Lifespan is how long a person lives. Healthspan is how many of those years are spent in good health, with acceptable function and well-being. A person can live to the same age as someone else yet spend far more of that time with disease, disability or loss of independence. That is why longevity statistics alone do not describe how people experience later life.
For population comparisons, life expectancy is the usual measure of length of life. The World Health Organization’s closest standardized measure of healthspan is healthy life expectancy (HALE): the average number of years a person can expect to live in “full health” under the mortality and health conditions of a particular place and year.

What is the difference between lifespan and healthspan?
Lifespan means the total time between birth and death for an individual. When researchers compare groups, they usually use life expectancy, a population statistic defined by the World Health Organization as “the average number of years a person is expected to live.” It summarizes survival, regardless of whether those years are healthy, disabled or affected by chronic illness.

The word is common in research and public communication, but it has no single universal clinical cutoff. Depending on the study, healthspan may mean years without a specified disease, years without disability, years above a functional threshold, or years rated as healthy by the person.

HALE provides a standardized way to estimate a related idea at population level. WHO defines healthy life expectancy as the “average number of years that a person can expect to live in ‘full health.’” The estimate adjusts total expected survival for time lived in health states that are less than full health.
The distinction matters because longer survival and better health are related outcomes, not identical ones. If mortality falls but disability and chronic disease occur earlier or last longer, life expectancy can rise while the gap between years alive and years in good health also widens.
Lifespan, life expectancy, healthspan and HALE compared
| Measure | What it counts | Typical level | How illness or disability is handled | Does function matter? |
|---|---|---|---|---|
| Lifespan | An individual’s actual time from birth to death | Individual | Not applicable; it ends at death | No health adjustment |
| Life expectancy | Average expected years of life under specified mortality conditions | Population | Counts all surviving years equally | Not directly |
| Healthspan | Years lived in good health, acceptable function and well-being | Individual or population, depending on the study | May exclude or discount disease, disability or poor self-rated health | Usually, but the definition varies |
| Healthy life expectancy (HALE) | Expected years in “full health,” adjusted for health-state severity | Population | Uses morbidity information and discounts less-than-full health | Included through the health states used in the model |
Because these measures answer different questions, one cannot be substituted for another. A life-expectancy figure does not tell you how many years people remain independent, and a healthspan estimate does not tell you the exact age at which any one person will develop a condition.
How HALE is calculated
HALE combines mortality and morbidity information. First, mortality patterns establish how many years people are expected to live. Health data then describe the prevalence or duration of different health states. The calculation applies weights that reflect the severity distribution of those states, reducing the total by time lived in less than full health.
This makes HALE a modelled population estimate rather than a promise about an individual’s future. It is best understood as the expected number of equivalent years in full health if the mortality and health conditions observed for the specified population and year continued.
The adjustment is not a simple subtraction of everyone with a diagnosis. Health states can be short or long lasting and can range from mild limitations to severe disability. A model therefore considers how common states are and how serious they are. Two populations with the same life expectancy can receive different HALE values if one has more years lived with severe limitations. Conversely, a population may have a substantial burden of mild, well-managed conditions that changes HALE less than a smaller burden of disabling illness.
Every HALE number needs a label
Interpretation depends on the details attached to the estimate:
- Geography: A country, region or other population may have different mortality, disease prevalence and access to care.
- Starting point: “At birth” is different from an estimate at age 60 or another age.
- Sex: Where estimates are reported separately, female and male values are not interchangeable.
- Reference year: Health conditions and statistical methods change over time.
- Data and model: Survey methods, mortality records, morbidity definitions and later revisions can alter the result.
WHO notes that modelled estimates can differ slightly from official national life tables. A small difference does not necessarily indicate an error; it may reflect different data sources or methods.
Why two people who reach the same age can have different healthspans
Chronological age records years since birth, not the condition in which those years are lived. Two people may both be 80, while one can move around independently and manage daily activities and the other needs substantial assistance. Both count as having reached 80 in lifespan terms, but their years of health and function differ.

Healthspan also extends beyond the absence of a diagnosis. A person’s experience can include mobility, strength, cognition, emotional well-being, relationships, participation and the ability to carry out valued activities. A diagnosed condition that is well controlled may have less effect on function than an apparently minor problem that causes severe pain or limits movement.
Population averages conceal this variation. A HALE value describes the expected pattern for a group exposed to its particular social, environmental and health conditions; it cannot identify which year an individual will lose a specific ability.

Healthspan and WHO’s healthy-ageing framework
WHO defines healthy ageing as “the process of developing and maintaining the functional ability that enables well-being in older age.” This definition shifts the focus from disease counts alone to what people can actually do and be.
Functional ability depends on three interacting elements:
- Intrinsic capacity: the person’s physical and mental capacities, such as walking, seeing, hearing, thinking and maintaining energy.
- The environment: housing, transport, social support, technology, services and the physical surroundings in which a person lives.
- The interaction between them: an accessible environment can preserve independence when capacity declines, while barriers can create disability even when capacity is relatively strong.
Under this framework, healthspan includes the ability to meet basic needs, learn and make decisions, build and maintain relationships, move around and contribute to society. A narrow count of diagnosed diseases misses these dimensions.
Is healthspan the same as life expectancy?
No. Life expectancy estimates total years of survival. Healthspan describes years in good health or with satisfactory function. HALE is a standardized population estimate that adjusts expected survival for time spent in less-than-full health.

HALE is modelled using weighted health states, not a simple count of years with any diagnosis. The result depends on which conditions are included, how severe states are judged and what data are available.
How many healthy years do people get?
There is no single timeless global answer. Healthy years vary by geography, sex, age, social conditions, health systems, survey design and the definition used. WHO revises estimates and methods, so a figure should always be quoted with its place and year.
For an example of how a dashboard reports a trend, WHO’s Europe display shows healthy life expectancy at birth increasing from 62.7 years in 2000 to 66.7 years in 2023, a four-year increase. Those numbers describe the European geography and the years shown on that dashboard; they are not a universal target or an individual forecast.

Other studies may report disability-free life expectancy, disease-free years, years without a particular condition, cognitive health or physical-function measures. Such values answer narrower questions and should not be presented as interchangeable with HALE.
What the gap between life expectancy and HALE can—and cannot—show
Comparing life expectancy with HALE can indicate how many expected years are lived in less-than-full health on an equivalent-year basis. A larger difference suggests a greater modeled burden of illness or disability, while a smaller difference suggests that more surviving years are expected to be in good health. It does not identify a single age when decline begins, and it does not show whether limitations are concentrated near the end of life or spread across many years.
The gap also cannot rank individuals. Someone may have a chronic diagnosis but retain mobility, cognition and independence, while another person with fewer diagnoses may have severe functional limitations. Nor does the gap reveal which service would help most. Prevention, treatment, rehabilitation, assistive technology, social support and changes to housing or transport can affect function in different ways.
How definitions change the answer
“Healthy” can refer to different endpoints. A disease-free measure might count the first diagnosis of a named condition as the endpoint. A disability-free measure might focus on activities of daily living. A functional measure may track walking, hearing, cognition or the ability to manage a home. A self-rated measure asks people how they judge their own health. These approaches are all meaningful, but they answer different questions.
Even within one endpoint, thresholds matter. A survey may classify a person as limited if assistance is needed for one task, while another may require limitations across several tasks. Changes in wording, sampling or response rates can alter estimates without any real change in people’s health. That is why comparisons should use the same definition and method whenever possible.

How to read a lifespan or healthspan statistic correctly
- Identify the measure. Check whether the figure is lifespan, life expectancy, HALE, disability-free life expectancy or another endpoint.
- Check the starting age. A value at birth includes the risks faced across the entire life course; a value at age 65 describes people who have already reached that age.
- Check the geography. Do not apply a country or regional estimate to every population without qualification.
- Check the year. A number from one year is not a permanent property of a country or a person.
- Check sex and other categories. If the table separates groups, use the value for the group being discussed.
- Read the method note. Look for the mortality source, health surveys, disability definitions, severity weights and whether the estimate is modelled.
- Separate averages from predictions. A population average cannot determine an individual’s exact healthy years, diagnosis date or remaining independence.
Why living longer does not always mean living better
Lower death rates can add years to life without eliminating the conditions that limit those years. People may survive heart disease, cancer, stroke or other illnesses and live for many years with treatment, symptoms or reduced function. In another scenario, prevention and care may delay both death and disability, increasing life expectancy and HALE together.
The policy question is therefore not only how many deaths are prevented, but also whether people retain the ability to move, think, relate to others and make choices. Healthspan puts those outcomes alongside survival rather than treating survival as the sole measure of progress.
What healthspan can mean in research and personal planning
Research definitions
Researchers choose an endpoint that fits the question. A trial may count years free of a named disease; an ageing study may use mobility or cognition; a public-health report may use disability status or self-rated health. These definitions can produce different answers for the same population because they measure different aspects of health.
Personal planning
For an individual, healthspan is useful as a planning concept rather than a guaranteed number. It encourages attention to the abilities and activities that matter personally, such as walking, communicating, managing a home, working, caring for others or participating in a community. No lifestyle action can guarantee a fixed number of extra healthy years, and a population statistic should not be treated as one.

A practical conversation can therefore begin with function: Which activities are essential? Which environments make them easier or harder? What support would preserve choice if capacity changed? This framing avoids reducing healthy ageing to a single score while still allowing measurable goals.
Health and social policy
Governments can use life expectancy to monitor survival and HALE or related measures to examine whether added years are lived in good health. A rising gap may indicate that people are surviving longer with substantial illness or disability; a narrowing gap may suggest that prevention, treatment, rehabilitation or supportive environments are preserving function. Interpretation still requires attention to data quality and changes in the model.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Common misunderstandings
“Healthspan is just lifespan minus years with disease.”
Not necessarily. Healthspan has no universal cutoff, and HALE uses weighted health states. A diagnosis may have little effect on function for one person and a major effect for another, while disability can arise from factors not captured by a simple disease list.
“HALE predicts how long I will stay healthy.”
It does not. HALE is an average for a defined population, place, age and year. Individual outcomes vary, and the estimate may be revised when new mortality or morbidity data become available.
“Healthy ageing means having no medical conditions.”
WHO’s framework emphasizes functional ability and well-being. People can age healthily while living with diagnosed conditions if they retain the capacity and environmental support needed for activities that matter to them.
“A higher life expectancy always proves better health.”
It proves that expected survival is higher under the mortality conditions measured. It does not by itself establish how many of those years are free from disability, cognitive limitation or loss of independence.
A practical checklist for comparing two healthspan figures
- Are both figures the same measure, or is one life expectancy and the other HALE?
- Do they use the same geography, age, sex category and reference year?
- Are both estimates at birth, or are they conditional on reaching a later age?
- Were the health states, disability definitions and severity weights defined in the same way?
- Are the figures official life-table results, modeled estimates or survey-based measures?
- Could a revision in data or methods explain part of the difference?
- Does the comparison concern survival, function, disease-free time or well-being?
Frequently asked questions
Is healthspan measured for an individual?
It can be described for an individual, but there is no single universally accepted personal cutoff. Clinicians and studies may define it using disease, disability, cognition, physical function or self-rated health. HALE is specifically a population estimate and should not be used as an individual forecast.
Does healthspan always end before lifespan?
Healthspan is intended to represent years in good or full health, so it will generally be shorter than total lifespan when a person experiences illness or functional limitation. The exact difference depends on the definition and the severity weights used.
Why do WHO figures change?
Mortality records, health surveys, disease estimates, definitions and statistical models are updated. WHO may also revise earlier years so that a time series uses improved or more consistent methods. Always retain the dashboard’s geography and reference-year labels when quoting a value.
Best Value
Can two countries have the same life expectancy but different HALE?
Yes. Life expectancy counts expected survival, while HALE also reflects the distribution and severity of health states during those years. Similar survival can therefore coexist with different expected years in full health.
Is disability-free life expectancy the same as HALE?
No. Disability-free life expectancy uses a disability endpoint, while HALE combines mortality and morbidity and adjusts for the severity distribution of health states. They may move in similar directions but are not interchangeable.
What should I track personally if I care about healthspan?
Focus on meaningful function and well-being, such as mobility, cognition, communication, relationships, independence and the ability to meet daily needs. These dimensions align with WHO’s healthy-ageing framework, but no personal checklist can convert directly into a guaranteed number of healthy years.

Lifespan tells you how long life lasts; healthspan asks how much of that life is lived in good health and with meaningful function. Life expectancy is the standard population measure of survival, while HALE adjusts expected years for time spent in less-than-full health. To understand any number, keep its geography, starting age, sex, year and method attached to it. The most useful view of longevity considers both the quantity of years and the functional ability that makes those years worthwhile.
Frequently Asked Questions
What is the simplest difference between lifespan and healthspan?
Lifespan is total time alive. Healthspan is the portion of life spent in good health, with acceptable function and well-being.
Is healthspan the same as life expectancy?
No. Life expectancy measures expected survival, while healthspan concerns the quality and function of those years. HALE is WHO’s standardized population measure related to healthspan.
What does HALE stand for?
HALE means healthy life expectancy: the average number of years a person can expect to live in full health under the mortality and health conditions of a specified place and year.
Can HALE predict my own healthy years?
No. HALE is a modeled average for a population, age, sex and year. It cannot predict an individual’s diagnosis, disability or exact years of independence.
Why can life expectancy rise while healthspan does not rise as much?
People may survive longer with chronic disease or disability. Mortality can improve without equivalent gains in function, so added years may include more time in less-than-full health.
How should I compare two healthy-life figures?
Confirm that they use the same measure, geography, starting age, sex category, reference year, health definitions and modeling approach.
The Bottom Line
Bottom line: Longer life and healthier life are different outcomes. Use life expectancy to describe survival and HALE or another clearly defined healthspan measure to describe health and function, always keeping the population, year and method in view.
