If FFmpeg is using the concat demuxer, the path inside the playlist needs concat-script quoting or escaping; quoting the playlist filename in your shell does not quote the media filenames written inside it. That is a strong first check, not a confirmed cause: the exact command, list format, error, and output protocol determine where the failure occurs.
First identify which playlist FFmpeg is reading
Concat-demuxer input is commonly indicated by a command such as -f concat -i list.txt, or by a text list that begins with ffconcat version 1.0. The list contains a file directive for each media file. Its path syntax is separate from your shell’s quoting rules.

For example, quoting list.txt in a shell protects that argument from the shell. It does not change a line inside the list from file /mnt/share/promo clip.mp4 into a valid concat-script path. FFmpeg’s format documentation says, “Path to a file to read; special characters and spaces must be escaped with backslash or single quotes,” and gives this example:

Check the concat list and its paths
- Open the exact list FFmpeg receives. Find the directive for the first file that fails. If its name contains spaces, quote the path using concat-script syntax, for example
file '/path/to/promo clip.mp4'. - Check the safe-path setting. The concat demuxer defaults to
safe=1. Under the documented rules, safe paths must be relative and use only letters, digits, period, underscore, and hyphen; a space is outside that character set. If the list and referenced files are trusted and locally controlled,-safe 0relaxes this filename validation. It does not fix a misspelled or missing file. Do not use it to accept paths from untrusted sources. - Verify path resolution and access. Relative paths must resolve from the FFmpeg process’s actual working directory, which may differ from the directory you expect when a script, service, or scheduled job launches it. Confirm each file exists at the resolved path and that the process can read it.
- Check how the list was generated. A script that writes filenames into a concat list must preserve the concat format’s quoting and escaping rules for spaces and other special characters. A list that works for simple filenames may break when a name includes punctuation.

Confirm that the promo clips can be concatenated this way
A corrected path may let FFmpeg open the next file but reveal a different issue. The concat demuxer expects the inputs to have matching streams, codecs, and time base. If the clips differ, a filename fix alone will not make them a compatible sequence. FFmpeg’s FAQ recommends the concat filter when re-encoding is needed. FFmpeg FAQ

Follow the log in order. If FFmpeg cannot open or parse the list, investigate the input path, list syntax, permissions, and file compatibility. If FFmpeg opens and advances through the files but then fails while publishing, inspect the later output error and identify the protocol in use.

YouTube documents distinct errors for HLS ingestion: HTTP 400 can indicate a malformed URL or an unparseable or unsupported playlist; HTTP 401 can indicate a corrupted or expired cid parameter. What to gather if the fix is not clear
- The full FFmpeg command, with stream keys and other secrets redacted.
- The exact playlist line for the file where processing stops, plus enough neighboring lines to show the list format.
- The filename and directory structure, with sensitive information removed.
- Your operating system, shell, and
ffmpeg -versionoutput. - The complete relevant error excerpt, including lines before and after the failure.
- The output protocol, such as RTMP or YouTube HLS.
