Some falsified medicines are almost visually identical to the real product. If a medicine looks unusual, seems ineffective, or causes an unexpected reaction, ask a pharmacist or clinician rather than trying to authenticate it yourself.
What “fake medicine” can mean
“Fake” is a useful everyday term, but health authorities distinguish between two problems. The World Health Organization (WHO) calls a medicine substandard when it fails to meet quality standards or specifications. A falsified medical product deliberately misrepresents its identity, composition, or source. A product that raises concern is not necessarily falsified; only appropriate assessment can help establish what is wrong.

Either kind of product can contain the wrong ingredient, too much or too little of an active ingredient, no active ingredient, or harmful substances. Treatment may fail, illness may worsen, or the product may cause direct harm. Ineffective antimicrobial medicines can also contribute to antimicrobial resistance. WHO’s 3 December 2024 fact sheet says countries spend an estimated US$30.5 billion per year on substandard and falsified medical products; the fact sheet does not specify the estimate’s base year. It also cites an estimate that at least 1 in 10 medicines in low- and middle-income countries are substandard or falsified, based on WHO’s 2017 analysis of medicines that failed quality-control tests. How to lower the chance of buying one
Use a licensed, trusted source
Get medicines from trusted outlets licensed in the place where you live. In the United States, the FDA advises buying only from state-licensed pharmacies. Elsewhere, check with your national medicines regulator or an appropriate pharmacy-verification service; licensing rules and verification systems vary by country. FDA: Counterfeit Medicine

Check who is selling and dispensing it
Before ordering, check whether the seller is licensed by the relevant jurisdiction, whether a pharmacist or clinician is available to answer questions, and whether the seller provides clear contact and location details. WHO flags unregistered websites that conceal a physical address or landline number as a concern. These checks can help you assess the source, but they do not prove that a particular medicine is genuine. WHO advice to patients and consumers
Can you spot a fake medicine by looking at it?
No visual check can reliably authenticate a medicine. WHO warns that some falsified products are “almost visually identical” to genuine ones. Damaged packaging, spelling mistakes, an unexpected expiry date, a change in appearance or smell, lack of effect, or an unusual reaction may be reasons to seek advice—but none proves falsification on its own. A genuine product can also look different after a packaging change, and other quality problems may have different causes.

Don’t rely on a barcode scan, phone app, or consumer gadget as proof that a medicine is authentic. If you have a concern, show the packaging and product to a pharmacist or clinician and explain where and when you obtained it. WHO’s guidance for health professionals describes identification of suspected substandard and falsified products; suspicion is a reason for appropriate follow-up, not a diagnosis by sight. What to do if you suspect a medicine is fake or unsafe
- Contact a pharmacist, doctor, or other healthcare professional. Explain what seems wrong and provide the product and packaging information. If it is not working properly or you have a reaction, seek professional advice promptly rather than waiting to confirm whether the product is falsified.
- Ask the relevant medicines regulator how to report it. Reporting routes differ by country. Share the product name, batch or lot number if available, where it was obtained, and what happened. Keep the packaging and receipt if you have them.
- Get urgent medical care for serious symptoms. Do not delay emergency care while trying to contact a regulator or investigate the product.
