Flu Shot vs. Nasal Spray: Which Vaccine Is Right for You in the 2026–2027 Season?

For most people, the right flu vaccine is the one that is approved for their age and safe with their health history—and getting it is more important than choosing an injection or a spray. The nasal spray is an option for many healthy people ages 2 through 49, while injectable vaccines cover a wider age range and are the recommended route during pregnancy. People who are immunocompromised, have certain medical conditions, recently used some influenza antivirals, or have particular vaccine precautions may need an injection or individualized clinical advice.

This comparison reflects U.S. guidance for the 2026–2027 influenza season. Recommendations and availability can change, so confirm the current Centers for Disease Control and Prevention (CDC) guidance with a clinician or pharmacist before vaccination.

Quick answer: shot or nasal spray?

Both routes are seasonal influenza vaccines, and neither is universally more effective for every eligible person. The injectable options use inactivated influenza viruses or a recombinant formulation. The nasal option is a live attenuated influenza vaccine (LAIV) made with weakened influenza viruses.

Decision point Injectable flu vaccine (the flu shot) Nasal spray flu vaccine
Who can use it Age-appropriate products are available beginning at 6 months; product indications differ. Approved for people ages 2 through 49.
Pregnancy CDC recommends an injectable vaccine during pregnancy. Do not use during pregnancy.
Immune system Used when the nasal vaccine is unsuitable, subject to product-specific precautions. Not recommended for immunocompromised people.
Medical conditions and medicines May be appropriate when a condition or recent antiviral use rules out LAIV; a product-specific review is still required. Certain conditions, medicines and recent influenza antiviral use can make it unsuitable.
Administration Usually given as an injection by a health professional. Eligible recipients ages 18–49 may self-administer under the approved arrangement; an adult caregiver may administer it to recipients ages 2–17.
Relative effectiveness No route is preferred for everyone who is eligible for both. Effectiveness changes by season, age group, vaccine type and circulating influenza viruses.

If you are pregnant, immunocompromised, outside the 2–49 age range, or managing a significant medical condition, start with the injectable option or ask a clinician which product is appropriate. For a healthy, nonpregnant person in the approved age range, convenience and comfort can help decide once eligibility is confirmed.

How the two vaccines work

Injectable influenza vaccines

“Flu shot” is a plain-language term for several injectable products. Inactivated influenza vaccines use viruses that cannot replicate. A recombinant influenza vaccine is another injectable option made without using whole influenza viruses. Age indications, formulations and precautions vary by product, so a pharmacy or clinic may offer different choices to different age groups.

Injectable vaccines are the route used for infants and toddlers who meet the minimum age for a particular product, for pregnant people, and for many people whose conditions or medicines make LAIV unsuitable. CDC recommends annual influenza vaccination for everyone 6 months and older, with rare exceptions.

Nasal spray influenza vaccine

The nasal spray is LAIV: a vaccine made with weakened influenza viruses and approved for ages 2 through 49. It is a vaccine, not a treatment for influenza and not a substitute for medical care if you are already ill. Because it is live attenuated, its eligibility rules differ from those for injectable vaccines.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The spray can avoid a needle, but that advantage matters only when the person is medically eligible and an approved administration arrangement is available. A preference for avoiding an injection does not override pregnancy, immune-system, medical-condition or medication restrictions.

Age rules that determine your starting options

Children younger than 2

The nasal spray is not approved for children under 2. An age-appropriate injectable vaccine is required when vaccination is indicated. Infants younger than 6 months are below the routine minimum age for influenza vaccination, so protection for them depends on vaccination of eligible household members and caregivers and on maternal vaccination during pregnancy.

Children ages 2 through 17

Children in this range may be eligible for either route if they meet the nasal vaccine’s health requirements. An adult caregiver can administer the nasal vaccine under the approved arrangement. Some children ages 6 months through 8 years need two influenza-vaccine doses in the same season, with doses at least four weeks apart. Whether a child needs one or two doses depends on age and prior influenza-vaccination history, so bring the child’s records or ask the vaccinating professional to check them.

Adults ages 18 through 49

Healthy adults in this age range may have both routes available. The nasal vaccine’s approved self-administration option applies to eligible recipients ages 18–49; availability and the practical process can still vary by location. An injectable vaccine remains an appropriate choice when a person prefers it or has a reason LAIV is not suitable.

Adults 50 and older

The nasal spray is not approved for adults 50 and older. These adults should receive an age-appropriate injectable influenza vaccine, with product selection based on current recommendations and individual health factors.

When the nasal spray should not be your default

CDC’s interim clinical considerations for the 2026–2027 season include detailed contraindications and precautions. The situations below explain the major route distinctions, but they are not a replacement for that product-specific table or a clinician’s assessment.

Pregnancy

People who are pregnant should receive an injectable influenza vaccine, not LAIV. CDC also notes that vaccination during pregnancy helps protect a baby during the first months after birth, when the infant is too young to be vaccinated. A CDC summary of a 2018 study reported that influenza vaccination during pregnancy reduced the risk of hospitalization with influenza by an average of 40 percent. That figure is a pregnancy-specific study result; it does not compare the shot with the nasal spray.

Immunocompromising conditions

The nasal spray is not recommended for immunocompromised people. This includes situations in which a disease or treatment weakens immune function. The appropriate injectable product and timing can depend on the specific condition or therapy, so ask the clinician managing that condition and the vaccinating professional to coordinate the decision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Certain medical conditions

Some health conditions make LAIV inappropriate or require extra precautions. The exact list and the strength of the precaution depend on age, diagnosis and product. Do not rely on a short internet checklist if you have a complex medical history; have the vaccinator review the current CDC clinical considerations.

Recent influenza antiviral use

Recent use of some influenza antiviral medicines can affect whether LAIV should be given. Tell the vaccinator which antiviral you took and when you took it. Do not stop or delay a prescribed medicine simply to obtain the spray; the clinician can determine the safe interval or select an injectable vaccine.

Prior vaccine reactions, Guillain–Barré syndrome or acute illness

A prior severe allergic reaction to a vaccine, a history of Guillain–Barré syndrome following vaccination, or a moderate or severe acute illness may change the recommendation. These are product- and situation-specific precautions, not automatic instructions to skip every influenza vaccine. The vaccinating professional should review what happened, which product was involved and your current health before administration.

Who should choose the shot even when the spray is convenient?

  • Anyone who is pregnant.
  • Anyone younger than 2 or age 50 and older.
  • People who are immunocompromised.
  • People with a medical condition listed as a contraindication or precaution for LAIV.
  • People whose recent influenza antiviral use affects LAIV eligibility.
  • People whose prior vaccine reaction, Guillain–Barré history or current illness requires a product-specific assessment.
  • Anyone for whom an eligible nasal-spray administration arrangement is not available.

For these groups, “choose the shot” means use an injectable vaccine if the clinician confirms it is appropriate—not that every injectable product is interchangeable. Product indications and precautions still matter.

Effectiveness: what the evidence actually supports

CDC states that influenza-vaccine effectiveness varies by year, age, risk group, vaccine type, and the influenza virus type and subtype circulating that season. A vaccine can perform differently when the dominant viruses change or when protection is measured in a different population.

Current guidance does not establish a universal winner between the injectable and nasal routes for every person who is eligible for both. A result from one past season, one age group or one virus subtype cannot be turned into a permanent claim that the shot is always better or that the spray is always better.

The practical conclusion is to use an age- and health-appropriate vaccine every year. If both routes are medically suitable, choose the one you can obtain promptly and are willing to receive. Delaying vaccination while searching for a preferred route leaves you unprotected for longer.

Timing, dose count and protection

Best time for most people

CDC generally identifies September and October as good months for vaccination, with a goal of being vaccinated by the end of October. Protection takes about two weeks to develop. Getting vaccinated later can still be worthwhile while influenza viruses are circulating; do not assume that missing October means vaccination is no longer useful.

Two-dose schedules for some children

Some children ages 6 months through 8 years need two doses in one season. The doses must be at least four weeks apart. The determining factor is the child’s previous influenza-vaccination history, not simply whether the child received a vaccine last year, so the clinic should verify the record.

Annual vaccination

Influenza viruses and vaccine formulations change over time, and CDC’s core advice is annual vaccination for people 6 months and older, subject to rare exceptions. A dose received in a prior season does not replace the current season’s dose.

Where and how vaccination is administered

Clinics and pharmacies

Influenza vaccines are commonly provided through medical practices and pharmacies. Pharmacists can administer flu vaccines, but the legal authority and age limits for pharmacist administration vary by state. Call the location first to confirm that it vaccinates your age group and has the product you need.

Nasal-spray self- or caregiver administration

The nasal vaccine has an approved self-administration option for eligible recipients ages 18–49. For recipients ages 2–17, an adult caregiver may administer it under the approved arrangement. This authorization does not mean every pharmacy, clinic or household can offer the spray in the same way; follow the product instructions and the provider’s screening process.

Information to bring

  • Your age and, for a child, accurate date of birth.
  • Pregnancy status or plans, when relevant.
  • A list of medical conditions and medicines, including recent influenza antivirals.
  • Details of any severe allergic reaction to a vaccine.
  • History of Guillain–Barré syndrome after vaccination, if applicable.
  • The child’s previous influenza-vaccination record when determining a two-dose schedule.

A step-by-step way to decide

  1. Confirm the season and your age. This article addresses U.S. 2026–2027 guidance. The nasal option is limited to ages 2 through 49; injectable options include products for younger children and older adults.
  2. Check pregnancy and immune status. If you are pregnant or immunocompromised, plan on discussing an injectable vaccine rather than LAIV.
  3. Review conditions and medicines. Tell the vaccinator about chronic illnesses, current treatments, recent influenza antivirals, prior severe vaccine reactions and any Guillain–Barré history.
  4. Determine the child’s dose count. If the recipient is 6 months through 8 years, ask whether previous vaccination means two doses are needed at least four weeks apart.
  5. Compare practical access. Check whether a nearby pharmacy or clinic can vaccinate your age group, which route it stocks, whether state rules permit pharmacist administration, and whether an approved self- or caregiver-administration pathway applies.
  6. Schedule promptly. Aim for September or October when possible, allowing roughly two weeks for protection to develop. If those months have passed, ask about vaccination rather than waiting for the next season.
  7. Follow the product-specific advice. If your history is complex, let the clinician select the formulation and route instead of relying on a general comparison.

Common questions that cause confusion

Does an egg allergy rule out either vaccine?

No. CDC says egg allergy alone does not require safety measures beyond those used for any vaccine. An age- and health-appropriate influenza vaccine can be used. Tell the vaccinator about the allergy so standard screening can be completed.

Can the nasal spray treat influenza after symptoms begin?

No. The nasal spray is a preventive vaccine for seasonal influenza, not a treatment for an illness that has already started. If you have influenza symptoms or have been exposed, ask a clinician what evaluation or treatment is appropriate.

Is avoiding a needle a medical reason to use the spray?

It is a preference, not an eligibility rule. The spray is suitable only when the person meets its age and health requirements and the administration arrangement is approved and available. If not, an injectable vaccine is the appropriate route to discuss.

Should I wait for a specific brand or formulation?

Current CDC guidance emphasizes receiving an age- and health-appropriate influenza vaccine rather than declaring one route universally superior. Waiting for a preferred product can postpone protection. Ask what is available and suitable at the time you are vaccinated.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Bottom line

Choose an injectable flu vaccine during pregnancy, for children under 2, adults 50 and older, immunocompromised people, and anyone whose medical history or recent antiviral use makes LAIV unsuitable. If you are a healthy person ages 2 through 49 and both routes pass the safety screening, either the shot or nasal spray can be a reasonable seasonal choice. Get the approved vaccine you can receive promptly, ideally in September or October, and verify the current CDC guidance when your appointment is scheduled.
