Before choosing travel insurance for a move to a Greek island, find out which country’s social-security system is responsible for your healthcare and which registration route applies to your situation. A European Health Insurance Card (EHIC) is for medically necessary care during an eligible temporary stay; it is not a substitute for arranging healthcare cover when you take up habitual residence in Greece.
Start with your residence and coverage status
The route depends on your nationality, residence status, work, pension and the country responsible for your social-security coverage. If you are moving, confirm your entitlement with the competent insurer or Greek National Contact Point rather than assuming that an EHIC will cover routine care after the move.

If you are an EU citizen moving to Greece
Greek National Contact Point guidance gives different examples for different circumstances. Some posted workers and pensioners whose healthcare remains the responsibility of another EU country may use an S1 to register entitlement in Greece. If you take up employment in Greece, you generally need to register with Greek social insurance, usually EFKA. Someone receiving unemployment benefits from another EU country while seeking work in Greece may use an EHIC for medically necessary care only during the period those benefits apply. These are examples, not a determination of your individual case. Check your route with the responsible authority. Greek National Contact Point for Cross-border Healthcare

EU citizens affiliated through an S1 are expected to obtain and use an EU-AMKA at the point of service. Confirm the registration and documentation steps for your case before relying on coverage.

If you are not an EU citizen
The requirements can depend on nationality, visa and residence status. The available Greek guidance includes AMKA documentation rules for certain third-country nationals and public-access provisions for legally resident uninsured people and specified vulnerable groups; it does not establish one universal rule for all newcomers or guarantee immediate routine-care coverage. Check current residence-permit, AMKA and insurance requirements with the relevant Greek authority and insurer. Greek AMKA portal
If you are visiting before the move
An EHIC can provide eligible holders access to medically necessary public-system care during a temporary stay in the EU, EEA or Switzerland, under the rules and cost-sharing that apply in the country visited. Care is not necessarily free. The card does not cover treatment sought as the purpose of travel, and it does not itself arrange healthcare registration for a change of habitual residence. Know what EHIC covers—and what it does not
EHIC is useful for medically necessary care during a temporary stay, but it is not comprehensive travel insurance or a replacement for resident coverage. The Hellenic National Contact Point states: “The EHIC is not an alternative to travel insurance.” EOPYY guidance on the EHIC

- It does not cover private healthcare outside the applicable public system.
- It does not cover treatment that is the reason for travelling.
- It does not generally cover return travel, rescue or medical repatriation costs.
- It does not guarantee treatment will be free; local patient charges may apply.
For any supplemental policy, check eligibility for your country of residence, destination, dates and intended length of stay. Compare emergency and hospital cover, pre-existing-condition rules, exclusions, deductibles and co-payments, and whether you must pay upfront or can use direct billing. Read the terms for medical evacuation—including inter-island or island-to-mainland transfer—rescue and repatriation, and confirm who arranges transport and whether approval is required. These are comparison points, not assurances that a particular policy provides those benefits. Ask the insurer to confirm cover in writing for your specific residence status, island and medical needs.

Use the right kind of provider in Greece
The Greek contact point describes care through public ESY facilities and private doctors, clinics and diagnostic laboratories contracted with EOPYY. For EHIC care, check that the provider participates in the public system or is EOPYY-contracted, and confirm that it can see you before attending. The contact point says non-contracted private care is not reimbursable under Greek national law for EHIC holders. Urgent care guidance for EHIC holders
Plan for emergencies and island transfers
For emergency ambulance transport in Greece, call 112 or 166. EKAV emergency ambulance transport takes patients to a public on-call hospital and is free; an ambulance operated by a private clinic may be charged. The contact point says EKAV air ambulance transport is free in most cases, including some urgent, life-threatening transfers from islands to the mainland, but exceptions may apply. Do not assume this covers every transfer or resolves any insurance claim.
Before settling on an island, identify the local public hospital and understand how to reach emergency services. Available guidance does not establish island-by-island hospital capacity, specialist availability or response times, so check arrangements for the specific island with local providers.
Keep records if you pay for care
If you pay for treatment, retain itemised invoices, prescriptions, diagnosis and treatment reports, and hospital admission and discharge documents. Whether you can claim reimbursement depends on your coverage route and the rules of the responsible health system. Do not assume that paying a bill makes it reimbursable.

Planned treatment is a separate route
If you are travelling to Greece specifically to receive treatment, distinguish the planned-care routes from urgent care during a temporary visit. The applicable route depends on your home system and the treatment.
| Route | How it works | Provider and payment |
|---|---|---|
| S2 / E112 | Requires prior authorisation from the responsible statutory insurer before treatment. | Applies to public or EOPYY-contracted care under Greek benefit rules. |
| Directive 2011/24/EU | Generally allows you to choose treatment and then seek reimbursement; prior authorisation can apply to some services. | You generally pay upfront and reimbursement is limited to the home-country tariff for care you are entitled to there. Public or private providers may be used. |
Check the authorisation, entitlement and reimbursement rules with your insurer before booking. The Greek National Contact Point explains the options for planned healthcare in Greece.
Check these items before the move
- Ask the competent insurer which country’s social-security system is responsible for your healthcare after the move.
- Confirm whether you need an S1, Greek social-insurance registration, EU-AMKA or another route for your status.
- Check how your intended providers are classified: public ESY, EOPYY-contracted or non-contracted private.
- Ask the insurer whether you need supplemental long-term medical or travel cover, and get written confirmation of eligibility and benefits for your dates and destination.
- Save emergency numbers, local hospital details and policy claims instructions where you can access them offline.
Greek Ministry of Health guidance describes public access for legally resident uninsured people and specified vulnerable groups, and says everyone—irrespective of legal status—retains access to hospital emergency departments. The page dates to 2016, while a Ministry FAQ search result points to a 2025 circular; verify current procedures and eligibility locally before relying on this for routine care.
