A systematic review published in BMC Oral Health on 3 October 2026 found generally favorable antibacterial results and improvements in selected mechanical tests, but the evidence comes overwhelmingly from laboratory experiments—not patient treatments.
The review included 25 studies: 24 in vitro investigations and one animal study. Because the particles, synthesis methods, concentrations, cements and test protocols differed substantially, the authors did not calculate one pooled treatment effect. Clinical studies are still needed to determine whether any formulation makes restorations last longer, reduces recurrent decay or improves patient safety.

What the 2026 review actually found
Mohamed and colleagues searched studies published through January 2025 and extracted results for antibacterial activity, mechanical performance, fluoride release, color stability and other physicochemical properties. Sixteen of the 25 studies tested antibacterial activity, while 11 assessed mechanical outcomes. Eleven of the 24 in vitro studies were judged to have low risk of bias and 13 medium risk; the single animal study had moderate overall risk of bias.

It does not identify a universally superior nanoparticle or prove that nanoparticle-modified GIC performs better in people.

Do the nanoparticles make GIC stronger?
Sometimes, depending on the formulation and test. Selected hydroxyapatite systems and chitosan–titania–zirconia–hydroxyapatite combinations showed favorable mechanical findings in laboratory testing. Researchers measured outcomes such as compressive or flexural strength and microhardness, but results cannot be compared as though every study used the same cement or dose.
Effects were not uniformly positive. Some silver nanoparticle formulations showed reduced microhardness or surface irregularities. A higher particle loading can change handling, dispersion and the cement’s setting structure, so adding more nanoparticles is not automatically better.

Can modified GIC fight oral bacteria?
Antibacterial activity was the most frequently examined outcome, and the review found generally favorable results for selected formulations. Plant-mediated silver particles produced comparatively consistent antibacterial enhancement across the included studies, although their mechanical effects varied.
These findings usually come from laboratory assays that expose specimens to selected organisms for a defined period. Such tests indicate antimicrobial potential; they do not establish prevention of secondary caries, protection against a patient’s complete oral microbiome or longer restoration survival.

Which green-synthesized particles have been tested?
| Particle or system | Example of the research approach | What was measured or reported |
|---|---|---|
| Hydroxyapatite | One study used calcium oxide derived from eggshells and Moringa oleifera leaf extract to synthesize particles before incorporating them into GIC. | Antimicrobial activity and compressive strength in the laboratory. |
| Titanium dioxide (TiO2) | A 2026 Scientific Reports study prepared TiO2 with Citrus aurantium seed extract. | In vitro mechanical and physical tests plus preliminary in-silico safety screening; in-silico screening is not a clinical safety trial. |
| Zinc oxide and hydroxyapatite | A separate reinforced-GIC study evaluated a combination of these materials. | Mechanical strength, biological activity and antibacterial properties; the available study record does not establish patient benefit. |
| Silver, copper, zirconia and gold | The systematic review includes plant-mediated and other green-synthesis routes for these particles. | Antibacterial and mechanical outcomes varied by composition, dose and test method. |
| Chitosan-based hybrids | Some formulations combined chitosan with titania, zirconia and hydroxyapatite. | Several selected systems showed favorable mechanical or antibacterial findings in vitro. |
Why the results cannot be reduced to one “best” cement
Nanoparticle loadings ranged from 0.05% to 10% by weight. Studies differed in whether particles were mixed into the GIC powder, liquid or both; how particles were synthesized and characterized; the commercial or laboratory cement used; specimen preparation; bacterial strains; aging time; and mechanical-test procedures.
For a meaningful comparison, a formulation needs to be described across all of those dimensions. A strong result at one concentration and test duration may not transfer to another cement or to a restoration exposed to saliva, chewing forces and temperature changes.

What is known about fluoride release and appearance?
Fluoride-release evidence is especially limited: only two studies in the review evaluated it. Four studies assessed color stability. Higher nanoparticle loadings sometimes caused noticeable discoloration, an important limitation for tooth-colored restorations. The available evidence does not show that green synthesis preserves fluoride release or appearance across GIC formulations.
What “green-synthesized” means—and what it does not mean
Green synthesis generally refers to using biological materials such as plant extracts, eggshell-derived compounds or other lower-chemical-input routes to produce nanoparticles. It describes how the particles were prepared, not proof that the final cement is safer, environmentally sustainable at manufacturing scale or clinically effective.

Residual chemicals, particle aggregation, release from the set cement, dose and tissue exposure still require direct characterization. A computational safety screen can suggest molecules or interactions worth studying, but it cannot replace toxicology, animal safety work or controlled human trials.
What evidence would be needed before clinical claims?
- Standardized formulation reporting: Researchers should specify particle identity, synthesis route, size and surface chemistry, concentration and whether the powder or liquid was modified.
- Comparable testing: Studies need agreed protocols for compressive and flexural strength, microhardness, antibacterial assays, fluoride release, color change and aging.
- Longer durability studies: Specimens should be challenged with moisture, thermal cycling, chewing-like fatigue and clinically relevant biofilms.
- Biocompatibility testing: Cell, animal and release studies must establish safety at realistic exposures.
- Clinical trials: Randomized, adequately followed restorations are necessary to measure survival, recurrent caries, postoperative symptoms, appearance and adverse effects.
What patients and clinicians should conclude now
The most defensible conclusion is that green-synthesized nanoparticles are a promising experimental strategy for tuning GIC properties. Laboratory studies suggest antibacterial gains and selected mechanical improvements, while also showing formulation-specific drawbacks such as lower microhardness, surface irregularities or discoloration.
The 2026 review does not establish a proven longer-lasting restoration, prevention of recurrent caries or superior clinical safety. Its database search ended in January 2025, and the evidence available at publication remained primarily in vitro. Experimental nanoparticle formulations should therefore not be treated as validated commercial replacements for established restorative materials.
