Female Hair Loss Treatments Compared: Minoxidil, Prescription Medicines, and Procedures

For female pattern hair loss, topical minoxidil is the best-supported first-line option among the treatments compared here. Prescription medicines and procedures may suit selected patients, but the right choice depends first on confirming the cause of the hair loss. Gradual thinning can have causes other than female pattern hair loss, so an atypical pattern or an unclear cause warrants clinical evaluation.

Start by finding out what is causing the hair loss

Female pattern hair loss is only one explanation for thinning or shedding. The American Academy of Dermatology (AAD) says treatment depends on the cause, so it is important not to assume that a product intended for pattern hair loss will address every kind of hair loss. A clinician can assess the pattern and help determine whether further evaluation is needed.

Bring up the concern with a dermatologist or other qualified clinician if the loss does not fit a gradual pattern or if you are unsure what is causing it. Diagnosis matters before comparing treatments: the options below are for female pattern hair loss, not a substitute for identifying another cause.

How the main options compare

Option How it is used Role and evidence described in the sources Key consideration
Topical minoxidil Applied to the scalp; AAD discusses 2% solution and 5% foam in the United States. First-line treatment in a 2019 multidisciplinary clinical report; FDA-approved for women’s hair loss in the United States, according to AAD. Ongoing use is needed to maintain benefit; avoid during pregnancy, when planning pregnancy, or while breastfeeding.
Prescription antiandrogen or 5-alpha-reductase inhibitor medicines Prescription treatment selected and monitored by a clinician; examples include spironolactone and finasteride. Possible off-label options in selected cases, rather than routine substitutes for minoxidil. Evidence is uneven, and reproductive risks require discussion; secure contraception is recommended for women of reproductive age taking these medicines.
Low-level laser therapy Device-based treatment involving repeated sessions. Possible option; the 2019 clinical report describes modest benefit and limited evidence. Repeated use over many months may be needed.
Platelet-rich plasma (PRP) Blood is processed and injected into the scalp, typically across repeat visits. Described as promising, but limited by the available evidence in the 2019 clinical report. Requires office visits and injections; discuss suitability and expectations with a clinician.
Hair transplantation Surgical transfer of hair to areas of thinning. An option for selected patients; the 2019 clinical report advises pairing it with continued medical therapy. Diffuse sparse hair may not leave enough healthy donor hair for transplantation.

Topical minoxidil: the first option to understand

The multidisciplinary Androgen Excess and PCOS Committee report published in the Journal of Clinical Endocrinology & Metabolism in 2019 states: “Topical minoxidil is an effective treatment of FPHL and should be used as the first-line treatment (level of evidence B).” AAD likewise identifies topical minoxidil as an FDA-approved treatment for women’s hair loss in the United States and discusses 2% solution and 5% foam.

  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.

A women’s 5% minoxidil foam is one product category, not a claim that a particular brand or formulation is superior. Follow the current product label and clinician guidance for use; this article does not prescribe a regimen.

Minoxidil’s benefit depends on continuing treatment. AAD says stopping can lead to loss of hair maintained or regrown during use. AAD also advises women who are pregnant, planning pregnancy, or breastfeeding to avoid minoxidil.

Prescription medicines: selective, off-label choices

The 2019 multidisciplinary report discusses spironolactone and 5-alpha-reductase inhibitors such as finasteride as possible additions in selected cases, particularly when hair loss is severe or hyperandrogenism is present. These are off-label uses for hair loss in the United States; they are not interchangeable over-the-counter alternatives to topical minoxidil.

The report notes that much of the evidence for these medicines comes from postmenopausal women. It recommends secure contraception for women of reproductive age taking antiandrogens or 5-alpha-reductase inhibitors because of teratogenic risk. A clinician should weigh the potential benefits, medication-specific adverse effects, reproductive plans, and any appropriate monitoring before prescribing.

A separate caution about compounded topical finasteride

Compounded topical finasteride deserves a specific safety discussion; it should not be assumed to avoid the concerns associated with finasteride simply because it is applied to the skin. In a 2025 alert, the U.S. Food and Drug Administration (FDA) said absorption through the skin is expected and warned about potential risks, including transfer to women who handle the product. The FDA reported 32 associated adverse-event reports in its reporting system from 2019 through 2024. That is a surveillance report count, not an incidence rate or proof that the product caused each event.

Procedures and devices: weigh evidence against time and suitability

Low-level laser therapy

AAD describes low-level laser therapy as a possible option, while the 2019 clinical report characterizes the benefit as modest and the evidence as limited. Treatment may require repeated sessions over many months, so ask the clinician or device provider what schedule is proposed and what outcome is realistic for your case.

Platelet-rich plasma

PRP involves processing a person’s blood and injecting the resulting preparation into the scalp. AAD describes repeat visits, and the 2019 report calls PRP promising while noting that evidence is limited. It is an office-based procedure, not a proven one-time fix; discuss the expected course, uncertainties, and suitability with a qualified clinician.

Hair transplantation

Transplantation may be considered for selected patients, but it depends on having enough healthy donor hair. People with diffuse sparse hair may not have enough donor hair for the procedure. The 2019 report advises continuing medical therapy alongside transplantation, rather than treating surgery as a replacement for ongoing management.

Questions to take to a clinician

  • Does my pattern of hair loss fit female pattern hair loss, or should another cause be evaluated?
  • Which option is appropriate for my diagnosis, health history, and reproductive plans?
  • What benefits are realistic, how will we assess them, and what happens if treatment is stopped?
  • For a procedure or device, how many visits or sessions are involved, and what are the limitations for someone with my pattern of thinning?
  • If a compounded product is proposed, what is known about absorption, handling, transfer risk, and alternatives?
