It is best used alongside clinical surveillance and interpreted in the local context.
What hospital wastewater surveillance measures
Wastewater and environmental surveillance (WES) uses population samples from sewage where sewer networks exist, or from other human-impacted waters where they do not. The World Health Organization (WHO) treats WES as one component of multimodal public-health surveillance, rather than a stand-alone replacement for clinical data. WHO’s pilot guidance, published 6 December 2024, sets out a framework for prioritizing, implementing and integrating these programs.

For antimicrobial resistance (AMR), monitoring may look for antimicrobial-resistant bacteria (ARB) or antimicrobial-resistance genes (ARGs). Those are related but distinct signals: finding a resistance gene does not by itself establish which organism carries it, whether it is viable, or whether a person has an infection. Residual antimicrobial drugs are another category of measurement, not interchangeable with ARB or ARG detection.

What a wastewater result can—and cannot—tell you
A facility sewer sample combines material entering the wastewater stream. Detection can indicate that a target is present in the sampled system, but it does not identify the patient or source, establish a clinical case, or quantify infections and transmission without additional evidence. Interpretation depends on the target, sampling design, laboratory method and local sewer conditions.

In a 2026 study published by the U.S. Centers for Disease Control and Prevention (CDC), researchers monitored five carbapenemase genes at one rehabilitation hospital over 16 months. The study found promise in hospital wastewater monitoring, but the authors said sewer biofilms and the small number of clinical infections during the study made correlations with clinical incidence difficult. They concluded that further research is needed; this single-facility study does not establish a universal protocol. Read the CDC Emerging Infectious Diseases article.
How sampling and testing choices differ
The CDC study compared several approaches. None was established as universally superior, and its design should be read as a comparison within one facility and study period—not a standard that every hospital can adopt unchanged.

- Professional Quality Testing Kit: The AAwipes Aquarium test kit (50 strips) is developed by experts for fish enthusiasts. It offers 8-in-1 testing capabilities, made with high-quality fiber paper and non-bleeding colors. The kit measures key parameters like Free Chlorine, Nitrate, Nitrite, Hardness, Sodium Chloride, Total Alkalinity, pH and Ammonia nitrogen. By identifying potential issues, our fish tank testing kit helps prevent hidden water problems and ensures the safety and well-being of your fish.
- Versatile Use for Freshwater and Ponds: This aquarium water test kit is versatile and suitable for both freshwater and saltwater environments. It can be used in various aquariums, fish tanks, ponds, pools, and shrimp tanks, making it easy to monitor water quality and ensure your filter functions properly. Experts recommend testing your aquarium water at least twice a week to maintain optimal water quality.
- Fast and Accurate Results: Get accurate results in minutes with just 3 steps: 1) Dip the test strip into water for 2 seconds for pads 1-7; dip pad 8 (Ammonia nitrogen) separately for 5 seconds. 2) Remove the strip without shaking off excess water. Hold horizontally for 30 seconds for pads 1-7, wait 180 seconds to read Pad 8 (Ammonia nitrogen). 3) Compare the colors to the chart for up to 99% accuracy and take necessary action.
- Extended Shelf Life: The test strips are packaged in a well-sealed bottle to ensure they remain effective over time. This protects the strips from moisture and other environmental factors, ensuring you have reliable testing materials on hand when needed.
- Convenient Use: With this kit, you can test your aquarium water at home without the need to send samples to a lab, saving you both time and money. Enjoy peace of mind knowing your aquarium water is balanced and safe without the stress or extra expense.
| Choice | Approaches in the 2026 study | What the choice changes |
|---|---|---|
| Sample collection | Composite, passive and biofilm-based sampling | Which wastewater material or sewer-associated signal is captured; biofilms can contribute signals that complicate comparison with clinical cases. |
| Collection frequency | Weekly, twice-weekly or monthly collection | How often samples are observed. The study does not identify one frequency as best for all facilities. |
| Laboratory analysis | qPCR, metagenomics and culturing | Different analytical approaches were used; results should be interpreted in light of the target and assay rather than treated as interchangeable measures. |
The five genes monitored were blaKPC, blaVIM, blaOXA-48-like, blaNDM and blaIMP. That list describes the study targets, not every resistance marker of concern or a recommended panel for all hospitals.

WHO recommends selecting targets where wastewater data could inform significant public-health decisions in the local context, where the methods are technically and operationally feasible, where surveillance is ethically and legally acceptable, and where it can be integrated with clinical surveillance and other targets. - Start with a decision: Define what action the results could inform. If no local decision would change, the monitoring may not justify its operational burden.
- Choose a relevant target and method: Align the organism or gene of interest, collection method and assay with the question being asked.
- Plan for interpretation: Read wastewater findings alongside clinical surveillance and relevant facility context; do not treat detection as a patient-level diagnosis.
- Check feasibility and acceptability: Consider technical and operational capacity as well as ethical and legal requirements.
Why hospital results need a wider environmental context
Hospitals contribute wastewater to sewer networks, but they are not the only source. The U.S. Environmental Protection Agency (EPA) describes hospital and residential flows as contributors to AMR in sewer networks, alongside household and industrial sources. It is therefore not valid to attribute a sewer signal to a hospital or its patients solely because a sample was collected there.
EPA identifies the occurrence, fate and significance of resistant bacteria and genes in treated municipal wastewater and biosolids as ongoing research needs. Its work includes studying the relative contributions of hospital and residential flows, biosolids, PCR panels, and molecular and cultivation-based assays. The agency reported awarding $9 million in grants for research into AMR in wastewater and sewage-sludge treatment and environmental impacts on a page last updated 13 July 2026; that funding reflects research needs, not proof of a particular health risk.
