Yes—fertility may be preservable before cancer treatment, but the right method depends on whether puberty has started, the child’s reproductive anatomy, the cancer and planned therapy, and how quickly treatment must begin. Ask the oncology team for an urgent referral to a pediatric oncofertility or reproductive-medicine service as soon as cancer is diagnosed. The fertility discussion can occur during staging and treatment planning; cancer control remains the priority, and a procedure should not create an unsafe delay.
Established options include sperm freezing for boys who have gone through puberty, egg or embryo freezing for girls who have gone through puberty, and ovarian-tissue freezing for prepubertal girls. Testicular-tissue freezing for prepubertal boys remains investigational and is generally available only through an approved clinical trial. Every decision should include the child’s assent when developmentally appropriate, parent or guardian consent, psychological support, and a plan for storage and future use.

What to do immediately after diagnosis
- Tell the oncology team that future fertility matters to your family. Ask for a fertility-preservation referral before chemotherapy, radiation, surgery, or conditioning for a transplant starts.
- Ask whether treatment can safely wait. The oncologist and fertility specialist should weigh the medical risk of even a short delay against the time needed for a collection, stimulation, anesthesia, or surgery.
- Confirm puberty status and anatomy. Available methods change substantially after puberty. A clinician should explain how puberty, prior treatments, and reproductive anatomy affect the choices.
- Request a written plan. It should identify the proposed method, required appointments and tests, who will perform the procedure, how samples or tissue will be stored, likely future-use pathways, and what happens if treatment must start sooner than expected.
- Include the child in decisions. Use age-appropriate language, invite questions, and document the patient’s assent or refusal alongside the legal consent of a parent or guardian.
American Society of Clinical Oncology (ASCO) guidance calls for reproductive-risk counseling before cancer-directed therapy and prompt access to a multidisciplinary fertility-preservation team. The American College of Obstetricians and Gynecologists (ACOG) likewise recommends discussing preservation before treatment and referring patients to clinicians experienced in oncofertility.
Why cancer treatment can threaten fertility
Fertility risk depends on the treatment’s effect on the ovaries, testes, uterus, hormone-producing structures, and the brain pathways that regulate reproduction. Chemotherapy drugs can damage eggs or sperm; radiation can injure reproductive organs or the brain’s hormone centers; pelvic surgery can remove or alter reproductive organs; and some combinations of treatments carry more risk than any single therapy.

The same diagnosis can lead to different fertility risks because doses, fields, schedules, surgery, age, puberty, and the use of stem-cell transplantation differ. Ask the oncology team which organs are exposed and whether the risk is expected to be temporary, permanent, or uncertain. A low or uncertain risk is not the same as no risk, but fertility procedures also have burdens and should be chosen in the context of the cancer plan.
Options by puberty, sex, and reproductive anatomy
| Method | Who may be eligible | Time and procedures | Status and important limits |
|---|---|---|---|
| Sperm cryopreservation | Pubertal or postpubertal boys who can produce semen | One or more semen collections before treatment; no ovarian-style stimulation | Established. ASCO recommends a flexible approach and collecting as many ejaculates as feasible. |
| Testicular sperm extraction (TESE) with cryopreservation | Pubertal or postpubertal boys unable to produce a semen sample | Minor surgical sperm retrieval, usually with anesthesia, before treatment | Established option when a semen sample cannot be obtained; surgical and anesthesia risks apply. |
| Testicular-tissue cryopreservation | Prepubertal boys in selected programs | Surgical removal and freezing of testicular tissue | Investigational; generally should be offered only through an approved clinical trial. Future fertility is not guaranteed. |
| Egg (oocyte) cryopreservation | Pubertal or postpubertal girls with functioning ovaries | Hormonal stimulation, monitoring, and egg-retrieval procedure | Established. Requires enough time and a medically safe stimulation plan. |
| Embryo cryopreservation | Pubertal or postpubertal girls who choose to create embryos | Egg stimulation and retrieval followed by fertilization with sperm and embryo freezing | Established. Requires sperm and raises additional decisions about future embryo use and consent. |
| Ovarian-tissue cryopreservation | Prepubertal girls and selected pubertal patients | Surgery to remove and freeze part of an ovary or ovarian tissue | Established for prepubertal girls according to ASCO; does not require sexual maturity or ovarian stimulation. Specialist review is essential. |
| Ovarian transposition | Patients whose pelvic radiation may damage the ovaries | Surgery to move ovaries away from the planned radiation field | Selected according to the radiation plan; it does not protect against every form of treatment-related ovarian injury. |
| Conservative gynecologic surgery | Selected patients when cancer type and stage permit organ-sparing treatment | Cancer surgery designed to retain reproductive organs when medically safe | Highly diagnosis-specific; cancer control determines whether it is appropriate. |
Pubertal or postpubertal boys
Sperm freezing is the first-line option
Once puberty has begun and ejaculation is possible, sperm cryopreservation should be offered before cancer-directed therapy. A collection is made by masturbation in a private clinical setting or, where permitted, through an approved home-collection process. The laboratory analyzes the sample, divides it into storage containers, and freezes it for potential future assisted reproduction.
ASCO advises clinicians to use a flexible approach and collect as many ejaculates as is feasible before treatment. More samples can provide more choices later, but the number depends on the child’s comfort, schedule, sperm production, and how urgently therapy must start. Do not postpone lifesaving treatment solely to pursue an ideal number of collections.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsWhen ejaculation or collection is not possible
A teenager may be unable to produce a semen sample because of age, anxiety, pain, neurologic effects, cultural or personal concerns, or illness. Testicular sperm extraction (TESE) with cryopreservation can retrieve sperm directly from testicular tissue before treatment. It is a procedure rather than a simple laboratory collection, so the team must explain anesthesia, bleeding, infection, discomfort, and the possibility that no usable sperm will be found.

Prepubertal boys
Before puberty, testes do not normally make mature sperm. Testicular-tissue cryopreservation is therefore different from sperm banking: tissue containing immature sperm-producing cells is removed and frozen with the hope that future technologies may mature or use those cells. ASCO describes this approach as investigational and generally recommends offering it only in a clinical trial. Ask whether a pediatric center has an approved protocol, what oversight applies, and what is known about future use. Families should not be told that storing tissue guarantees biological parenthood.
Pubertal or postpubertal girls
Egg freezing
Oocyte cryopreservation uses medicines to stimulate the ovaries, ultrasound and laboratory monitoring, and a retrieval procedure to collect mature eggs. The process requires enough time for stimulation and a medically safe plan that fits the cancer schedule. The fertility specialist should explain injections, side effects, anesthesia or sedation, the number and maturity of eggs retrieved, and the fact that not every frozen egg will later produce an embryo or birth.
Embryo freezing
Embryo cryopreservation follows the same stimulation and egg-retrieval process, but the eggs are fertilized with sperm before freezing. This can provide a defined reproductive plan for some families, while also creating decisions about whose sperm is used, who controls the embryos, storage duration, and what happens if the child’s wishes change. Embryo creation cannot proceed without sperm, so egg freezing may be the more flexible choice for a teenager who does not want or cannot arrange fertilization now.

Ovarian tissue, transposition, and conservative surgery
Ovarian-tissue cryopreservation may be considered when stimulation is not feasible or when treatment must begin quickly. A surgeon removes and freezes ovarian tissue for possible later reimplantation or other future use. The team must discuss operative and anesthesia risks, the possibility of cancer cells being present in the tissue, and whether re-transplantation would be safe for that cancer.

If pelvic radiation is planned, ovarian transposition can move the ovaries outside the main radiation field. Protection is incomplete and depends on radiation scatter, dose, surgical anatomy, and whether chemotherapy also harms ovarian reserve. Conservative gynecologic surgery may preserve reproductive organs in carefully selected cancers, but only when it does not compromise cancer treatment.
Prepubertal girls
For a girl who has not started puberty, ovarian-tissue cryopreservation is the established fertility-preservation method identified by ASCO. It does not require sexual maturity, ovulation, or ovarian stimulation and can be performed when treatment cannot be delayed for a stimulation cycle. Usually, a laparoscopic procedure removes part of an ovary or ovarian tissue for freezing while leaving functioning tissue in place.
This is still major medical care. Families should ask about anesthesia, bleeding, infection, postoperative pain, possible reduction in ovarian reserve, and the risk that cancer cells could be stored in the tissue. If tissue is later considered for transplantation, specialists must reassess recurrence risk and the safety of reintroducing cells. The science and long-term outcomes continue to evolve, so the specialist should describe what is established for the child’s diagnosis rather than promise a particular result.
Timing when treatment is urgent
Fertility counseling can begin while scans, biopsies, and staging are underway. Some procedures fit into a short pre-treatment window; others require more visits or surgery. The oncologist should state the latest safe treatment-start date, and the fertility team should design the fastest medically appropriate pathway.

Reduced sperm quality, a small number of available collections, or limited time should not automatically dissuade banking before treatment. Even a limited sample may provide future options. Conversely, a procedure should be stopped or deferred if it would create unacceptable oncologic risk. Document the agreed trade-off so every team member understands the schedule.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Consent, assent, and emotional support
Established preservation methods for children and adolescents who have initiated puberty should be offered with the patient’s assent and parent or guardian consent. Assent means the child receives understandable information, has a genuine opportunity to ask questions, and agrees when able; it is not merely a signature. A child may decline a procedure, and ACOG highlights the ethical importance of addressing minors’ assent, consent, and refusal.
Privacy and dignity matter during semen collection, genital examinations, injections, and surgery. Ask for a clinician of the child’s preferred gender when possible, private counseling without parents for part of the visit when appropriate, interpreters, and mental-health support. A psychologist, social worker, nurse navigator, or adolescent-medicine clinician can help with body image, grief, sexuality, religious concerns, and communication between the teenager and family.
Storage, future use, and costs
Before signing consent, ask who owns or controls stored sperm, eggs, embryos, or tissue; how long storage continues; how annual fees are billed; what happens if payments stop; and how samples are transferred if the family moves. The future-use plan may involve assisted reproduction, surgery, transplantation, or technologies that are not yet available for investigational tissue.
Best Value
Insurance coverage varies by country, state or province, policy, diagnosis, and whether preservation is considered medically necessary. Request a benefits review before the procedure, but do not assume that coverage includes medications, anesthesia, laboratory processing, long-term storage, future fertility treatment, or travel. A financial counselor or patient navigator can identify prior-authorization requirements, grants, hospital assistance, and payment deadlines. Get every estimate in writing because fees and eligibility rules change.
What success rates can—and cannot—tell you
There is no single success percentage that applies to every child or teenager. Outcomes vary with age, puberty, ovarian or testicular reserve, cancer type, treatment exposure, tissue quality, the number of stored samples, later transplantation, and the assisted-reproduction method used. A frozen sample is an opportunity, not a guarantee of pregnancy or biological parenthood.
Reported ovarian-tissue transplantation cases have included pregnancies and live births, according to ACOG, but recurrence risk and rapidly evolving science remain important limitations. Ask the specialist for outcomes that match the child’s age, diagnosis, treatment, and proposed method, and ask which figures describe laboratory survival, fertilization, pregnancy, or live birth—these are different endpoints.

Warning signs that the plan needs clarification
- No fertility referral has been offered before treatment begins.
- The proposed method is described without explaining puberty status, treatment urgency, or alternatives.
- A clinician promises a specific pregnancy or live-birth rate without discussing the child’s individual factors.
- Investigational prepubertal testicular tissue is presented as proven or guaranteed.
- Storage duration, ownership, annual fees, or future-use consent is left undocumented.
- The child’s questions, assent, privacy, or refusal are ignored.
Fertility preservation is one part of a child’s cancer plan, not a substitute for effective cancer treatment. A prompt, coordinated discussion gives families the best chance to consider an appropriate option without losing sight of the therapy that must control the cancer.
