AI in cancer research is used to find patterns in biological and clinical data, generate predictions, and support research decisions. Its roles range from identifying possible drug targets and predicting treatment response to analyzing scans, improving screening studies, matching volunteers to clinical trials, and monitoring patients in studies. A promising model or proof-of-concept result is not, by itself, evidence that an AI system is safe, effective, unbiased, or ready to guide routine patient care.
What AI means in cancer research
Artificial intelligence is a broad family of computational methods. Some systems learn statistical relationships from examples; others process language, images, molecular measurements, or combinations of data. The U.S. Food and Drug Administration’s Oncology Center of Excellence defines AI as “a machine-based system that can, for a given set of human-defined objectives, make predictions, recommendations, or decisions influencing real or virtual environments.”

In oncology, the objective might be to identify a suspicious area on an image, estimate whether a tumor will respond to a drug, find patients who meet a study’s eligibility rules, or help researchers design a trial. The same underlying techniques can have very different reliability depending on the cancer type, data source, patient population, and clinical workflow.
The National Cancer Institute describes AI activity across cancer biology, screening and diagnosis, drug discovery, treatment-response prediction, surveillance, and health-care delivery. FDA’s Oncology AI program also studies how AI and machine learning could affect oncology drug development and regulatory science. These are research and development uses; they should not be confused with a general-purpose AI that can diagnose an individual from a photo or recommend treatment without a clinician.

Where AI fits in the cancer-research pipeline
| Research task | Typical inputs | What an AI model may produce | What must still be established |
|---|---|---|---|
| Drug discovery | Molecular structures, genomic data, laboratory results, scientific literature and databases | Candidate targets, compounds, mechanisms or repurposing hypotheses | Laboratory activity, safety, pharmacology, clinical benefit and regulatory evidence |
| Treatment-response prediction | Tumor gene expression, mutation profiles, clinical records and drug-response measurements | A probability or ranking of likely response for a defined setting | Prospective validation, clinically meaningful benefit and safe integration into treatment decisions |
| Screening and diagnosis | Medical images and other clinical or biological measurements | A flag, classification, measurement or decision-support output | Accuracy in the intended population and workflow, plus evidence that use improves care |
| Clinical trials | Eligibility data, trial records, adverse events, follow-up measurements and site information | Potentially eligible participants, monitoring signals, progression estimates or design recommendations | Reliable performance across sites, protection of participants and evidence that the trial process is improved |
There is no single “cancer AI.” A model trained on one cancer, imaging device, hospital or population cannot automatically be applied to another. Comparing systems therefore requires comparing the task, data, validation design and intended use rather than the marketing label.
AI for cancer drug discovery
Finding targets and mechanisms
Drug discovery begins with biological questions: which molecule, pathway or cellular process is driving a cancer, and which intervention could alter it without unacceptable harm? AI can examine genomic, transcriptomic, proteomic, chemical and clinical data to detect relationships that would be difficult to find by manually reviewing each variable. Researchers may use these patterns to prioritize targets, connect a tumor feature with a mechanism, or propose experiments.
A model’s output is a research hypothesis, not a drug. Scientists still need to test whether the proposed target is present and relevant in the cancer, whether changing it affects tumor biology, and whether the intervention can reach the target safely in people.
Designing and screening compounds
Machine-learning methods can rank existing compounds, estimate how molecules may interact with a target, and suggest new molecular structures for laboratory synthesis. Natural-language systems can help organize findings in the scientific literature or connect information spread across databases. These approaches may reduce the number of candidates that researchers test first, but they do not remove the need for chemistry, toxicology, pharmacology and controlled experiments.
Drug repurposing
AI can also search for relationships between approved or previously studied medicines and cancer mechanisms. A repurposing prediction may identify a plausible combination or a disease context worth testing. It cannot establish that a medicine works for that cancer, at a particular dose, or with another treatment. Those questions require appropriate laboratory studies and clinical trials.

Predicting how a tumor may respond to treatment
One of the most attractive research applications is matching a tumor’s molecular profile with a treatment likely to help. Models may combine gene-expression measurements, mutations, single-cell data and clinical information to estimate response or resistance. The intended result is often a ranked list of hypotheses for researchers, rather than an autonomous prescription.
The PERCEPTION proof of concept
In a report dated April 18, 2024, the National Cancer Institute described PERCEPTION, an AI tool developed in a proof-of-concept study. It used gene-expression data from individual tumor cells to predict responses to cancer drugs. The work illustrates how single-cell measurements could eventually help identify differences within a tumor and explore which cells might resist therapy.

A proof of concept shows that a method can produce a technically interesting result in the studied setting. It does not show that using the method prospectively improves survival, works for every cancer, or performs equally well in ordinary hospitals.
AI in cancer screening and diagnosis
What the models analyze
Screening and diagnostic research can apply AI to medical images and to other clinical or biological data. A model may identify patterns associated with a lesion, segment a tumor, measure change over time, or help prioritize cases for human review. In principle, this can make large datasets easier to analyze and help researchers quantify features that are difficult to describe consistently.
Why the setting matters
Performance depends on details that are easy to overlook: the cancer type, disease stage, prevalence in the screened population, imaging equipment, acquisition protocol, demographic mix, and the definition of a positive result. A model developed using one hospital’s scans may encounter different image quality or disease patterns elsewhere. A tool designed for screening people without symptoms is not automatically suitable for diagnosing symptomatic patients, estimating prognosis or selecting treatment.

Finding a statistical pattern is also not the same as improving health. Researchers need to know whether clinicians can use the output without creating delays, unnecessary biopsies, missed cancers or unequal access to follow-up. Comparative performance results for named diagnostic products are not established by the sources summarized here, so broad claims that AI is “more accurate than doctors” would be unwarranted.
AI and cancer clinical trials
Matching people to studies
Eligibility criteria are often written in complex language and may be scattered across medical records, laboratory results and pathology reports. Informatics and AI tools can help search records for possible matches, identify missing information and present candidates to research teams. A suggested match still requires confirmation by qualified study staff, because criteria can include timing, prior therapies, organ function, measurable disease and safety considerations that a model may misunderstand.
Improving trial design and representativeness
Researchers can use data analysis to examine how eligibility rules affect enrollment, identify communities that are underrepresented, and explore recruitment strategies. FDA scientific-interest work includes AI and machine-learning methods related to clinical-trial design, real-world data and data quality. The objective is to make studies more informative and feasible, not to replace the protocol, investigators or informed consent.
Monitoring safety and disease progression
AI may help organize adverse-event reports, detect unusual patterns, estimate progression from repeated measurements or prioritize records for review. These systems can support faster attention to a possible signal, but a flagged pattern must be investigated. Automated monitoring should not be treated as proof that an event was caused by a treatment or that a participant is safe to continue.
Keeping trial evidence separate from AI evidence
The NCI explains that clinical trials methodically test interventions and move findings from laboratory research toward clinical settings. An AI model used to plan, monitor or analyze a trial is not the same thing as an AI-directed intervention shown to improve patient outcomes. The intervention, the trial design and the model’s own performance each need appropriate evaluation.

How researchers validate an oncology AI system
A credible result depends on more than a high score on the dataset used to train a model. When assessing an application, researchers and readers should look for the following sequence:
- Define the intended task. State whether the model is for discovery, screening, diagnosis, response prediction, trial recruitment, monitoring or another purpose. A vague objective makes meaningful evaluation impossible.
- Describe the data. Identify the cancer type, patient characteristics, sample size, measurement methods, institutions and outcome definition. Training data that omit important groups can produce systematic errors.
- Separate development from testing. A model should be tested on data it did not use to learn its parameters. Testing at an independent site is stronger than testing only on a randomly held-out portion of the same source.
- Use prospective evaluation when the claim is clinical. Retrospective records can show association, but they do not fully capture how clinicians and patients will use an output in real time. Prospective studies can measure workflow effects, safety and patient outcomes.
- Measure more than one metric. Depending on the task, relevant measures may include sensitivity, specificity, calibration, false-positive and false-negative rates, time to review, treatment changes and patient-important outcomes.
- Check performance across groups and sites. Overall accuracy can hide poor results for a demographic group, rare presentation, different scanner, community hospital or new time period.
- Make the output understandable and actionable. Explainable AI can help users understand which information influenced a result and when the model is uncertain. Explanations should support a decision, not create false confidence.
- Plan for monitoring after deployment. Data distributions, clinical practice and disease patterns change. Performance needs ongoing review, version control, incident reporting and a process for updating or withdrawing the system.
Data quality, bias and privacy
Representation and bias
The NCI warns that nondiverse or unrepresentative training data can perpetuate bias. If a dataset contains few examples from a population, the model may perform poorly for that group even when its average result looks good. Bias can also enter through inconsistent labels, differences in access to testing, missing records or historical treatment inequities. Adding more data does not automatically fix the problem if the new data reproduce the same imbalance.
Reproducibility
Independent teams should be able to understand how a model was trained, what data were used, which version produced a result and how outcomes were measured. Reproducibility standards matter because an impressive result that cannot be repeated is not a dependable foundation for care or regulation.
Privacy and governance
Cancer datasets can contain identifiable clinical, genomic and family information. Responsible projects need lawful data use, access controls, secure storage, clear consent or other appropriate governance, and rules for sharing results. De-identification reduces risk but does not make every dataset impossible to re-identify, especially when genomic or linked records are involved.

AI research can expand the number of hypotheses scientists test and may eventually support more individualized diagnosis or treatment. It cannot justify assuming that an online model has reviewed a person’s complete medical context, that a research prediction is a treatment recommendation, or that a trial-matching result guarantees eligibility.

Patients should ask their care team what evidence supports a tool, whether it is being used for research or routine care, what information it considered, and who reviews the result. A person considering a clinical trial should rely on the study team for the official eligibility decision and consent discussion.
A practical framework for comparing two AI cancer applications
Use these questions before treating one application as better than another:
- Task: Are the systems solving the same problem, or is one for discovery and the other for clinical workflow?
- Population: Which cancers, stages, demographic groups and care settings were represented?
- Data: Were the inputs images, molecular measurements, records, literature or a combination? Were data collected prospectively or retrospectively?
- Validation: Was testing independent, multi-site and prospective, or limited to the development dataset?
- Outcome: Did the system improve a patient-important result, or only produce a technical prediction?
- Transparency: Can users see limitations, uncertainty, model version and the basis for a result?
- Equity and privacy: How are subgroup performance, access, consent and data security handled?
- Workflow: Who reviews the output, what happens when it is wrong, and how is performance monitored over time?
The FDA’s oncology scientific-interest work specifically includes AI and machine-learning methods for scientific literature, real-world data, data quality, clinical-trial design and regulatory decision-making. Those areas show why evaluation must cover both the algorithm and the system around it.
Research resources and the path to clinical use
The NCI maintains a resources-and-tools directory covering research software, datasets and models. Such resources can help investigators explore methods and reproduce analyses; they are not consumer diagnostic services or guarantees that a listed model is ready for clinical deployment.
Moving from a research model to clinical use generally requires a defined intended use, suitable data, analytical and clinical validation, attention to bias and privacy, integration testing in the target workflow, appropriate oversight and continuing performance monitoring. The exact regulatory pathway depends on what the system does, how it is used and the jurisdiction in which it is deployed.
Bottom line
AI is already useful in cancer research because it can analyze complex biological, imaging and clinical data at a scale that supports new hypotheses and more targeted experiments. Its strongest established role in the evidence described here is as a research and decision-support method: proposing drug candidates, studying treatment response, finding patterns in screening data, and assisting clinical-trial operations. The essential boundary is evidence. A model’s prediction, a proof-of-concept such as PERCEPTION, or an automated trial match does not by itself prove clinical benefit. Diverse data, reproducible methods, independent and prospective validation, explainability, privacy safeguards and careful human oversight determine whether an AI application can responsibly move from research into oncology care.
