How Accurate Are Fitness Trackers for Steps, Sleep, and Heart Rate?

Fitness trackers are best treated as tools for following personal trends, not as precise measuring instruments. Accuracy depends on the metric, the device and software, and the conditions in which you wear it. A 2024 umbrella review found different patterns for steps, heart rate and sleep, but its pooled results do not guarantee how any particular current tracker will perform for you.

What the evidence says about tracker accuracy

A 2024 living umbrella review by Doherty and colleagues brought together 24 systematic reviews covering 249 non-duplicate validation studies and 430,465 participants. The authors compared consumer wearables with accepted reference standards, but the underlying studies differed in devices, methods and measurement contexts. Their headline numbers describe that varied body of research—not a controlled test of every tracker on sale today.

The review found that only about 11% of 310 consumer wearable devices in its inventory had been validated for at least one biometric outcome. Validation for one measure does not establish accuracy for other features on the same device. The authors cautioned that device hardware and software change faster than validation research can keep pace. Read the 2024 umbrella review in Sports Medicine.

Metric Finding in the 2024 umbrella review What it means for an individual reading
Steps Reported mean absolute percentage errors ranged from −9% to 12% across the included evidence. Wearables mostly underestimated steps overall, but the range is not a fixed error for every model or wearer.
Heart rate Mean bias was approximately ±3% across the summarized evidence. A pooled average does not promise that a reading will be close during every activity or for every person.
Total sleep time Trackers tended to overestimate sleep; typical mean absolute percentage error was greater than 10%. Use sleep duration as an estimate and look for patterns rather than treating the displayed time as exact.

These figures summarize heterogeneous prior reviews; they are not product-specific precision guarantees. A separate 2020 systematic review found that commercial wearables can measure steps and heart rate accurately in laboratory settings, while noting variation by manufacturer and device type. How accurate are fitness trackers for steps?

Step counts were usually underestimated in the umbrella review, but the reported mean absolute percentage error range—from −9% to 12%—does not mean that your tracker will consistently miss a particular share of your steps. The result varies across studies and devices, and an error seen in a validation setting may not carry over to another model, software version or way of walking.

When comparing step counts, check what the device was validated against. Studies may use manual observation or accelerometry as reference standards. Also consider whether the evidence matches your use: a tracker assessed during steady walking is not automatically validated for every daily activity or movement pattern.

Can you trust a fitness watch’s heart-rate reading?

The 2024 review reported heart-rate mean bias of approximately ±3%, but that pooled figure should not be read as an accuracy guarantee during a specific workout. The review found that performance varied with device model, activity intensity, user characteristics and context. A result at rest or during a controlled test does not establish performance during vigorous or irregular exercise.

For a model comparison, look for validation against an appropriate reference, such as an electrocardiogram (ECG) or a validated chest strap, and check whether the tested activity resembles yours. The 2020 systematic review likewise found that performance varied by manufacturer and device type, despite evidence that wearables can measure heart rate accurately in laboratory settings.

Do fitness trackers accurately measure sleep?

Trackers tended to overestimate total sleep time in the 2024 review, with typical mean absolute percentage error greater than 10%. They also tended to underestimate sleep-onset latency—the time it takes to fall asleep—and wakefulness after sleep onset. That combination can make a night appear longer or more continuous than it was.

Sleep stages need particular caution

Sleep-stage labels and estimates of wakefulness should not be treated as clinical measurements. A tracker’s overnight output can be useful for noticing broad patterns, but the review does not establish that consumer sleep-stage readings diagnose a sleep condition. Polysomnography is an example of a reference standard used in sleep validation research; a consumer tracker is not a substitute for clinical assessment.

How to compare two trackers fairly

There is no universal most-accurate current tracker established by the evidence summarized here. A useful comparison has to be specific to the measurement you care about and the conditions in which you will use it.

  1. Choose the metric. Compare steps with steps, heart rate with heart rate, and sleep measures with the same sleep measures. Accuracy in one feature does not establish accuracy in another.
  2. Match the use context. Rest, steady walking, vigorous or irregular exercise, and overnight sleep are different measurement conditions.
  3. Check the reference standard. Look for the standard used in validation: for example, manual observation or accelerometry for steps, ECG or a validated chest strap for heart rate, and polysomnography for sleep.
  4. Match the exact device and software. Evidence for one model or version may not transfer to a later model or algorithm update.
  5. Consider fit and wearer factors. Placement, wear time and user characteristics can affect what the device records.

How to use tracker readings day to day

  • Use readings to observe patterns, particularly when comparing the same device under similar conditions.
  • Treat individual step, heart-rate and sleep values as estimates, not exact measurements.
  • Do not use consumer sleep-stage output or a wearable heart-rate value to diagnose a medical condition; tracker validation evidence does not show that these readings replace clinical measurement.
