Short answer: smartwatch readings are useful estimates, not ground truth. Across the broadest recent evidence, average heart-rate bias was about ±3%, reported step-count errors ranged from −9% to 12%, and total sleep was typically overestimated by more than 10%. Those are pooled research results, not a guaranteed error margin for your watch. Accuracy changes with the model, software, wearer, activity and testing method.
Use a smartwatch to follow personal trends—such as whether your resting heart rate is usually rising or whether your activity is increasing. Do not use one reading to diagnose a heart problem, confirm a sleep disorder or make treatment decisions. A clinician and an appropriate reference test are needed when symptoms or persistent abnormal readings matter.

The evidence in one view
| Measurement | What broad reviews found | What that means in practice |
|---|---|---|
| Heart rate | Mean bias of about ±3% across included wearable studies in a 2024 living umbrella review. | Often close at rest and lower-intensity exercise, but device, fit, motion and exercise intensity can materially change results. |
| Step count | Reported mean absolute percentage error values ranged from −9% to 12% across reviewed evidence. | Controlled walking can produce close counts, while slow, irregular or everyday movement may be counted differently by another model. |
| Total sleep time | Wearables generally overestimated sleep, typically with mean absolute percentage error above 10%. | A watch may be better at showing a rough pattern than the exact time you were asleep. |
| Sleep stages | A prospective study of 11 consumer trackers, summarized in a 2026 review, reported macro F1 scores from 0.26 to 0.69. | Stage labels such as light, deep and REM are algorithmic estimates and are not interchangeable with polysomnography. |
The most comprehensive synthesis is Doherty and colleagues’ living umbrella review in Sports Medicine, published 30 July 2024. It found major differences among devices, outcomes and study methods, and warned that “a conclusive assessment of their accuracy is impeded by pervasive heterogeneity in research outcomes and methodologies.” Read the review.

Heart rate, steps and sleep are separate measurement problems. A watch can perform well for resting pulse but lose accuracy during intervals; count steps reasonably on a treadmill yet miss movement around the house; and estimate sleep duration acceptably while misclassifying individual stages. Combining those outputs into one “accuracy” number hides the differences that matter.

Accuracy also depends on the reference standard. A laboratory heart-rate test may compare a watch with an electrocardiogram while a step study may use manually counted steps or video. Sleep validation usually compares a tracker with polysomnography, which records brain activity, eye movements, muscle activity and other signals that a wrist device does not directly measure. Results from one reference method cannot automatically be transferred to another.
Generation matters too. Manufacturers change optical sensors, motion sensors, firmware and algorithms. A result for an older model is evidence about that model and protocol, not proof about every current watch from the same company. The 2024 review identified validation for only 34 of 310 devices (11%) in its inventory for at least one biometric outcome. Under its assumptions of five possible outcomes, only 54 of 1,550 device-outcome validations (3.5%) had been conducted. Those percentages describe the review’s catalog and assumptions, not every device now sold.
How accurate is smartwatch heart rate?
What the watch is measuring
Most wrist-worn consumer watches estimate pulse with photoplethysmography (PPG). LEDs shine light into the skin and sensors analyze changes in reflected or absorbed light as blood volume changes with each beat. The device then applies filtering and an algorithm to produce a heart-rate estimate.

PPG is not the same signal as an electrocardiogram. It is vulnerable to movement, changing contact pressure and anything that alters the optical path between sensor and skin. The watch may also smooth or reject readings when its software judges the signal unreliable.
What studies report
The 2024 umbrella review found a mean heart-rate bias of about ±3% across the wearable studies it included. “Mean bias” is an average difference from the reference measurement; it does not mean every reading is within 3%, nor that the error is identical at rest and during exercise.

A 2020 systematic review of 158 publications covering nine commercial wearable brands found that performance varied by manufacturer and study. Apple and Garmin devices were among the more accurate in the laboratory studies included in that review, while Fitbit devices tended toward underestimation. Those studies largely predate newer generations, so they should not be read as a current, universal brand ranking. See the 2020 systematic review.
The INTERLIVE expert statement emphasizes that heart-rate validity should be tested under defined conditions and with an appropriate reference. Validation studies often find better agreement at rest or lower intensity than during vigorous exercise, although the size and direction of the difference vary by protocol. Read the INTERLIVE recommendations.

Why active heart rate is harder
- Motion: Arm swing and rapid changes in wrist position can create optical and mechanical noise.
- Fit: A loose band lets the sensor move; an excessively tight band can be uncomfortable and may still distort contact.
- Exercise intensity: Fast intervals, strength training and sports with repeated wrist flexion are more demanding than quiet sitting.
- Skin and anatomy: Hair, tattoos, skin properties, wrist shape and perspiration can affect the optical signal.
- Protocol: A supervised laboratory test with a steady workload does not reproduce every free-living workout.
How to use a heart-rate reading
For personal tracking, consistency is more useful than chasing a single exact number. Wear the watch in the same position and compare readings taken under similar conditions: for example, morning resting values or the same workout route. Look for a sustained change from your own baseline rather than treating a one-minute spike as a diagnosis.
If a displayed value conflicts with how you feel, stop and check manually or with a clinically appropriate device. Chest discomfort, fainting, severe breathlessness or other urgent symptoms require medical attention regardless of what the watch says. A consumer reading cannot rule a dangerous rhythm in or out.
Are smartwatch step counts accurate?
What the evidence shows
The 2024 umbrella review reported that wearables mostly underestimated step counts overall, with reported mean absolute percentage error values spanning −9% to 12% across the reviewed evidence. The range combines different devices, populations and protocols, so it is not a tolerance specification for an individual watch.

The 2020 systematic review found that Fitbit, Apple Watch and Samsung devices appeared to measure steps accurately in laboratory-based studies. The authors’ conclusion was that commercial wearables are accurate for measuring steps and heart rate in laboratory settings, but that performance varies by manufacturer and device type. Laboratory success does not guarantee the same result during daily life.

Why everyday counts differ
- Walking speed: Very slow steps can produce a motion pattern the algorithm does not recognize reliably.
- Irregular movement: Pushing a stroller, carrying bags, using a walker or walking with limited arm swing can reduce wrist motion.
- Non-walking motion: Repetitive arm movements may be counted as steps, depending on the algorithm.
- Placement: A watch worn on a dominant or non-dominant wrist, or transferred between wrists, may produce different results.
- Thresholds and updates: Each manufacturer chooses filtering rules, and firmware updates can change how borderline movements are classified.
How to check your own count
- Choose a straight route and determine a reference distance or manually count a manageable number of steps.
- Wear the watch as you normally do, start the activity if your device requires it, and walk at your usual pace.
- Repeat the walk several times, including one slow segment if that reflects your routine.
- Compare the watch with your reference across the whole set, not one pass.
- Repeat the check during ordinary days if you need to know how it behaves with shopping, stairs, desk work and other movement.
This kind of self-check tells you how the device behaves on your wrist. It does not establish that another model, another wearer or another activity will have the same error.
Can a smartwatch accurately track sleep?
Total sleep time
Sleep tracking is usually an inference problem. A watch observes signals such as movement and pulse patterns, then estimates when you are asleep or awake. It does not directly observe the brain signals used in a clinical sleep study.
The 2024 umbrella review found a consistent tendency to overestimate total sleep time, typically with mean absolute percentage error above 10%. Quiet wakefulness can look like sleep to an algorithm, and brief awakenings may be missed. Consequently, time in bed, estimated sleep duration and actual physiological sleep are not interchangeable.
Sleep stages
Stage labels are less dependable than a simple bedtime or wake-time trend. A 2026 systematic review summarized a prospective multicenter validation of 11 consumer sleep trackers in which macro F1 scores ranged from 0.26 to 0.69. Macro F1 combines sensitivity and precision across classes; the wide range shows substantial variation in how well trackers classified sleep states.

The same review summarized comparisons indicating that several trackers still need improvement in identifying specific stages. A watch’s “deep,” “light” or “REM” graph should therefore be treated as an estimate, not as a substitute for polysomnography or a clinical sleep evaluation.
What sleep data is good for
- Watching whether your usual sleep window is shifting over weeks.
- Noticing repeated changes that coincide with travel, illness, alcohol, schedule changes or training.
- Providing a conversation starter for a clinician when symptoms and patterns persist.
Do not use a stage score alone to diagnose insomnia, sleep apnea or another disorder. If you have loud snoring, witnessed breathing pauses, excessive daytime sleepiness, repeated awakenings or another concern, seek an appropriate medical assessment.
Laboratory accuracy versus real-life accuracy
| Evidence setting | Strength | Limitation |
|---|---|---|
| Controlled laboratory test | Known workload, reference instrument and repeatable conditions make device differences easier to quantify. | May not include loose fit, sweat, unusual movement, varied walking speeds or a normal night at home. |
| Free-living field study | Captures ordinary behavior, environmental variation and long-term wear. | Reference measurements are harder to collect continuously, and adherence or missing data can affect results. |
| Single-person self-check | Shows how one exact watch behaves for one wearer and task. | Cannot establish population accuracy or predict another metric, activity or model generation. |
The 2020 review’s favorable conclusions for steps and heart rate were specifically strongest in laboratory settings. The INTERLIVE statement likewise stresses a defined protocol and reference standard. When reading a validation claim, check whether the task resembles the way you intend to use the watch.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to compare smartwatch accuracy claims
There is no evidence here for one universally most accurate current smartwatch across heart rate, steps and sleep. Compare a device using the following questions instead of a marketing-wide accuracy label.
- Is it the exact model and generation? An older study may not cover the current hardware or software.
- Which metric was validated? Separate resting heart rate, active heart rate, steps, total sleep and individual sleep stages.
- What conditions were tested? Look for walking speed, exercise intensity, sleep duration and free-living versus laboratory use.
- What was the reference standard? An electrocardiogram, manual step count and polysomnography answer different questions.
- Who participated? Age, fitness, skin characteristics, health status and sample size affect generalizability.
- Which error statistic was reported? Bias, mean absolute error, percentage error, limits of agreement and classification scores are not interchangeable.
- Was the study independent and peer reviewed? Manufacturer testing can be useful, but independent validation provides a different level of scrutiny.
The 2024 review’s inventory illustrates why this diligence matters: most consumer devices and device-outcome combinations have not been validated under a common protocol. A claim that a watch is “accurate” is incomplete unless it specifies the outcome and conditions.

Steps to improve the quality of your readings
- Wear the watch consistently. Use the same wrist and a stable position above the wrist bone unless the manufacturer specifies otherwise.
- Keep the optical sensor clean. Sweat, sunscreen and dirt can interfere with skin contact and light transmission.
- Use a secure, comfortable fit. The sensor should remain in contact during movement without causing pain or numbness.
- Record context. Note exercise intensity, unusual motion, illness, alcohol, travel and any change in firmware or device settings.
- Compare like with like. Evaluate resting heart rate against resting heart rate, a normal walk against a normal walk and sleep trends across similar nights.
- Allow enough repeated observations. A trend over multiple days or sessions is less vulnerable to one bad signal than a single reading.
- Cross-check important findings. Use a validated home instrument or clinical measurement when a result affects a health decision.
- Keep software current, but note updates. Algorithms can change after an update; mark the date if your baseline suddenly shifts.
When a reading should not guide a decision
- A sudden high or low heart-rate value that does not match repeated checks or your symptoms.
- A sleep-stage score used to decide whether you have a sleep disorder.
- A step total used as proof that a particular exercise prescription is safe or complete.
- Any reading used to start, stop or change medication.
- A device result that conflicts with severe symptoms, fainting, chest pain or serious breathing difficulty.
In these situations, treat the watch as a prompt to verify the information, not as the authority. Clinical assessment may require an electrocardiogram, medical-grade activity measurement or polysomnography, depending on the question.
What the current research can and cannot establish
The 2024 living review searched MEDLINE/PubMed, Embase, CINAHL and SPORTDiscus through 5 June 2024. Its pooled results are valuable for showing broad patterns, but the authors found substantial heterogeneity in devices, algorithms, participants, reference standards and outcomes.
The 2020 review remains useful foundational context because it examined 158 publications and nine commercial brands, but its evidence predates many current generations. The 2021 INTERLIVE statement supplies a framework for validating consumer heart-rate devices rather than a ranking of products. The 2026 review adds newer sleep-tracker evidence, including the 11-device prospective study, but likewise summarizes heterogeneous studies.

Bottom line
Smartwatches are best treated as personal trend instruments. Heart-rate averages can be close, especially at rest; step counts can be reasonably accurate in controlled walking; and sleep duration is often overestimated, with stage labels varying substantially. Because performance changes by metric, model and conditions, use repeated readings for context and seek clinical testing when a health decision depends on accuracy.
