Glass Ionomer vs Composite Resin: Which Dental Cement Is Right for the Restoration?

There is no universal winner. Composite resin is commonly chosen for tooth-colored direct fillings when the dentist can keep the tooth clean and dry. Glass ionomer can be the better fit for selected small restorations, higher caries risk, limited cooperation, or situations in which moisture control is difficult. The tooth, cavity class, chewing load, appearance goals, caries risk, and the product’s approved indication determine the choice.

“Glass ionomer” is a broad term. Conventional glass ionomer and resin-modified glass ionomer do not have identical handling or strength, so ask which formulation is proposed rather than treating every glass-ionomer restoration as the same material.

What the two materials are

Glass ionomer

Glass-ionomer restorative materials are made from an acid-reactive glass and an ion-containing polymer. They can chemically adhere to enamel and dentin, rather than relying only on a separate adhesive layer. They can also release and later recharge fluoride. That fluoride behavior is clinically relevant for some patients, but the amount of protection against future caries remains debated; fluoride release does not make a glass ionomer suitable for every chewing surface.

Conventional glass ionomer is used primarily as a cement, cavity liner, or filling in sites that do not carry heavy stress. Its relatively low stress-bearing capacity and greater susceptibility to abrasive wear than resin composite limit its use in high-stress locations. Resin-modified glass ionomer adds a resin component and uses a different setting process. Its properties and approved indications depend on the specific product, so the dentist must match the formulation to the restoration rather than assuming that every “modified” material performs identically.

Composite resin

Composite resin is a tooth-colored restorative material used directly in the prepared tooth. It is bonded with an adhesive technique and can be shaped and polished to reproduce tooth anatomy and appearance. Direct composite has broad indications, including many Class I and II restorations in permanent teeth and direct treatment of Class III, Class IV, and selected Class V defects.

Composite is technique-sensitive. Saliva, blood, crevicular fluid, or persistent moisture can interfere with the adhesive procedure. A dentist may therefore recommend another material when a reliable dry working field cannot be maintained, even if composite would otherwise provide the preferred appearance or load-bearing performance.

Glass ionomer versus composite at a glance

Decision axis Glass ionomer Composite resin
Typical strengths Chemical adhesion to tooth structure, fluoride release and recharge, and lower moisture sensitivity in certain uses. Natural tooth color, adhesive bonding, and broad direct-restoration indications.
Main constraints Conventional types have lower stress-bearing properties and are more susceptible to abrasive wear than composite; they are not appropriate for every high-stress site. Success depends on adhesive technique and control of contamination and moisture.
Situations in which it may be considered Selected small cavities, increased caries risk, limited compliance, or sites where dry isolation is challenging, provided the product is approved for the intended use. Many direct Class I and II restorations in permanent teeth, Class III and IV defects, and Class V defects when contamination control and adhesive technique are adequate.
What the evidence can establish Performance depends on formulation, tooth, restoration class, and indication; there is no universal longevity ranking. The same limitation applies. Comparisons vary by tooth, cavity class, follow-up period, operator, and material generation.

Why the restoration class changes the answer

Dentists classify cavities by location. The class describes the geometry and typical forces, so it is more useful than asking which material is “stronger” in the abstract.

Class I: pits and fissures on chewing surfaces

Class I cavities are on the occlusal surfaces of posterior teeth. They can experience substantial compressive and abrasive forces, especially when the cavity is broad or the tooth carries a heavy bite. Composite is a common direct option when isolation and bonding are reliable. A high-viscosity glass ionomer may be considered for a smaller lesion or for a patient with elevated caries risk or limited cooperation, but the dentist must confirm that the particular product is approved for permanent posterior use and suitable for the expected load.

Class II: cavities between posterior teeth

Class II restorations replace a proximal wall and often include an occlusal portion. They require a sound contact, resistance to chewing forces, and access that allows the dentist to control moisture. Guidelines support direct composite for Class II restorations in permanent teeth. Glass ionomer can be an alternative in specific circumstances such as a smaller cavity, higher caries risk, or limited compliance, but not simply because it releases fluoride.

Class III: front-tooth surfaces away from the biting edge

Class III defects affect the proximal surfaces of incisors and canines without involving the incisal edge. Appearance and a smooth, cleanable contour are usually important. Guidance supports direct composite for permanent Class III restorations and advises against glass ionomer for this permanent-tooth indication. The final choice still depends on the exact defect and the product’s labeling.

Class IV: front-tooth defects that include the incisal edge

Class IV restorations involve an incisal corner or edge, so appearance and resistance to edge forces both matter. Direct composite is the supported approach in the cited guideline for permanent teeth. Conventional glass ionomer is not a routine substitute for this high-visibility, edge-bearing indication.

Class V: near the gumline

Class V lesions occur on the cervical third of a tooth. The area may be hard to isolate because the gum can bleed or crevicular fluid can remain close to the margin. Guidance allows glass ionomer or a modified glass ionomer as alternatives for Class V defects. Composite is also possible when the dentist can achieve adequate contamination control and use the adhesive technique correctly.

The factors your dentist weighs

Chewing load and cavity size

A small, shallow lesion in a low-stress area presents a different mechanical problem from a large posterior cavity that replaces a cusp or carries a heavy occlusal contact. Conventional glass ionomer’s lower stress-bearing properties and wear susceptibility matter most when the restoration is repeatedly loaded or rubbed. Composite can tolerate many direct load-bearing indications, but it is not automatically the right answer if the field cannot be kept dry.

Caries risk

Frequent new cavities, exposed root surfaces, reduced saliva, difficulty cleaning, or a history of recurrent decay can influence the choice. Glass ionomer’s fluoride release and recharge may be useful as part of a broader preventive plan. They do not guarantee that decay will not recur, and the evidence does not justify describing glass ionomer as cavity-proof. Diet, fluoride exposure, brushing, saliva, plaque control, and follow-up remain important regardless of the restorative material.

Moisture and contamination control

Composite bonding requires an appropriately isolated field. A dentist may use a rubber dam, cotton isolation, suction, retraction, or other methods, but the decisive question is whether the specific site can stay clean and dry for the adhesive steps. Glass ionomer can be less moisture-sensitive in certain uses, which is one reason it may be selected when isolation is difficult. That does not mean it should be placed in a wet field without technique control; the product’s instructions still govern placement.

Cooperation and appointment conditions

Children, patients with limited ability to remain open, or anyone who cannot tolerate a lengthy isolation procedure may need a material and technique that can be delivered predictably under those conditions. Guidelines specifically recognize limited compliance as one circumstance in which glass ionomer may be considered for selected posterior permanent restorations. The dentist must still select a product approved for that tooth and restoration.

Appearance

Composite is tooth colored and can be layered, shaped, and polished for a close visual match. Glass ionomer can be acceptable where appearance is less critical, but shade, translucency, surface finish, and long-term wear vary by formulation. For a visible front-tooth defect, the guideline support for direct composite reflects both esthetic and functional demands.

Product indication and formulation

“Glass ionomer” does not identify one universal product. Conventional, high-viscosity, and resin-modified formulations have different instructions, strength profiles, setting behavior, and approved uses. A material that is appropriate as a liner or cement is not automatically approved as a permanent posterior filling. Ask whether the proposed product is indicated for the tooth, cavity class, and restoration type being treated.

What clinical evidence says about longevity

The broad ADA-supported review

An American Dental Association Council on Scientific Affairs systematic review published in 2023 assessed 38 randomized controlled trials involving primary and permanent teeth and several restoration classes. Failure events were infrequent, and the certainty of evidence varied. Its conclusion was that there was not enough evidence to establish important overall differences among direct restorative materials. The ADA materials overview states: "Overall, there is limited evidence to indicate a significant difference among restorative materials in longevity."

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

This conclusion is not a promise that every glass-ionomer and composite restoration lasts the same amount of time. It means that available trials, taken together, did not provide a reliable universal ranking that predicts an individual patient’s result.

Posterior permanent teeth

A separate 2023 review focused on posterior permanent teeth and included 10 studies. Its analyses contained 849 high-viscosity glass-ionomer restorations and 800 composite restorations. Clinical performance was comparable through 36 months in the reported comparisons. The only poorer result reported for high-viscosity glass ionomer was greater wear in Class I restorations at 24 months. The longest follow-up reported for Class I restorations was 36 months, so these findings do not establish equal lifetime performance or a permanent advantage for either material.

Non-carious cervical lesions

A review of restorations for non-carious cervical lesions found similar performance across its evaluated outcomes. The authors emphasized that follow-up periods, operators, lesion characteristics, brands, and outcome definitions varied. Because these are cervical lesions caused by processes other than ordinary caries, the results should not be generalized to every posterior or anterior cavity.

Why a study result may not predict your filling

  • A trial may involve a different tooth, cavity class, or restoration size.
  • The material may be an older generation or a different formulation from the one your dentist uses.
  • Follow-up may end after two or three years even though the restoration is expected to function longer.
  • Operators, isolation methods, finishing, bite forces, diet, and home care can differ substantially.
  • A restoration can need repair or replacement because of a new cavity, a fracture, a leaking margin, wear, or a change in the tooth, not because one material universally fails.

When glass ionomer may be the more practical choice

A small, selected posterior lesion

For a smaller posterior cavity in a permanent tooth, a dentist may consider a high-viscosity glass ionomer when the product is approved for that use and the patient’s caries risk or cooperation makes it attractive. The decision should include the expected chewing load and the possibility of wear, not fluoride release alone.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

High caries risk with a preventive plan

In a patient who repeatedly develops decay, fluoride-releasing and rechargeable behavior may be one factor in a material choice. It is an adjunct to prevention, not a substitute for fluoride toothpaste, plaque control, dietary changes, saliva management, and regular examinations.

A site where isolation is difficult

Near the gumline, around a partially erupted tooth, or in a patient who cannot maintain an open mouth, achieving a dry field may be challenging. A glass-ionomer or resin-modified glass-ionomer option may be considered if its handling and indication fit the site.

When composite resin may be the more practical choice

Visible front-tooth restorations

For permanent Class III and Class IV defects, direct composite is the guideline-supported approach. Its color and ability to recreate contours make it useful when the restoration will be visible, provided the dentist can control contamination.

Many routine posterior direct restorations

Composite is supported for direct Class I and II restorations in permanent teeth. It can be selected when the cavity size, occlusal forces, isolation, and bonding conditions are favorable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A restoration that needs a carefully shaped contact or anatomy

Composite is placed incrementally and sculpted before finishing, which can help the dentist reproduce proximal contacts and occlusal anatomy in suitable cases. The benefit depends on the operator’s technique and on keeping the adhesive field clean and dry.

Questions to ask before treatment

  1. Which tooth and restoration class are being treated? Ask whether the defect is Class I, II, III, IV, V, or another situation, and what forces that location normally receives.
  2. How much chewing load will this restoration carry? Find out whether the cavity is small and low stress or whether it includes a cusp, broad occlusal area, or another high-load feature.
  3. Which formulation is proposed? Clarify whether it is conventional glass ionomer, high-viscosity glass ionomer, resin-modified glass ionomer, or composite resin.
  4. Is the product approved for this exact use? For a permanent posterior glass-ionomer restoration, ask whether the manufacturer’s indication covers that tooth and restoration.
  5. Can the area be isolated reliably? Discuss saliva, gum bleeding, tongue movement, eruption status, and whether the planned adhesive steps can be performed without contamination.
  6. How does my caries risk affect the decision? Ask what preventive measures will accompany the restoration and what signs would prompt an earlier review.
  7. What are the appearance and wear trade-offs? A front-tooth restoration may prioritize shade and polish, while a cervical or high-caries-risk site may prioritize other handling characteristics.
  8. What maintenance, repair, or replacement could be needed? Ask how the dentist will monitor the margin, wear, contact, bite, and the tooth itself.

Common misunderstandings

“Fluoride release means glass ionomer prevents cavities.”

It does not. Fluoride release and recharge are real material properties, but the degree of caries protection remains debated. A restoration cannot compensate for ongoing plaque, sugar exposure, low fluoride exposure, or dry-mouth problems.

“Composite always lasts longer.”

The evidence does not support an absolute claim. The ADA-supported review found too few failures to establish important overall differences, and focused reviews produced bounded results that depend on location, follow-up, and formulation.

“Glass ionomer is only for children.”

Age is not the deciding criterion. Caries risk, cooperation, moisture control, tooth type, cavity class, load, and product indication apply to adults as well as children.

“A dental cement can be bought and placed at home.”

Permanent restorations require diagnosis, caries removal, isolation, material selection, shaping, bite adjustment, and follow-up. Consumer repair kits cannot determine whether the tooth needs a filling, a crown, treatment of the pulp, or urgent care. Do not attempt to perform a permanent restoration yourself.

How to interpret your dentist’s recommendation

A sound recommendation should connect the material to the site rather than rely on a slogan such as “strongest,” “most natural,” or “releases fluoride.” The explanation should identify the restoration class, expected forces, isolation conditions, caries risk, appearance requirements, formulation, and approved indication. It should also explain what could cause maintenance later, such as wear, a new cavity, a fractured margin, or changes in the tooth.

If the proposed choice surprises you, ask whether another material was considered and why it was rejected. A different answer may be reasonable for a small cervical lesion, a large posterior cavity, a visible incisor, or a patient who cannot tolerate prolonged isolation. The material decision is part of a treatment plan, not a product-shopping decision.

Bottom line

Composite resin is often the practical choice for tooth-colored direct restorations when bonding and moisture control are achievable, including many permanent Class I and II restorations and guideline-supported Class III and IV defects. Glass ionomer, including selected resin-modified formulations, can be useful for specific small or cervical restorations, higher caries risk, limited cooperation, or difficult isolation when the product is approved for that indication. Conventional glass ionomer’s lower stress-bearing capacity and greater wear susceptibility rule it out for some high-load sites. Your dentist should choose the formulation and technique that fit the tooth, cavity class, forces, moisture conditions, appearance goals, and preventive needs.
