The token is described as bringing you back after elimination with your carried inventory. These methods are season-specific; current-season availability is not confirmed.
What the 1-Up Token does
Epic’s v42.00 Fortnite Override update notes describe the 1-Up Token as: “Respawn with your full inventory after you’ve been eliminated.” A guide reports that it activates automatically when you are eliminated, sending you into the sky to redeploy with full health and the inventory you carried. The automatic trigger and full-health detail come from the guide; Epic’s notes confirm the inventory-retaining respawn effect.

Defeat Doctor Eggman
The guide reports that Doctor Eggman drops a 1-Up Token when defeated. It does not provide a drop probability.
Get the Extra Life Match Override
Activate an Override Console and receive the Extra Life outcome. Epic describes that outcome as “Respawn when on the brink of elimination one time.” It is one possible Match Override, not a guaranteed result, and should be distinguished from finding a 1-Up Token through the other routes.
