Short answer: In male survivors of childhood cancer, a persistently high follicle-stimulating hormone (FSH) level during puberty—particularly after testicular radiotherapy—can indicate a higher risk of impaired sperm production in adulthood. Normal testosterone or apparently normal pubertal development does not prove that sperm production is intact.
Hormone results must be interpreted with the treatment history, radiation dose, age, physical findings and repeated measurements. A single FSH result is not a diagnosis of infertility, and the strongest published thresholds so far come from one Finnish cohort rather than from universal clinical standards.

What the newest study found
Koskela and colleagues reported the direct hormone-to-fertility evidence in JAMA Network Open on October 9, 2026. The retrospective, registry-based cohort included 221 male survivors treated before age 17 at Helsinki Children’s Hospital. Their cancers were treated from 1964 to 2000; the median age at diagnosis was 6.0 years (interquartile range, 3.2–11.7).

Serum FSH, luteinizing hormone (LH) and testosterone were measured at two-year intervals from ages 12 through 18 and again at age 20 or older. The investigators converted results to age-specific z scores and related pubertal hormone patterns to adult semen quality, paternity, testicular volume and testosterone replacement.

The clearest signal was FSH. Among survivors who had received testicular radiotherapy, the highest FSH recorded at ages 14–16 was associated with adult azoospermia, meaning no sperm detected in an ejaculate sample. The study’s best-performing research thresholds corresponded to 11.2 IU/L at age 14 and 12.5 IU/L at age 16, with an area under the receiver-operating-characteristic curve of 0.87.
Those values are exploratory, cohort-derived thresholds—not universal diagnostic cutoffs. They should not be used alone to label a child infertile or to predict an individual adult’s reproductive outcome.

How each pubertal measure should be interpreted
| Measure | What it reflects | What it can and cannot show |
|---|---|---|
| FSH | Pituitary stimulation of Sertoli cells and the sperm-producing compartment of the testes | The most useful pubertal signal in the 2026 cohort for later sperm-production risk. Persistent elevation can suggest Sertoli-cell or germ-cell injury, but one result does not establish sterility. |
| LH | Pituitary stimulation of Leydig cells | May rise because of primary testicular injury. It is not, by itself, proof that central puberty has begun. |
| Testosterone | Leydig-cell steroid production and pubertal androgen activity | Helps assess pubertal progression. Testosterone can remain normal even when spermatogenesis is severely impaired. |
| Testicular volume | Physical growth of the testes | Useful during examination, but not a reliable sole marker of puberty or sperm production after alkylating agents or direct testicular radiotherapy. |
“Elevated LH levels in such patients may be due to primary gonadal injury rather than to the onset of central puberty.” — Endocrine Society clinical practice guideline

Why treatment exposure changes the meaning of a result
Radiation field and dose, cumulative alkylating-agent exposure, hematopoietic stem-cell transplantation, hypothalamic-pituitary radiation, age at treatment and combinations of therapies all affect gonadal risk. The same hormone value can therefore mean different things in two survivors with different treatment histories.

| Exposure pattern in the Helsinki cohort | Observed pubertal pattern | How to use the finding |
|---|---|---|
| Testicular radiotherapy above 1 Gy | FSH elevation appeared from about age 12 and increased with dose. | Persistent or marked FSH elevation warrants focused survivorship and fertility assessment; the study thresholds are not universal cutoffs. |
| Intermediate cumulative alkylating exposure: 4 to less than 12 g/m² cyclophosphamide-equivalent dose | FSH often moved toward the reference range by adulthood, suggesting that recovery may be possible for some survivors. | Do not assume that an abnormal pubertal result is permanently fixed; follow the trend over time. |
| High cumulative alkylating exposure: at least 12 g/m² | FSH elevation was more persistent. | Long-term monitoring is particularly important, even when testosterone is preserved. |
| High cumulative alkylating exposure overall | Was not associated with impaired Leydig-cell function in this cohort; testosterone varied less than FSH. | Normal androgen production does not rule out damage to the Sertoli-cell or germ-cell compartment. |
The pattern supports a biological distinction: sperm-producing cells and their Sertoli-cell support appear more treatment-sensitive than Leydig cells. The National Cancer Institute cautions that an infertility risk category based on treatment exposure does not translate directly into an adult fertility diagnosis. Combined exposures may have additive effects, so a guideline risk group is not a personal prognosis.
Does normal puberty mean fertility is normal?
No. Testosterone-driven changes such as growth of the penis, voice deepening and muscle development primarily show Leydig-cell function. Sperm production depends on a different testicular compartment and may be impaired while testosterone remains adequate.

Likewise, a single high FSH value is not proof that sperm production has permanently failed. Pubertal hormone concentrations change with age, illness, laboratory method and treatment recovery. Clinicians need the trajectory—repeated values interpreted against age-appropriate ranges—alongside treatment records and examination findings.

How survivors can approach fertility follow-up
- Collect the treatment history. Ask the survivorship team for radiation fields and doses, cumulative alkylating-agent exposure, transplant history, hypothalamic-pituitary treatment and age at each therapy.
- Review puberty over time. Record growth, physical pubertal progression, testicular examination and symptoms rather than relying on one laboratory result or testicular volume alone.
- Use appropriately timed laboratory tests. For boys at risk of central precocious puberty after gonadotoxic treatment, the Endocrine Society recommends morning testosterone and LH measurement, preferably before 10 a.m. FSH should be interpreted with the same clinical context and repeated when appropriate.
- Screen for pituitary injury when indicated. The Endocrine Society recommends screening for LH/FSH deficiency after hypothalamic-pituitary radiation of at least 30 Gy or after a tumor or surgery affecting that region.
- Discuss definitive fertility assessment at the right stage. When clinically appropriate, semen analysis, reproductive-endocrinology evaluation and paternity history provide adult outcome information that hormones alone cannot supply.
- Refer early when fertility matters. A childhood-cancer survivorship endocrinology clinic and a fertility-preservation service can explain whether cryopreservation of sperm, germ cells or germ-cell tissue is feasible for the survivor’s age and pubertal stage.
What guidance says about preservation and counseling
The 2023 German S1 guideline describes fertility impairment as a possible consequence of childhood-cancer treatment and recommends individualized information and self-determined decisions about cryopreservation of germ cells or germ-cell tissue. Counseling should be based on actual exposures and current findings, not on the appearance of puberty alone.

Children’s Oncology Group long-term follow-up guidance lists delayed or arrested puberty, abnormal hormone levels, menstrual abnormalities and treatment-related gonadal dysfunction as surveillance concerns. Follow-up should be coordinated with the survivor’s oncology, endocrinology and reproductive-care teams.
What this means for female survivors
The Helsinki study included only male survivors, so its FSH thresholds cannot be transferred to girls or women. Female assessment may include pubertal development, menstrual history, FSH, LH and estradiol, with anti-Müllerian hormone used in some clinical situations. Ovarian risk also depends on ovarian radiation, alkylating exposure, transplant and hypothalamic-pituitary treatment.
No equivalent universal pubertal hormone thresholds for female survivors, every cancer diagnosis or every treatment regimen are established by this study.
Limits of the evidence
- The cohort came from one Finnish center and included treatments delivered between 1964 and 2000.
- It was retrospective and registry-based, with limited subgroup sizes.
- Hormone assays changed over the long study period.
- The receiver-operating-characteristic thresholds were exploratory rather than validated clinical cutoffs.
- Patients with brain tumors treated at another hospital were excluded.
- The results concern male survivors and cannot establish a universal rule for female survivors or for every modern treatment protocol.
The practical conclusion is narrow but useful: after childhood cancer, a rising or persistently high pubertal FSH—especially following testicular radiotherapy—deserves careful longitudinal review because it may identify later sperm-production risk before adult fertility is tested. Normal testosterone or completion of puberty should not be treated as proof of normal fertility.
