If a hospital bill does not match what you paid or what insurance processed, request an itemized statement and compare it with your insurer’s Explanation of Benefits (EOB) and your payment records. Ask the billing office to explain each adjustment, payment, and remaining balance. A deposit may not be your final responsibility, and federal guidance reviewed here does not establish one nationwide deposit rule or refund deadline.
How do I get an itemized bill from the hospital?
Contact the hospital’s billing department and request an itemized statement for the account and dates of care in question. If a charge is unclear or appears inaccurate, an itemized bill lets you review the services and amounts rather than relying only on a total balance. The Centers for Medicare & Medicaid Services (CMS) recommends checking the dates and services and asking the provider about anything that does not look right. - Check that the dates and descriptions match the care you received.
- Look for duplicate charges.
- Confirm that insurance payments and payments you already made are recorded.
- Ask the billing office to explain any charge or adjustment you cannot identify.
Keep the itemized bill with the deposit receipt, any estimate, relevant EOBs, and proof of payments so you can compare the account history.

A line labeled “adjustment” often reflects a discount or a lower amount the provider agreed to accept; it is not automatically a refund or an amount you owe. A hospital statement may list total provider charges, adjustments, insurance payments, patient payments already made, and a balance due. CMS explains these terms in its medical-bill guide.

Your insurer’s EOB uses a separate accounting of the claim. It may show the provider’s charges, the allowed charges, the insurer’s payment, and the patient balance. The allowed amount is the amount the insurer recognizes under the plan’s claim processing; your share may include a deductible or coinsurance. An EOB explains the insurer’s decision—it is not itself a bill. CMS states, “No, a bill isn’t the same as an Explanation of Benefits,” in How to read your medical bill.
Compare the provider statement with the EOB for each claim. The EOB does not show whether you have already paid part of the patient balance. CMS says the provider bill should not be higher than the EOB’s stated patient balance; if it is, ask the hospital to reconcile the account. If the insurer’s claim processing or patient-balance calculation seems wrong, contact the insurer and review the appeal process in your plan documents. CMS guidance on using insurance

Why does my bill say I owe money after insurance paid?
An insurer’s payment does not necessarily cover the full allowed amount. The remaining patient responsibility may include a deductible, coinsurance, or another amount assigned to you under the plan. Also check whether the hospital has included your earlier payments and whether every relevant claim has finished processing. The EOB tells you how the insurer handled a claim; the provider’s statement shows what the provider says remains due.
- Match each hospital service and date to the corresponding EOB.
- Compare the EOB’s patient balance with the hospital’s balance for that care.
- Check that the hospital credited insurance payments and your prior payments.
- Ask the hospital to explain any difference; contact the insurer if you believe the claim was processed incorrectly.
If the insurer denies a claim or assigns an amount you believe is wrong, review the denial notice and plan documents, then use the plan’s appeal process when appropriate. Federal surprise-billing protections may also apply, depending on the service and plan.

Will I get my hospital deposit back?
Possibly, but a deposit is not necessarily the final patient responsibility. Ask the hospital how it classified the payment—such as a prepayment, estimate, copayment, or another kind of payment—and how it will be credited after insurance finishes processing the claims. Request the explanation and the hospital’s policy in writing.
Once the claims have been processed, ask for a final account reconciliation. If your payments exceed the reconciled patient responsibility, ask the billing office to confirm whether there is a credit balance and whether it will refund the credit or apply it to another account. The federal guidance cited here does not establish a universal legal deposit rule or refund deadline; the answer may depend on the hospital’s policy, payer rules, and applicable state law.

How long does a hospital refund take?
There is no single nationwide refund timeline established by the federal sources cited here. Ask the hospital when it expects to complete the account reconciliation and issue any refund, and request the answer in writing. If a refund has not arrived by the date the hospital gave you, follow up with the billing department using the account number and your payment records.

What if I cannot afford the balance?
Ask the hospital for its financial assistance policy and application before agreeing to a payment plan or putting the bill on a medical credit card. The policy should explain eligibility, whether help is free or discounted, how charges are calculated, and how to apply. Tax-exempt hospital organizations must establish and widely publicize financial assistance policies under federal requirements, but eligibility and the assistance available vary. CFPB explains what to look for in its guidance on hospital financial assistance policies.

The Consumer Financial Protection Bureau cautions that putting a medical bill on a credit card may make it harder to obtain financial assistance. Read the CFPB’s guidance on using a credit card to pay a medical bill. State rules may provide additional protections; check the rules for the state where the care was provided.
Do federal protections apply to this bill?
Insured patients and surprise bills
The No Surprises Act limits certain out-of-network bills for people with most types of private insurance. The protections cover emergency services, some non-emergency services at in-network hospitals and similar facilities, and out-of-network air ambulance services. Whether a particular bill qualifies depends on the plan and service; ground ambulance services are generally outside the federal protections described in the CFPB guidance. See the CFPB’s explanation of the No Surprises Act. For questions or complaints, contact the CMS No Surprises Help Desk using CMS’s medical-bill dispute guidance.
Uninsured or self-pay patients and good faith estimates
If you have no insurance or choose not to use it, providers generally must give a good faith estimate when you request one or when care is scheduled at least three business days in advance. The estimate is not a bill, and it may cover only one provider or facility. If you meet the eligibility conditions and your initial bill is at least $400 higher than the estimate, CMS describes a patient-provider dispute process that generally must be started within 120 days of the initial bill. Check the current process and all eligibility requirements before filing. CMS guidance on good faith estimates and disputes
