AI-MARRVEL helped researchers prioritize a rare BRSK1 variant as a candidate explanation for one unresolved case; it did not diagnose patients on its own. Follow-up matching of other families and experiments in fruit flies added evidence linking monoallelic BRSK1 variants to a neurodevelopmental disorder that may occur with or without epilepsy.

What did AI-MARRVEL contribute?

In an unresolved case from the Texome Project, standard genetic analyses of a parent and child had not identified an answer. AI-MARRVEL analyzed genomic data and highlighted a rare change in BRSK1 as a promising candidate. As co-lead author Hugo Bellen put it, the tool “highlighted a rare change in the BRSK1 gene as a promising candidate for a genetic diagnosis.” (Texas Children’s Hospital, September 28, 2026)

The researchers then used GeneMatcher to find additional families with rare BRSK1 variants and performed functional experiments. The study reports 10 affected individuals from seven unrelated families, including nine people identified after the initial Texome Project case. The evidence therefore came from a broader process of candidate prioritization, family matching, and laboratory investigation—not from an AI result alone. The available report does not establish AI-MARRVEL’s accuracy, sensitivity, benchmark performance, or ability to generalize across patients. One successful candidate-prioritization example is not a measure of how often the tool finds a correct diagnosis.

What condition did the study describe?

All 10 affected individuals had some degree of developmental delay, but their symptoms and severity differed. Reported features included delayed speech and language, intellectual disability, autism spectrum disorder, ADHD, anxiety, low muscle tone, and microcephaly. Two individuals had seizures. Thus, seizures were present in this series but were not universal.

Symptoms could also differ among relatives carrying the same variant. Texas Children’s Hospital noted that “Even among members of the same family carrying the same genetic variant, symptoms ranged from mild to severe.” The small number of reported individuals does not establish how common each feature is across everyone with a BRSK1 variant, or predict an individual’s course. (Texas Children’s Hospital)

What did the fruit-fly experiments show?

Researchers studied sff, the fruit-fly ortholog of human BRSK1. Disabling sff caused locomotor problems, sensitivity to stress, paralysis when exposed to heat, and a shorter lifespan. Normal human BRSK1 largely corrected movement and neurological problems in the flies, while patient-derived variants only partially restored function.

The journal abstract specifies three tested alleles: BRSK1 p.Ile202Val, p.Arg237Cys, and p.Thr406Ile. In the fly model, each provided only partial rescue and did not normalize neuromuscular-junction morphology or levels of Futsch, a protein involved in microtubule organization. The authors interpret these results as supporting partial loss-of-function effects for the tested alleles. These are findings in flies, not direct measurements of disease severity or prognosis in human carriers. (American Journal of Human Genetics abstract indexed by JoVE Visualize)

The institutional account also describes overgrowth of connections between neurons and muscles and increased levels of a protein involved in microtubule organization after BRSK1 activity was lost. The experiments suggest a possible biological connection between BRSK1 and microtubule organization; they do not establish that this mechanism explains every clinical feature. (Texas Children’s Hospital)

How is this different from another BRSK1 epilepsy study?

A separate 2025 paper in Epilepsia examined BRSK1 in an epilepsy-focused cohort. It reported trio-based exome sequencing in 394 epilepsy probands and six novel variants in seven probands, alongside mouse-knockout and other functional work. That cohort and its experiments are distinct from the 10-person, seven-family study involving AI-MARRVEL and fruit flies; its participants and results should not be added to the latter study’s totals. What does this mean for families seeking answers?

The findings make BRSK1 a candidate gene associated with a neurodevelopmental disorder that can include epilepsy, but they do not mean that every BRSK1 variant causes the condition. Interpreting a particular genetic result requires evidence about that variant and the person’s clinical context; this study does not provide an individual diagnosis or a prediction of symptoms.

The Texome Project is described by Texas Children’s Hospital as providing free genetic testing to medically underserved individuals with rare, undiagnosed conditions. That offer applies to the program’s eligible audience and should not be read as a promise that testing is universally free or that AI-MARRVEL is a consumer diagnostic service.
