In How to Fish, Forest Island’s three leeches are ground pickups in the grass—not fish to catch. First speak to the woman by the lake on Island 2, collect all three when the pickup prompt appears, and return them to her for the bait that summons the Giant Piranha.
Where to find the Forest Island leeches
Go to Island 2, also called Forest Island, and look through the grassy area near the lake. The leeches are small pickups on the ground; you do not catch them with a fishing rod or search underwater. Walk through the grass and watch for the interaction prompt. No fixed coordinates are established, so sweep the area rather than relying on one exact spot.

How to collect all three leeches
- Talk to the woman by the lake. Activate the quest before searching. A walkthrough reports that doing so starts the quest tracker.
- Search the nearby grass on foot. Move slowly and use the interaction prompt when a leech pickup appears.
- Continue until the quest count reaches three. Then return to the woman by the lake.
- Hand in the leeches. She gives you bait used to summon the Giant Piranha. The item is called Modified Leech in walkthroughs; check the current in-game label, as item naming may vary.
How to summon and defeat the Giant Piranha
Use the quest bait at the lake to start the Giant Piranha encounter. Keep moving during the fight and be ready to deal with the smaller piranhas as well as the boss. Walkthroughs differ on the boss’s health, so there is no reliable number to plan around; follow the encounter cues in your current game version.

What to do after the boss fight
Pick up the Giant Piranha Skeleton after defeating it and take it back to the woman by the lake. The skeleton hand-in is the step that completes this quest stage and advances island progression; turning in the leeches alone does not finish it. Keep the skeleton until you have handed it over.

If the leeches do not appear
- Confirm that you spoke to the lakeside woman and activated the quest.
- Walk slowly through the grass and look for the interaction prompt instead of casting into the lake.
- If you have already defeated the boss, retain the skeleton for the quest hand-in.
