Rest from sport and protect the leg at first; seek medical advice for severe or worsening symptoms; then resume gentle movement and rehabilitation gradually as pain allows.
What a hamstring injury can feel like
A sudden sharp pain at the back of the thigh, sometimes with a popping sensation, followed by tenderness, swelling or bruising can indicate a hamstring injury. Difficulty standing or walking may signal a more serious injury. Symptoms alone cannot tell you exactly which structure is injured or how severe it is.

A severe injury can involve a tendon pulling away from the bone, rather than only a strain in the muscle. A pop, pronounced bruising, inability to use the leg, or pain high near the sit bone deserves careful assessment; these signs do not confirm a tendon avulsion. A clinician may use an examination and imaging such as an X-ray or MRI to assess the injury. AAOS OrthoInfo notes that severe tendon injuries may need specialist treatment and, occasionally, surgical reattachment.

What to do in the first two to three days
For a mild to moderate injury, the NHS recommends PRICE during the first two to three days: protect, rest, ice, compression and elevation. NHS hamstring injury guidance advises using an ice pack or frozen item wrapped in a cloth for up to 20 minutes every two to three hours. Never put ice directly on your skin.
#1 Best Overall
- Tights vs Sleeves: Get targeted relief for problematic areas rather than an all-over "squeeze" of the entire leg! This is extremely helpful for pulled hamstrings and overworked quadriceps to promote healthy blood flow, ensure optimized performance and minimize the risk of injury.
- Enhances performance by promoting blood flow, reducing muscle vibrations, and decreasing fatigue while increasing structural support and combatting the annoying effects of DOMS. Compression is also beneficial for healing damaged or torn muscles after sudden injuries.
- Wearing the BODYPROX thigh sleeves during heavy lifts can help with mind and body connection, helping to "engage" your muscles during training sessions. They ensure your muscles are getting the oxygen they need to function properly while simultaneously preventing lactic acid build-up in your muscles.
- Great to wear on and off the track! Thigh compression sleeves fit well under clothing so you can prevent and protect against further injury while at work or around the house.
- Made with a knitted blend of Nylon and Latex with 10% Spandex for extreme stretch that releases excess body heat and prevents skin irritation. Silicone waves keep sleeves in place. Comes in 4 sizes: measure the circumference at the top of your thigh to find your best fit!
- Protect: Avoid activity that aggravates the injury or risks another pull.
- Rest: Stop sport and reduce activity initially. Rest is protection while symptoms are acute, not a reason to keep the leg immobile indefinitely.
- Ice: Apply a wrapped ice pack for up to 20 minutes at a time, repeating every two to three hours while awake.
- Compression: If comfortable, use a tubular or elastic bandage snugly, not tightly. Remove it before sleep.
- Elevation: Raise the leg when practical to help limit swelling.
For the first three days, the NHS also advises avoiding heat, massage and alcohol, which can worsen swelling. If pain no longer stops you from moving the leg, begin gentle movement rather than avoiding all movement. NHS advice puts it simply: “When you can move your leg without pain stopping you, try to keep moving it so your hamstring does not become stiff.” Too much avoidance may contribute to muscle shrinkage and scar tissue, while strenuous exercise too soon can worsen the injury, according to nidirect.
When to get medical advice
Seek urgent advice if any of the following apply. In England, the NHS directs people with these symptoms to NHS 111; elsewhere, use the local urgent-care or medical advice service.

- The pain is very bad or getting worse.
- Swelling or bruising is substantial or increasing.
- It hurts to stand or walk on the affected leg.
- The leg is very stiff or difficult to move.
- Self-care is not helping.
- You have a high temperature or hot-and-cold shivers alongside the injury.
Inability to use the leg, marked bruising, or pain high near the sit bone also warrants prompt assessment because a tendon injury can require specialist care. Do not try to diagnose an avulsion yourself.

As the initial pain and swelling settle, build back movement gradually. NHS and AAOS guidance describes moving from gentle stretching toward activities such as walking, cycling and strengthening. A physiotherapist can tailor the plan, particularly after a severe, recurrent or slow-to-heal injury. AAOS says physical therapy aims to restore range of motion and strength, typically starting with flexibility and adding strengthening gradually after acute symptoms settle.

There is no one-size-fits-all exercise sequence. For athletes, the 2022 JOSPT/AOPT clinical practice guideline and an international consensus identify exercise choice and dosage, progression criteria, running and sprinting, and return-to-sport criteria as key parts of rehabilitation. The plan should prepare you for the actual demands of your sport or activity. Imaging can help classify an injury, but the consensus says it should not be used on its own to decide when an athlete can return.
How long does a pulled hamstring take to heal?
Recovery depends on injury severity and the person. NHS guidance says a mild hamstring injury often improves in a few days, while a severe one may take weeks or months. AAOS OrthoInfo notes that a complete grade 3 muscle tear can take months to heal. After proximal tendon repair, rehabilitation typically takes at least six months; after distal repair, it may be about three months before athletic activity. These are broad ranges, not a personal forecast or a clearance date.

For context, general NHS advice for sprains and strains says many feel better after two weeks and advises avoiding strenuous exercise such as running for up to eight weeks; severe cases can take months. Those broad estimates are not specific to hamstring injuries and should not be treated as a return-to-sport rule. NHS sprain and strain guidance provides that general advice.
When can you return to sport?
Do not use a calendar estimate alone to decide. Return gradually, based on recovered function and, when appropriate, a clinician’s or physiotherapist’s guidance. A 2018 peer-reviewed review reported that nearly one third of hamstring strains recur within the first year after return to sport. That is a population-level finding, not an individual prediction; the authors discuss reinjury susceptibility and possible premature return or inadequate criteria as reasons to take progressive rehabilitation seriously. The review supports treating readiness as more than simply waiting until pain fades.
