EDR (endpoint detection and response) is a security capability focused on activity on covered computers and other endpoints; MDR (managed detection and response) is a service in which a provider monitors, investigates, and may respond to threats. They are not mutually exclusive: an MDR provider may use or manage an organization’s EDR tools. For healthcare organizations, the choice is less “EDR or MDR” than what systems are covered, who handles alerts, and which response actions are safe and authorized in a clinical environment.
What EDR and MDR mean
| Question | EDR | MDR |
|---|---|---|
| What is it? | An endpoint-focused technology capability for detecting suspicious activity and supporting response on covered devices. | A managed security service that supplies some combination of monitoring, investigation, threat hunting, and response. |
| Who operates it? | The healthcare organization, an outside provider, or a combination may operate the EDR platform and handle its alerts. | A service provider performs the activities specified in its contract; the organization may still retain security responsibilities and staffing needs. |
| What data does it use? | Primarily endpoint telemetry, depending on the product and configuration. | Endpoint data and potentially identity, network, cloud, email, or other sources. The actual sources depend on the service. |
| Who can take action? | Actions depend on the product configuration and the organization’s procedures. | Authority varies: a provider may recommend actions for approval or be permitted to take specified actions directly. |
| How do they relate? | Can be used by an in-house team or managed or augmented by a service provider. | May incorporate EDR, but MDR is not synonymous with a particular EDR product. |
These are practical category distinctions, not a universal MDR service definition. HHS healthcare guidance discusses endpoint protection and incident response, but does not establish one standard set of MDR features. The scope, hours, supported technologies, and response authority must be confirmed with each provider.

How EDR and MDR can fit into a healthcare security program
EDR operated by an internal team
An organization can deploy EDR and have its own staff review alerts, investigate incidents, and coordinate response. This can provide direct control over decisions, but the organization must have the people, processes, and coverage needed to operate the capability. EDR does not by itself guarantee that every device is covered or that alerts will be handled promptly.
MDR managing or augmenting endpoint detection
An MDR provider can supply monitoring and investigation capacity around endpoint tools, including EDR. This may suit an organization that needs outside operational support, but the contract determines whether monitoring is continuous, which data sources are reviewed, what reports or threat hunting are included, and whether the provider can act without prior approval.

A combined model
A healthcare organization may retain internal ownership of security decisions while an MDR provider performs defined monitoring or investigation tasks. Before adopting this model, identify who owns each alert, who contacts clinical and IT teams, who approves containment, and how handoffs work outside normal business hours. Do not assume that buying a managed service removes the need for internal incident coordination.
Medical devices require a separate operational check
Connected clinical equipment cannot always be treated like a standard workstation. HHS describes medical devices as a specialized class of IoT and encourages healthcare organizations to adapt relevant cybersecurity practices to device management. Its healthcare guidance does not establish that every device can support an endpoint agent or tolerate the same containment action as an ordinary computer. See HHS’s Health Industry Cybersecurity Practices (HICP).
When evaluating endpoint coverage or a managed service, ask how it handles devices that cannot run supported software, are managed by a manufacturer, or could be disrupted by isolation or configuration changes. Define a clinical escalation path before enabling automated containment. A technically fast response is not necessarily a safe response if it interrupts patient care.

What HHS guidance and HIPAA do—and do not—say
HHS healthcare guidance recommends adding EDR to detect and mitigate cyber threats. That is a healthcare recommendation, not proof that every endpoint can run an agent or that EDR alone is sufficient. HHS also identifies endpoint protection as one practice and security operations and incident response as another, underscoring that endpoint tooling and the work of operating a security program are related but distinct. See the HHS EMR/EHR guidance and HHS HICP.
The HIPAA Security Rule requires appropriate administrative, physical, and technical safeguards for electronic protected health information (ePHI). The reviewed HHS overview of the Security Rule does not specify EDR or MDR as a required product or service. HHS’s page lists a proposed Security Rule update dated January 6, 2025; a proposal should not be treated as binding. Check the current rulemaking record before relying on any proposed provision.
HHS’s Healthcare and Public Health Cybersecurity Performance Goals are voluntary prioritization guidance, not a regulation. They include detecting relevant threats and tactics at endpoints. A voluntary goal can inform a security program, but it should not be described as a legal mandate.

Coverage and clinical compatibility
- Which workstations, servers, remote endpoints, operating systems, and clinical environments are included? Get exclusions and unsupported systems in writing.
- How are connected medical devices monitored or protected when an agent is unsupported or could interfere with operation?
- How are coverage gaps identified, reported, and addressed as devices and systems change?
Monitoring, data, and investigation
- Who reviews alerts, at what hours, and who owns escalation? Identify any internal staffing that remains necessary.
- Which sources are monitored: endpoint events only, or also identity, network, cloud, email, and other logs? Confirm integrations with existing security tools and ticketing workflows.
- What investigation evidence, timelines, incident reports, threat hunting, and post-incident support are included?
Response authority and service commitments
- Can the provider isolate an endpoint or disable an account directly, or does it recommend action for the organization to approve?
- Set severity definitions, notification windows, response targets, escalation contacts, service availability, and any emergency exceptions in the contract.
- Document how clinical teams are consulted when a response could affect patient care or device availability.
Privacy, cost, and operational ownership
- Determine what data the provider handles and which contractual, privacy, and security terms apply. A vendor’s marketing label is not evidence of compliance.
- Compare total costs over the same period: licensing, implementation, tuning, retained internal staffing, service fees, and incident-response charges.
- Assign internal owners for alert escalation, risk acceptance, clinical coordination, and incident decisions, even when a provider performs monitoring.
Healthcare threat figures are context, not an EDR/MDR scorecard
HHS’s Hospital Resiliency Landscape Analysis describes an analysis of threats and cybersecurity capabilities at U.S. hospitals. The page reports the following figures, but does not state the publication year for these statistics or provide all denominators; treat them as page-reported landscape context rather than a current, fully specified survey or proof that EDR or MDR alone prevents attacks.
| Figure reported by HHS | Qualification |
|---|---|
| 71% of attacks were human-directed | HHS landscape page; publication year not stated. |
| 112% increase in access-broker theft used by human-directed attacks | HHS landscape page; publication year not stated. |
| 1 hour 28 minutes to move off an initial intrusion point | HHS landscape page; publication year not stated. |
| Over 90% of surveyed hospitals reported MFA adoption | HHS landscape page; publication year not stated and the denominator is not fully specified there. |
| 89% of surveyed hospitals reported regular vulnerability scanning at least quarterly | HHS landscape page; publication year not stated and the denominator is not fully specified there. |
| 86% of surveyed hospitals reported that users were informed and trained on cybersecurity duties | HHS landscape page; publication year not stated and the denominator is not fully specified there. |
| 49% of hospitals reported adequate supply-chain risk-management coverage | HHS landscape page; publication year not stated and the denominator is not fully specified there. |
The same HHS page marks endpoint protection, identity and access management, network management, vulnerability management, and security operations and incident response among areas needing urgent improvement. That breadth is a reminder that endpoint detection is one part of a wider security program.
Which approach makes sense for your organization?
- Consider an internally operated EDR capability if the organization can staff alert review and response, manage coverage and tuning, and coordinate investigations with clinical and IT teams.
- Consider MDR support if the organization needs outside monitoring or investigation capacity, after confirming service hours, data sources, response permissions, and escalation arrangements.
- Consider a combined arrangement if internal teams need to retain operational authority while a provider supplies defined monitoring or response support.
In any model, evaluate endpoint coverage alongside medical-device constraints, incident-response procedures, and the organization’s broader safeguards for ePHI. The right comparison is between the capabilities and responsibilities actually delivered—not the labels “EDR” and “MDR” alone.
