For higher-risk agentic use cases, the guidance calls for more intensive supervision and clear, risk-based points at which an AI agent must obtain human approval.
What the FSB’s guidance says
The FSB published its final Sound Practices for Responsible Adoption of Artificial Intelligence on 15 October 2026. It is nonbinding guidance and a toolkit, not a single new financial-services statute. Institutions are expected to adapt it to their business model, risk appetite and jurisdiction, while applying existing rules on financial conduct, outsourcing, cybersecurity, consumer protection and artificial intelligence.

The 12 practices cover organization-wide governance and the full AI lifecycle. They include board and senior-management accountability, documented responsibilities, use-case assessments, explainability, data governance, performance evaluation, cyber and information-and-communications-technology resilience, third-party dependency controls, and continuous adaptation as systems and threats change.

The final text gives agentic AI more prominent treatment than earlier drafts. Reuters reported that it calls for “more intensive human oversight” in higher-risk cases and for “clear, risk-based triggers for when an agent must seek human approval.” The FSB also removed language that described AI systems as “synthetic employees.” Its final framing recognizes that AI agents do not reason like humans and can interpret predefined goals in unexpected ways.

Why agentic AI changes the control problem
A conventional model may generate a score, recommendation or draft for a person to review. An agentic system can plan a sequence of steps, call software tools, alter records or initiate actions with limited supervision. A mistake therefore can propagate through several systems before anyone notices it.
The FSB’s approach is to assess risk at the use-case level rather than treating “AI” as one uniform category. Materiality, the ability to explain decisions, data quality, model performance, connected systems and the consequences of failure all determine how much human control is needed.
The Cambridge Centre for Alternative Finance’s 2026 Global AI in Financial Services Report, based on 352 industry respondents and 130 regulators, identifies loss of human oversight and collective forgetting among the leading AI risks. Fifty-one percent of respondents selected that risk. The same report says 52% reported rapid deployment of agentic AI, while manual oversight is becoming less effective as systems plan and execute actions independently.

What human-approval triggers should do
The FSB does not prescribe one universal threshold. Instead, each institution should define triggers that match the potential harm and speed of an agent’s actions. A trigger should pause or constrain the workflow and route the decision to an appropriately authorized person.
- Limit consequential actions: Require approval before an agent can execute a materially large payment, change a customer’s financial terms, submit a market order or alter a critical risk setting.
- Escalate uncertainty: Send cases for review when confidence, data quality, explainability or performance falls outside documented tolerances.
- React to abnormal conditions: Increase supervision during cyber incidents, market stress, system outages or unusual transaction patterns.
- Control permission changes: Require a human decision before an agent gains a new tool, data source, account privilege or ability to delegate tasks.
- Preserve a stop mechanism: Maintain a tested way for authorized staff to suspend the agent, revoke access and contain downstream actions.
Higher-risk agents may need approval for every consequential step, while lower-risk systems may operate within preapproved limits with sampling and exception review. The key requirement is that the trigger is defined in advance, tied to risk and recorded so the institution can show who approved an action and why.
What banks and other firms are expected to implement
1. Make accountability explicit
Boards and senior management remain responsible for how AI fits the business strategy and risk appetite. Delegating operation to an autonomous system does not delegate accountability. The firm should name owners for each use case, define decision rights and ensure that risk, compliance, technology and business teams can challenge deployment decisions.

2. Assess each use case before and after launch
Documentation should describe the agent’s purpose, permitted actions, data, connected tools, expected performance, limitations and failure consequences. Materiality and explainability should be reassessed when the model, tools, data, operating environment or task changes.

3. Add agentic risks to existing controls
AI oversight should connect to established enterprise-risk, cyber, ICT-resilience and third-party-risk frameworks. Firms need monitoring for drift, security events, access misuse, degraded performance and unexpected tool calls, plus tested continuity and recovery procedures.
4. Manage external dependencies
Cloud providers, foundation-model suppliers, data vendors and software tools can become points of concentration or failure. Contracts and technical controls should address access, incident notification, audit evidence, data handling, service disruption and the ability to switch off or replace a dependency.

| Control question | Output-focused AI | Agentic AI |
|---|---|---|
| Who can approve or stop an action? | A user commonly reviews the model’s output before acting. | Named staff must retain authority to approve consequential actions and stop the workflow. |
| When must a person intervene? | Usually when an output is uncertain, sensitive or outside policy. | Whenever predefined, risk-based triggers are reached, including during multi-step execution. |
| What is monitored? | Accuracy, bias, data quality and output performance. | Those measures plus plans, tool calls, permissions, state changes, side effects and cumulative behavior. |
| What must be documented? | Model version, data, validation and decisions. | All of those, along with goals, delegated tasks, approvals, interventions and the action trail. |
| How quickly can an error spread? | Often limited to an individual recommendation or transaction. | It can cascade across connected accounts, systems or institutions before manual review. |
Why regulators see a wider financial-stability issue
Connected agents could amplify a common error, cyber compromise or flawed objective across firms that use similar models, data or infrastructure. That is why the FSB links individual-use-case controls with system-wide concerns such as concentration, third-party dependence and collective loss of human expertise.
The UK Financial Conduct Authority’s Mills Review says AI could reshape retail financial services by 2030 and beyond. Its seven recommendations include securing the regulatory perimeter, improving system-wide coordination and oversight, monitoring autonomous models, creating foundations for agentic finance and developing an AI-enabled supervisory model. FCA executive director Sheldon Mills said, “Artificial intelligence will transform financial services by 2030. It creates significant opportunities for consumers, firms and the wider economy.”
Best Value
Demand is already measurable: 20% of UK retail-finance consumers—equivalent to 11 million adults—said in the FCA’s 2026 research that they would be likely to use AI capable of acting autonomously within preset goals. Adoption interest makes weak controls a consumer-protection issue as well as an internal technology risk.
What the guidance does—and does not—require
The FSB practices provide a common baseline, but they do not create one global approval workflow or replace national law. A bank must still meet the requirements of the regulators where it operates, including rules governing advice, credit, payments, market conduct, privacy, outsourcing and operational resilience. The FSB says it will monitor developments and update the practices as needed.
For institutions, the practical test is straightforward: identify every action an agent can take, determine the harm if it is wrong or compromised, set a human decision point before that harm can occur, and maintain evidence that the control works. As agents gain broader permissions and operate faster, the depth and speed of human oversight must rise with the risk.
