Hives are raised, itchy welts on the skin’s surface. Angioedema is swelling in deeper layers of skin and tissue, often around the lips, eyelids, face, hands, feet, or throat. The two can happen together, but either can also occur alone. Hives are usually intensely itchy; angioedema is often not itchy and may hurt or feel tight. The appearance cannot tell you the cause, and throat or airway symptoms can become life-threatening. Use the guide below to recognize the pattern, decide how urgently to get help, and understand what information a clinician will need.
First, check for an emergency
Call your local emergency number or go to emergency care immediately if swelling or a rash is accompanied by any of the following:
As an Amazon Associate I earn from qualifying purchases.
- Difficulty breathing, wheezing, or struggling to breathe
- Tightness or swelling in the throat
- Sudden swelling of the lips, mouth, tongue, or throat
- Fainting, collapse, or feeling that you may pass out
Airway swelling can worsen quickly. The American Academy of Allergy, Asthma & Immunology (AAAAI) states: “Throat swelling can be life threatening and requires immediate medical attention.” Do not wait to see whether an over-the-counter antihistamine works when the mouth, tongue, throat, breathing, wheezing, or fainting is involved. If you have been given an emergency treatment plan by a clinician, follow that plan while emergency help is on the way.
The NHS also advises immediate emergency action for sudden swelling of the lips, mouth, throat, or tongue and for serious breathing difficulty. The appropriate emergency number depends on your location, so use your local service.

Hives and angioedema compared
| Feature | Hives (urticaria) | Angioedema |
|---|---|---|
| Where the problem is | Raised welts in the more superficial layers of skin | Swelling in deeper skin and tissue layers, often around the lips, eyelids, face, hands, feet, or throat |
| What it looks or feels like | Pink, red, or skin-colored bumps or patches that can vary greatly in size; usually itchy | Puffiness or enlargement of a deeper area; may feel tight or painful and is often not itchy |
| How lesions behave | Individual welts commonly fade within 24 hours without leaving marks or bruising, according to AAAAI | The swelling is deeper and may be more difficult to judge by looking at the surface alone |
| Can it occur alone? | Yes | Yes, or alongside hives |
| Typical urgency | Usually not an emergency unless airway or whole-body symptoms occur | Urgency depends on location and associated symptoms; throat or breathing involvement is an emergency |
| Longer-term label | Hives that persist or recur for more than six weeks are called chronic urticaria | Repeated or unexplained episodes require medical assessment because causes and treatments vary |
This table describes common patterns, not a home diagnosis. A person can have hives and angioedema at the same time, and the absence of hives does not rule out angioedema.
Read the pattern in four steps
- Check depth and location. Distinct, raised spots that seem to sit on the skin suggest hives. Smooth puffiness of a lip, eyelid, hand, foot, or other area suggests deeper swelling. Swelling of the tongue, mouth, or throat is an emergency concern regardless of whether welts are visible.
- Notice the dominant sensation. Marked itching points more toward hives. Pain, pressure, tenderness, or tightness without much itch points more toward angioedema, although sensations can overlap.
- Watch one lesion, not just the whole episode. A single hive commonly disappears within 24 hours while new welts appear elsewhere. A fixed area that remains swollen or a lesion that leaves bruising needs clinical review.
- Assess urgency before naming it. Breathing trouble, wheezing, fainting, throat tightness, or sudden mouth and tongue swelling override the usual comparison and require emergency help.
What hives look and feel like
Raised, changeable welts
Hives are raised areas on the skin that may be small and round, broad and irregular, or joined into larger patches. They can appear anywhere, move to different parts of the body, and vary in color depending on your skin tone. Itching is the characteristic sensation, although an individual person may also notice warmth, stinging, or a burning feeling.
The 24-hour clue
AAAAI says an individual hive usually fades within 24 hours and does not leave a mark or bruise. New welts can appear as older ones disappear, making the overall episode seem continuous. That pattern is different from one fixed spot that remains unchanged for days. If a lesion lasts longer than expected, leaves bruising, or the pattern is unclear, take a photograph and ask a clinician rather than trying to label it yourself.

Hives that persist or recur for more than six weeks are classified as chronic urticaria. The six-week threshold refers to the duration of the condition, not to one uninterrupted welt. Recurrent symptoms that disturb sleep, work, school, or daily activities deserve a clinical discussion even when there is no emergency symptom.
What angioedema is
Swelling under the skin
Angioedema affects deeper tissue layers, so the surface may look smooth and puffy rather than covered in distinct welts. Common sites include the lips, eyelids, cheeks, face, hands, and feet. Swelling can also involve the tongue, mouth, or throat, where it may threaten the airway.
Pain or pressure rather than itch
Angioedema is often not itchy. It may be painful, tender, tight, or uncomfortable when the swollen area is stretched. A person can therefore have significant angioedema without the intense itch associated with hives.

With or without hives
Angioedema sometimes accompanies hives, but it can occur without any visible rash. Medication-associated, hereditary, and other forms may have different mechanisms and need different evaluation. Do not assume that every episode of deep swelling is an allergy, and do not assume that the absence of hives makes it harmless.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsWhy the two conditions are easy to confuse
Both conditions can start suddenly and can follow an exposure, illness, or no obvious event. Hives may surround an area that is also swollen, while deeper angioedema can make the nearby skin look raised. Lighting, skin tone, and the amount of swelling can change what you see. The most useful distinctions are depth, sensation, and how each individual lesion changes over time, but none of these can identify the underlying cause by themselves.
For example, itchy welts on an arm that come and go in different places fit a hives pattern. A suddenly enlarged, tender lip without welts fits an angioedema pattern. If both occur, describe both rather than choosing only one label when you contact a healthcare professional.
When to arrange medical assessment, even without an emergency
Seek prompt clinical advice for a first episode of angioedema, severe swelling, swelling that does not improve with the treatment you were told to use, recurrent or unexplained swelling, or symptoms that continue. A clinician may need to review medicines, illnesses, family history, and the timing of each episode. MedlinePlus notes that a cause is often not found, so the lack of an obvious trigger does not make the symptoms imaginary or eliminate the need for assessment.
Ask for an appointment about hives that continue or recur beyond six weeks, or sooner if the episodes are severe or disruptive. Arrange care if the same area repeatedly swells, if symptoms follow a new prescription or dose change, or if you are unsure whether the mouth or throat is involved. If symptoms evolve while you are waiting for an appointment, use the emergency criteria at the start of this guide rather than waiting for the scheduled visit.

Hives
Hives may follow food or medicine exposure, an insect bite, an infection, or an environmental trigger. Some episodes have no identifiable cause. Chronic hives can be inducible: heat, cold, pressure, or rubbing may provoke welts in susceptible people. Finding a trigger is useful, but a person can have genuine hives even after a careful history finds no single cause.

A pattern can be more informative than one suspected exposure. Note whether welts follow pressure from clothing or a bag strap, a temperature change, or rubbing the skin. Do not deliberately repeat a food, medicine, bite, or other exposure to test the theory; a repeat reaction could be more serious.
Angioedema
Angioedema can be associated with allergic reactions, but it also has medication-related, hereditary, and other forms. MedlinePlus specifically identifies ACE inhibitors as medicines that can be associated with angioedema. The mechanism matters because a treatment that helps one form may not solve another. Never stop a prescribed medicine on your own; contact the prescriber or another qualified clinician for instructions.
What to do while arranging care for non-emergency hives
- Remove an obvious, safe-to-remove trigger. Stop a newly introduced cosmetic or avoid a suspected food only if doing so is safe. Do not deliberately re-expose yourself to test a theory.
- Keep the skin cool and reduce friction. A cool compress and loose clothing may make itching more tolerable. Avoid scratching or rubbing, which can aggravate some inducible hives.
- Record the episode. Note when it began, where it appeared, how long each welt lasted, what you ate or took beforehand, recent illness, insect exposure, heat, cold, pressure, or rubbing, and whether swelling or breathing symptoms occurred. Photographs taken in good light can help a clinician see a fleeting rash.
- Use medicines only as directed. A clinician or pharmacist may recommend an antihistamine for hives. Follow the product label and professional advice, and check that the product is appropriate for the person taking it. Do not use an antihistamine as a substitute for emergency care.
Antihistamines: where they fit and where they do not
AAAAI describes antihistamines as initial medication options for hives and names cetirizine, fexofenadine, loratadine, and others. MedlinePlus also says a provider may suggest an antihistamine such as cetirizine or diphenhydramine and advises following provider or package instructions. The correct product depends on the person’s age, health conditions, other medicines, and local labeling, so individualized dosing belongs with a clinician or pharmacist.

Antihistamines are not a universal treatment for angioedema. They may be relevant to some histamine-driven episodes, but medication-associated, hereditary, and other forms can require a different assessment and treatment. If swelling involves the mouth, tongue, throat, breathing, wheezing, or fainting, do not conduct a trial of an over-the-counter product; seek emergency help.
How clinicians sort out the cause
There is no reliable home test that separates every type of hives or angioedema. During an assessment, a clinician may ask:
- When did the first symptom start, and how quickly did it spread?
- Did each individual welt disappear within about 24 hours, or did one fixed area remain?
- Was the main sensation itching, pain, pressure, or tightness?
- Which body sites were involved, especially the lips, tongue, mouth, eyelids, hands, feet, or throat?
- Were there breathing problems, wheezing, fainting, vomiting, or other sudden symptoms?
- What medicines, foods, supplements, insect exposures, infections, heat, cold, pressure, or rubbing occurred beforehand?
- Has this happened before, and does anyone in the family have episodes of unexplained swelling?
Bring a complete medicine list, including nonprescription products, and mention any ACE inhibitor or recent medication change. Because causes are often not found, the evaluation may focus on the pattern and safety plan rather than identifying one definitive trigger.

Photos can be especially useful when the rash has faded by the appointment. Take them in good light from a comfortable distance, and record the date and approximate time. Also note whether swelling progressed, resolved, or returned and whether a new area became involved. These details help distinguish migrating welts from persistent deep swelling without requiring you to make a diagnosis.
A practical decision path
- Look for airway or circulation danger first. Breathing difficulty, wheezing, fainting, throat tightness, or sudden swelling of the lips, mouth, tongue, or throat means emergency care now.
- If there is no emergency sign, identify the dominant pattern. Itchy, raised, shifting welts suggest hives; deeper, painful or tight swelling suggests angioedema. Both patterns may be present.
- Check the timeline. Individual hives commonly fade within 24 hours. Recurrent hives lasting more than six weeks meet the chronic urticaria definition and warrant discussion with a clinician.
- Look for a medicine connection. Report every prescription and nonprescription medicine, especially if swelling began after starting or changing one. Do not discontinue a prescribed drug without medical advice.
- Arrange follow-up for uncertainty or recurrence. A first angioedema episode, severe or persistent swelling, treatment-resistant symptoms, and unexplained repeats all merit prompt assessment.
When in doubt, describe the symptom rather than relying on a label: say whether the area is raised or smoothly swollen, itchy or painful, and whether the change is moving or staying fixed. If the mouth or throat may be involved, treat that uncertainty as urgent rather than waiting for a clearer view.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Questions people commonly ask
Can hives and angioedema happen together?
Yes. Hives may appear with deeper swelling, but either condition can also occur alone. Report both the surface rash and any deeper swelling when seeking care.
Does angioedema always itch?
No. Angioedema is often not itchy and may be painful or tight. Hives are typically itchy, but sensations can vary.
How common are hives?
AAAAI says about one quarter of the general population may have hives during their lifetime. AAAAI does not state a year for that estimate.
Should I stop a medicine that might have caused swelling?
No. Contact the prescriber or another qualified clinician for advice. ACE inhibitors and other medicines can be associated with angioedema, but stopping a prescribed treatment without guidance can create separate risks.

Can I wait at home if my throat only feels slightly tight?
No. Throat tightness is an emergency warning sign because airway swelling can become life-threatening. Call your local emergency number.
What if I cannot find a trigger?
That is common. Hives and angioedema can occur without an identifiable cause. Keep a symptom and medicine record and arrange clinical assessment for a first, recurrent, severe, or persistent episode.
Key points to remember
- Hives are superficial, raised, and usually itchy; individual welts commonly fade within 24 hours without marks or bruising.
- Angioedema is deeper swelling that may hurt or feel tight and can occur with or without hives.
- Throat, tongue, mouth, breathing, wheezing, or fainting symptoms require immediate emergency help.
- More than six weeks of recurring hives fits chronic urticaria and deserves medical advice.
- Causes and treatments differ, so do not assume every episode is an allergy or that an antihistamine treats every type of angioedema.
Frequently Asked Questions
Can swelling around an eye be angioedema if there are no hives?
Yes. Angioedema is deeper swelling and can occur without a visible rash. Eyelid or facial swelling should be assessed promptly, especially if it is severe, recurrent, unexplained, or accompanied by mouth, tongue, throat, or breathing symptoms.
Recommended Free Tools
Why do my hives seem to last longer than 24 hours?
An individual welt commonly fades within 24 hours, but new welts can appear as old ones disappear, making the episode look continuous. If one fixed lesion remains, leaves bruising, or the pattern is uncertain, photograph it and contact a clinician.
What details should I bring to an appointment?
Bring a complete list of prescription and nonprescription medicines, including recent changes, and note foods, infections, insect exposure, heat, cold, pressure, rubbing, timing, locations, sensations, and any breathing or fainting symptoms. Photos can capture a rash that has already faded.
Is an unexplained episode still worth medical evaluation?
Yes. Causes are often not found. A first angioedema episode, recurrent or persistent swelling, severe symptoms, or hives recurring beyond six weeks warrants clinical advice even when no trigger is apparent.
Can an antihistamine treat every kind of angioedema?
No. Antihistamines are initial options for hives and may help some histamine-driven swelling, but medication-associated, hereditary, and other forms can require different assessment and treatment. Airway symptoms require emergency care, not an over-the-counter trial.
The Bottom Line
Hives are itchy, raised surface welts; angioedema is deeper swelling that may be painful or tight and may occur without hives. Individual hives usually fade within 24 hours, while recurring symptoms beyond six weeks are chronic urticaria. Any throat or tongue swelling, breathing difficulty, wheezing, fainting, or sudden mouth swelling requires immediate emergency help. For non-emergency or unexplained episodes, arrange clinical assessment and do not stop prescribed medicines without medical advice.
