting. It may organize history, findings, assessment, plan, instructions, or correspondence, but there is no single note structure used by all systems.
- Human review. The clinician reads the draft, checks it against the encounter, edits omissions or errors, and validates that the assessment and plan are accurate. The output remains a draft until the responsible professional approves it.
- Record workflow. The approved document may be copied, exported, or transferred into an electronic health record (EHR), depending on the implementation. Integration, supported fields, authentication, and interoperability standards must be checked in the intended clinical environment.
NHS England’s 2025 guidance describes the intended benefit as allowing a caregiver to focus on the patient rather than a keyboard or dictaphone while the conversation becomes an “accurate draft summarisation for review and validation after the visit.” That is a description of the intended workflow, not a guarantee that every generated note will be accurate.
How capture modes differ
| Capture mode | What the system receives | Typical advantage | Questions to verify |
|---|---|---|---|
| Ambient conversation | Audio of the patient–clinician interaction, usually started by the clinician | Less manual typing during the encounter | Is recording continuous? Are speakers separated? How are interruptions, multiple people, and noisy rooms handled? |
| Clinician dictation | A spoken summary recorded by the clinician | More control over what enters the draft and when it is recorded | Can the clinician use a preferred format? Is the recording retained? Does the service support specialty terminology? |
| Uploaded summary | Written text supplied after the encounter | Works without capturing the patient conversation | Which file and text formats are accepted? Does the service create a note from only the supplied material? |
These modes are not interchangeable from a privacy or accuracy perspective. Ambient capture may contain sensitive information that was not intended for the final note. Dictation and uploads can omit context that would have been present in a full conversation. A practice should select the mode that fits its consent process, workflow, and documentation policy.
What happens between audio and a note
Speech recognition
The first transformation is audio-to-text transcription. Clinical vocabulary, accents, background noise, overlapping speech, and medication names can affect the transcript. A transcript can therefore contain errors even before a note is drafted. Some services label likely speakers or assign patient and clinician roles; others may provide only undifferentiated text.
Clinical organization
The service then identifies information that appears relevant to the requested document. It may group statements by topic, extract terms, and classify sections of the conversation. A product might be configured for a particular specialty or letter type, but the reviewed official guidance does not establish one universal template or a universal set of extracted fields.
Generative drafting
A language model produces prose or structured fields from the available input and configuration. The output can be concise or detailed, depending on the chosen format. Generated wording is not evidence that a symptom, diagnosis, medication, or instruction was actually discussed. Clinicians must compare every clinically material statement with the encounter and their own judgment.

Editing and sign-off
The safe endpoint is a reviewed document in the clinician’s normal record process. Review should include names and demographics, dates, symptoms, examination findings, allergies, medications, test results, assessment, plan, follow-up, and any negations such as “no chest pain.” The clinician—not the software—decides what is entered into the official record.
What the patient and clinician should expect in the room
- Introduce the tool. Explain that the conversation may be recorded or processed to create a draft note, who provides the service, and what will happen to the resulting documentation.
- Offer a real choice. Amazon Web Services’ service guidance says customers and downstream integrators should inform patients that recording is used by an AI service provider, that information may be shared with service providers, and that a patient may decline without affecting care. This is implementation guidance, not a universal statement of law; local consent and recording rules still apply.
- Start only after the required consent step. The exact control may be a button, a verbal announcement, or another local procedure. The organization should document how consent or refusal is recorded.
- Conduct the visit normally. Patients should be able to ask questions or request that recording stop. The clinician should not assume the software heard or understood every statement.
- Review privately. After the encounter, the clinician checks the draft, removes irrelevant material, corrects errors, and adds information that was not captured.
- Finalize through the approved EHR process. Only the validated version should become part of the patient record, subject to local retention and amendment policies.
Audio, transcripts, and the official medical record
There may be several data objects: the original audio, an intermediate transcript, the generated draft, and the signed final note. They do not automatically have the same status or retention period. NHS England’s information-governance guidance directs organizations to their own privacy notice and governance arrangements. It does not establish one retention period for every ambient scribe.
UCSF’s patient information gives one example in which a recording is a temporary aid and is not part of the final official medical record or Designated Record Set. That example cannot be generalized to other providers. Before deployment, ask specifically:
- Is audio stored, and if so, for how long and in which region?
- Is a transcript retained after the note is signed?
- Who can access recordings, transcripts, drafts, and audit logs?
- Are data shared with subcontracted service providers?
- Can an organization delete temporary data or export it for an audit?
- Which object is discoverable, amendable, or included in the designated record set under local policy?
Do not infer a vendor’s retention or deletion practice from the fact that its final note appears in an EHR. Those are separate policy and technical questions.
Privacy, consent, and governance requirements
An ambient scribe processes a conversation that can contain protected health information. The healthcare organization and any downstream integrators remain responsible for complying with applicable privacy, consent, and recording requirements. A patient notice should be understandable and should identify the purpose of capture, the service provider’s role, possible sharing with service providers, and the available alternative if the patient declines.

Governance should cover more than the initial consent script. Establish who may activate a session, how interruptions and visitors are handled, how a refusal is recorded, how staff report an incorrect note, and how access is reviewed. Include a process for disabling the tool during sensitive portions of an encounter when policy or patient preference requires it.

An AI scribe can be useful without automatic EHR insertion, but manual transfer adds work and creates opportunities for copy-and-paste errors. An integrated workflow may place a draft in a review queue, map fields to an encounter, or export a document for clinician approval. The exact behavior is product- and EHR-specific.
NHS England’s implementation guidance highlights vendor EHR experience and interoperability standards. During evaluation, test the complete path in a non-production environment: launch, capture, draft creation, editing, approval, transfer, correction, and audit history. Confirm what happens when an encounter is cancelled, a patient has multiple appointments, or a clinician edits the note after it reaches the EHR.
Regulatory status is jurisdiction-specific
Whether a digital or AI scribe is regulated as a medical device depends on its intended use, product characteristics, and the jurisdiction. NHS England’s guidance applies to England and does not decide whether individual products meet a medical-device definition. Australia’s Therapeutic Goods Administration separately explains circumstances in which digital scribes may be regulated as medical devices. These sources support a jurisdiction-by-jurisdiction assessment, not a worldwide claim that all scribes are either regulated or exempt.
Organizations should document the product’s stated intended purpose, the functions it performs, the claims made in its documentation, and the rules that apply where it will be used. A tool that only drafts documentation may be treated differently from software that makes or recommends clinical decisions, but the boundary must be assessed under the applicable law.

What is known about time savings
The American Medical Association reported in 2025 that The Permanente Medical Group had documented more than 2.5 million patient encounters after one year of rollout. The rollout began in late 2023. In a 63-week evaluation covering October 2023 through December 2024, the organization reported statistically significant reductions in note-taking time, time spent per appointment, and after-hours documentation, sometimes called “pajama time.”
Those figures describe one organization’s implementation and evaluation. They are not a universal effect size for every vendor, specialty, staffing model, or EHR. The reviewed official workflow sources do not establish a general AI-scribe accuracy percentage or error rate, so a practice should measure its own review time, correction rate, patient experience, and documentation quality.
How to evaluate a product before deployment
| Evaluation area | Evidence to request | Why it matters |
|---|---|---|
| Capture | Supported ambient, dictation, and upload modes; behavior with multiple speakers and interruptions | Determines whether the tool fits the real encounter rather than a demonstration |
| Output and review | Available note or letter types, editable fields, approval controls, and draft history | Shows whether clinicians can validate the document efficiently |
| Integration | Supported EHR versions, interoperability standards, field mapping, authentication, and failure handling | Prevents a useful draft from becoming a manual re-entry burden |
| Data handling | Audio and transcript retention, access controls, service-provider sharing, deletion, export, and audit information | Separates the final note from temporary or secondary data |
| Consent and governance | Patient notice, refusal workflow, local privacy-notice alignment, and incident procedures | Supports lawful and trustworthy use |
| Regulatory context | Intended-use statement and jurisdiction-specific assessment | Prevents an inappropriate blanket assumption about medical-device status |
Common failure modes and practical controls
The transcript misses or mishears a statement
Noise, accents, overlapping speech, and medication names can produce transcription errors. Use a quiet setup where possible, pause or repeat critical information, and verify clinically material text against the encounter.
The draft invents or overstates a fact
A generated sentence can sound plausible without being supported by the conversation. Treat every diagnosis, dosage, result, negative finding, and follow-up instruction as an item requiring confirmation. Remove unsupported content rather than “trusting” fluent wording.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
A speaker is assigned the wrong role
Role labeling is not universal. Check whether a statement attributed to the patient was actually made by the clinician, interpreter, caregiver, or another participant.
Best Value
Irrelevant conversation enters the note
Ambient capture may include administrative discussion or sensitive details unrelated to the encounter. Edit the draft to the minimum clinically appropriate content and follow local policy for handling information that should not enter the record.
The EHR transfer fails
Keep a defined fallback, such as reviewing and entering the note through the ordinary EHR workflow. Test duplicate encounters, network interruption, expired sessions, and rejected field mappings before relying on automation.
A patient declines recording
Do not make the scribe a condition of receiving care. Follow the organization’s documented alternative, such as clinician dictation after the visit or ordinary manual documentation, and record the refusal as required by local policy.

- Define the approved capture modes and the clinical settings in which each may be used.
- Publish a patient-facing explanation and a refusal alternative.
- Map audio, transcript, draft, and final-note retention separately.
- Confirm access, sharing, deletion, export, and audit controls with the provider.
- Test the exact EHR workflow, including corrections and downtime fallback.
- Train clinicians to review names, dates, medications, negations, results, assessment, and plan.
- Monitor local measures such as review time, edits per note, rejected drafts, patient complaints, and documentation completed after hours.
- Reassess regulatory obligations when the product’s intended use, features, or operating jurisdiction changes.
The practical bottom line
An AI medical scribe is best understood as a pipeline that captures permitted input, transcribes and organizes it, generates a configurable draft, and returns that draft to a clinician for validation. Its value depends less on the novelty of the language model than on capture quality, review discipline, privacy governance, and a reliable EHR workflow. Treat recordings and transcripts as sensitive data, verify the product’s actual behavior, and never allow a fluent draft to substitute for clinical judgment or professional sign-off.
