Healthcare workforce partnerships connect students to employers by aligning education with real vacancies and then adding supervised workplace experience. Depending on the pathway, a student may shadow staff, complete a required clinical rotation, join a stipend-supported externship, or enter a registered apprenticeship that combines classes with paid on-the-job learning. Colleges teach and support learners; employers provide worksites, mentors and hiring opportunities; sponsors and intermediaries organize the program and its compliance.
What a healthcare workforce partnership does
A partnership is a role-based collaboration rather than a single program type. An employer identifies occupations it needs to fill, such as medical assistant or another allied-health role. An education provider determines which instruction, labs and assessments can prepare learners. The partners then design a route from recruitment to training, supervised practice, credentials and employment.

The route can be informal, such as a short job-shadowing visit, or formal, such as a Registered Apprenticeship. The important question is not whether a program uses the word “partnership,” but what each organization is responsible for and what the student receives in return.

| Partner | Typical responsibility |
|---|---|
| Employer or healthcare provider | Defines job competencies, hosts workplace learning, assigns supervisors or mentors, and may hire and pay learners. |
| College, university or other educator | Delivers classroom, laboratory and standards-driven instruction; may provide credentials, tutoring, counseling, financial-aid access and outreach. |
| Registered Apprenticeship sponsor | Assumes responsibility for operating and administering the apprenticeship. A sponsor can be one employer, an employer consortium, a workforce intermediary, a community college or a community-based organization. |
| Apprenticeship intermediary | Helps businesses create, launch and expand apprenticeship programs, often handling shared administration for employers that lack internal capacity. |
| Workforce board or public agency | Can direct workforce-development funding and coordinate regional training priorities. |
| Community organization or school district | May recruit students, provide early career exposure and connect the pathway to local support services. |
The U.S. Department of Labor describes an apprenticeship intermediary as “an organization with the capacity, expertise, and network to help businesses successfully create, launch, and expand apprenticeship programs.” Apprenticeship.gov explains the partner roles and sponsor responsibilities.

How the connection works from school to a job
- Start with an employer need. Employers identify vacancies, turnover problems or new skills they cannot fill reliably. They specify the job duties and competencies a learner must demonstrate.
- Match instruction to those competencies. Educators map courses, laboratory work and assessments to the occupation. Partners decide whether learners need a clinical rotation, an externship, an apprenticeship or a shorter exposure activity.
- Set eligibility and recruitment rules. The group decides who can apply, what prerequisites apply, how applicants are screened and which organization communicates with students. Recruitment may involve schools, colleges, community groups or an intermediary.
- Define the workplace experience. The plan states the setting, hours, supervision, mentor duties, permitted tasks and safety requirements. This is where a brief shadowing visit differs from a required clinical practice or paid apprenticeship.
- Assign payment and cost responsibilities. Partners document who pays wages or stipends, who pays tuition and supportive services, and whether the learner receives financial aid. Work-based learning is not automatically paid.
- Document progress and compliance. Sponsors and educators track attendance, competencies, evaluations, credentials and any reporting required by the occupation or apprenticeship system.
- Connect completion to hiring or advancement. The employer may hire the learner before training starts, after a placement, or when the learner earns the required credential. Some pathways also provide advancement for incumbent workers.
These design decisions vary by occupation, employer and state. Before enrolling, ask for the written pathway description rather than assuming that a program with a familiar label has the same pay, hours or hiring commitment as another.
Which pathway is being offered?
Clinical practice, shadowing, externships and Registered Apprenticeships all place learners near real work, but they are not interchangeable. Compare them by pay, purpose, duration, supervision, hiring timing, credential and funding.
| Pathway | What the learner does | Pay and duration evidence | How it connects to employment |
|---|---|---|---|
| Clinical rotation or clinical practice | Performs supervised practice in a clinical setting, commonly as part of an education or licensing requirement. | In a 2022 Department of Labor study of healthcare regional partnerships, clinical practice commonly lasted 75–100 hours. Placements were often unpaid unless they were part of an apprenticeship or incumbent-worker training; those findings describe the studied partnerships, not a universal rule. | Builds required experience and employer references; hiring may occur later and is not inherent in every rotation. |
| Job shadowing | Observes workers in a real setting for a short period. The Compton partnership lists shadowing among employer-hosted activities. | The Compton page does not specify a standard duration or payment. | Provides career information and contacts, but observation alone is not a job offer or a qualification. |
| Registered Apprenticeship | Combines structured classroom instruction with practical, hands-on training and a mentor. | Paid on-the-job learning is a defining feature of the model described by the Department of Labor. Length and occupation-specific requirements vary; the studied programs commonly led to nationally recognized credentials, often journeyperson status. | Employers can train workers against their own standards while the apprentice earns experience, wages and a recognized credential. |
| Clinical externship | Completes supervised clinical work within a specific fellowship or training model. | Propel America describes its medical-assistant fellow externship as including stipends and coaching. That is a feature of Propel’s model, not an industry-wide standard. | Combines a worksite experience with coaching and an employer relationship that may support hiring. |
The Department of Labor’s regional-partnership report is the source for the 75–100-hour clinical-practice figure and the observation that many placements were unpaid. Read the 2022 Employer Engagement Strategies in Regional Partnerships report.

Why Registered Apprenticeship is different
Registered Apprenticeship makes the school-to-work connection explicit. Academic preparation is paired with paid on-the-job learning, practical training and a mentor. The sponsor is accountable for operating the program, while educators and employers carry out their respective instructional and workplace roles.
At the postsecondary level, colleges and universities can combine academic preparation with practical training and mentorship, creating a direct link between a course of study and an occupation. Educators may also help apprentices obtain credentials, access financial aid, receive tutoring and counseling, and stay engaged through outreach. The Department of Labor’s postsecondary-education guidance details these education-partner functions.
Who employs the apprentice?
The answer depends on the model. A single health system can sponsor and employ apprentices itself. An employer consortium can share a program. An intermediary can sponsor and administer the program while participating employers employ and train the apprentices. In every case, ask which legal entity issues the paycheck, who supervises daily work and who is responsible for records.

What the apprentice earns
An apprenticeship should specify the wage, the schedule for any wage progression, classroom hours and the credential attached to completion. “Training” by itself does not establish that a learner is paid; the paid work component is tied to the Registered Apprenticeship structure, while a separate clinical placement may be unpaid.
Examples of partnership models
Compton Community Health Professions Partnership in California
Compton College identifies a network that includes Compton Unified School District, Compton College, CSU Dominguez Hills, Charles R. Drew University, local healthcare providers, community organizations and policy partners. Its employers host clinical rotations, job shadowing and apprenticeships. The example shows how a regional pathway can span school, college, university and healthcare settings, but it does not establish that every California region offers the same activities. See Compton College’s partner description.
Texas community health-center medical-assistant apprenticeship
A 2024 Texas Healthcare Workforce Task Force report describes a model in which the Texas Association of Community Health Centers serves as sponsor and administrative support, while individual health centers employ and train apprentices. This division lets smaller employers participate without each building a complete apprenticeship office.

Propel America as an intermediary example
Propel America describes services that connect talent, training and employers through apprenticeships, externships, recruiting and post-hire coaching. On its healthcare-employer page, Propel reports that its participants were 60% more likely to stay in a role after one year, that 92% felt prepared for a first job, and that 91% of alumni who continued education remained in healthcare. Those figures are reported by Propel from its program data and Alumni Survey; the page does not establish an independent comparison or prove that the program caused the outcomes. Review Propel America’s description and its stated outcomes.
Questions students should ask before accepting a placement
- Is it paid? Ask whether compensation is an hourly wage, a stipend, reimbursement or nothing, and when payment begins.
- What exactly is the activity? Confirm whether it is a required clinical experience, job shadowing, an externship or a Registered Apprenticeship.
- Who is responsible? Identify the employer, apprenticeship sponsor, education provider and any intermediary. Ask who handles payroll, insurance, attendance and complaints.
- Who supervises me? Request the supervisor’s role, mentor contact and rules for tasks you may perform.
- What credential or license requirement does it satisfy? A placement may provide useful experience without counting toward a local credential or licensure requirement.
- What happens after completion? Ask whether there is a hiring commitment, a recognized credential, wage progression or a pathway to further education.
- What support is available? Check for tutoring, counseling, transportation or childcare assistance, financial-aid guidance and help resolving schedule conflicts.
- How much time is required? Get classroom hours, workplace hours, start and end dates, attendance rules and any shift or weekend expectations in writing.
Implementation challenges for employers and educators
Administrative capacity
Registered Apprenticeship setup can be lengthy and complex. Sponsors must coordinate standards, documentation, mentor arrangements, progress records and reporting. An intermediary can centralize those tasks, but participating employers still need staff who can train, supervise and evaluate learners.
Best Value
Clinical and simulation capacity
Healthcare programs compete for clinical sites, qualified supervisors and safe opportunities for students to practice. Simulation can expand training capacity, yet labs require room, specialized equipment and ongoing upkeep. Partners should budget those costs before promising placements.
Alignment without overpromising
Employers should define competencies they actually use, while educators should preserve required academic and licensing standards. A partnership works best when the job description, curriculum, workplace tasks and assessment criteria use compatible language. Neither a shadowing visit nor an unpaid rotation should be marketed as guaranteed employment unless the employer has made that commitment.
How to evaluate a partnership’s quality
- Trace the full route. Can a prospective student see recruitment, instruction, workplace learning, assessment and the next step after completion?
- Verify compensation. Is pay or a stipend documented, and does it cover every workplace hour?
- Check supervision. Are mentors trained, available and authorized to assign the learner’s tasks?
- Confirm credentials. Does completion produce a credential or satisfy a requirement recognized by the relevant school, licensing body or employer?
- Test the support system. Are tutoring, counseling, financial-aid help and communication channels available when work and school collide?
- Review employer accountability. Is there a named sponsor or administrator, a process for resolving problems and a clear statement of who hires?
- Look for transparent outcomes. Retention or employment statistics should identify who collected them, the population measured, the date and whether they are independently verified.
Bottom line for students
Healthcare workforce partnerships are bridges, not guarantees. They can turn classroom learning into supervised experience and an employer relationship, but the value depends on the pathway’s design. A Registered Apprenticeship normally combines paid work, instruction and mentorship; a clinical rotation may be required yet unpaid; shadowing may offer exposure without a credential; and an externship’s stipend or coaching depends on the sponsoring organization. Read the written terms, identify every responsible partner and confirm pay, supervision, credential and hiring expectations before you commit.

Are healthcare clinical placements always paid?
No. The Department of Labor’s 2022 study found that clinical placements in the healthcare partnerships it examined were often unpaid, except when offered through an apprenticeship or incumbent-worker training. Local rules and program terms control.
Does completing a healthcare partnership guarantee a job?
No. Some employers hire during or after training, but a rotation, shadowing activity or externship is not automatically a job offer. Ask for any hiring commitment in writing.
Who should I contact if an apprenticeship’s roles are unclear?
Ask the education provider for the named Registered Apprenticeship sponsor, the employing healthcare organization, the workplace supervisor and the organization handling payroll and records.
