Family Caregiving FAQs: 2024 Costs, Legal Authority, Paying Relatives, and Sharing the Work

Short answer: Family caregiving can range from a few hours of unpaid help to round-the-clock supervision and skilled tasks. In the United States, 2024 national median costs ran from $26,000 a year for adult day care to $127,750 for a nursing-home private room. Medicare generally does not pay for long-term custodial care. A health-care proxy controls treatment decisions when a person cannot communicate; a separate financial power of attorney is needed for money and contracts. Families usually cope best with a written plan that names a coordinator, assigns specific jobs, records information securely, and is reviewed as needs change.

What family caregiving involves—and why a plan matters

Caregiving is assistance with daily living, health-related tasks, transportation, supervision, paperwork, or household needs. It may include bathing, meals, medication reminders, shopping, appointment coordination, mobility help, or complex medical and nursing tasks. The care recipient may live at home, with a relative, in assisted living, or in a nursing facility.

The Administration for Community Living reported in 2025 that 63 million Americans provide ongoing care. The average caregiver provides 27 hours each week; 24% provide more than 40 hours. Fifty-five percent perform complex medical or nursing tasks, yet only 22% receive training. AARP Public Policy Institute valued family caregiving at $600 billion in 2024. These figures describe unpaid work nationally, not what any one family must provide or what a particular state will reimburse.

A written plan prevents assumptions from becoming crises. The National Institute on Aging describes splitting caregiving responsibilities as a multistep process. Start with the care recipient and all people willing to help, then document needs, assignments, contacts, costs, and limits. Use a paper notebook or a secure digital equivalent with access limited to the people who need it.

Information worth keeping in one shared record

  • Current medication and allergy lists, clinicians, pharmacies, diagnoses, and appointment dates.
  • Emergency contacts, preferred hospital, insurance and benefits paperwork, and copies of signed authorizations.
  • A calendar of transportation, visits, meals, personal-care shifts, respite, and handoffs.
  • Receipts, mileage, paid-care invoices, reimbursements, and who approved each expense.
  • Notes about changes in symptoms, safety concerns, questions for clinicians, and decisions made by the team.

Review the plan after a hospitalization, diagnosis, fall, move, change in work schedule, or any increase in supervision. A plan that cannot be followed with the available time, money, transportation, and health support needs to be redesigned rather than quietly assigned to one exhausted person.

How much does family caregiving or paid care cost?

Costs depend on location, hours, care intensity, staffing, and whether the service is medical or custodial. The following are 2024 national median annual costs published by Genworth and CareScout in 2025. Monthly figures are simple annual-median divisions by 12, included for budgeting; they are not provider quotes and do not account for regional variation, taxes, deposits, or extra services.

Care setting or service 2024 national median annual cost Approximate monthly equivalent What it generally represents
Homemaker services $75,504 $6,292 Household help and other non-medical assistance
Home health aide $77,792 $6,483 In-home personal care; hours and tasks vary by agency and plan
Adult day care $26,000 $2,167 Structured daytime supervision and activities; medical services vary
Assisted living $70,800 $5,900 Residential housing, meals, and varying levels of personal care
Nursing-home semi-private room $111,325 $9,277 Institutional care with a shared room
Nursing-home private room $127,750 $10,646 Institutional care with a private room

A family should model at least two arrangements—for example, fewer paid home hours plus adult day care versus assisted living—because the cheapest hourly rate may not be the lowest total cost once transportation, missed work, supplies, and overnight coverage are counted. Ask every provider what is included, the minimum shift length, overtime or holiday rates, care-plan limits, and how prices change if needs increase.

What can help pay for care?

  • Medicare: Medicare’s official position is, “Medicare doesn’t pay for long-term care.” It generally does not cover ongoing custodial help with bathing, dressing, meals, or supervision. Short-term skilled services can have different rules, so check the specific benefit rather than treating Medicare as long-term-care insurance.
  • Medicaid: A person who meets the state’s financial and functional rules may qualify for Medicaid long-term services and supports, including home- and community-based services. Programs, waiting lists, covered hours, and estate-recovery rules vary by state.
  • Private long-term-care insurance: Coverage depends on the policy’s benefit trigger, elimination period, daily or monthly limit, inflation protection, and remaining benefit period.
  • Veterans’ benefits: Eligible veterans or survivors may have programs that help with care or attendance. The Department of Veterans Affairs can explain current eligibility.
  • Personal and housing resources: NIA identifies personal funds, reverse mortgages, life-insurance options, annuities, and trusts as possible financing sources. Each can have tax, benefit-eligibility, or legal consequences; obtain qualified advice before transferring assets or borrowing against a home.

Do not use a national median as a quote for a particular city. Confirm current prices and eligibility with the state Medicaid agency, insurer, facility, Veterans Affairs, Area Agency on Aging, or Eldercare Locator.

Can you get paid to take care of a parent or another relative?

Sometimes. NIA reports that many states have payment pathways for family caregivers through Medicaid home- and community-based services, adult foster care, or personal-care programs. USAGov likewise notes that some programs and insurance policies may pay a family member or friend. Payment is not automatic: the care recipient usually must meet program criteria, and the caregiver may have to enroll, pass a background check, document hours, complete training, or use an approved agency.

Questions to ask before relying on caregiver pay

  1. Which state program applies—Medicaid self-direction, a personal-care benefit, adult foster care, veterans’ assistance, or an insurance policy?
  2. Does the program allow the specific relationship, such as an adult child, spouse, sibling, or friend?
  3. What functional assessment, income and asset limits, service plan, hourly cap, and waiting list apply?
  4. Who is the employer of record, and who handles payroll taxes, workers’ compensation, time sheets, supervision, and backup coverage?
  5. Are training, background checks, electronic visit verification, or agency enrollment required?
  6. How will wages affect Medicaid, Supplemental Security Income, taxes, or other benefits?

Because allowed relatives, rates, enrollment rules, and covered tasks differ, contact the state Medicaid agency and the local Area Agency on Aging before making a job or housing decision. Keep written approval and time records; an informal promise from a relative or facility is not a benefit determination.

Who has legal authority to make medical and financial decisions?

Health-care proxy or durable power of attorney for health care

A health-care proxy—also called a durable power of attorney for health care—names an agent to make treatment decisions if the person cannot communicate. It is an advance directive. The document normally takes effect under the conditions specified by state law and the form, not merely because a relative is present. State rules govern the form, eligible agents, witnesses, notarization, revocation, and when the authority begins.

Living will

A living will records the person’s preferences about treatments and end-of-life care. It guides the proxy and clinicians but does not give anyone authority over bank accounts or contracts. Give current copies to the agent, clinicians, hospital, and other people identified in the plan, and replace outdated versions when preferences or law change.

Financial power of attorney

A financial power of attorney is separate. It can authorize tasks such as paying bills, managing benefits, signing a lease, or handling a bank account. A health-care proxy does not, by itself, grant banking authority. State forms and execution requirements differ, so use the state’s current form and local legal guidance.

Contracts and personal liability

Read facility and home-care agreements carefully. Signing as a guarantor or cosigner can make the signer personally liable for charges even when the care recipient is the resident. Sign only in the capacity actually authorized, ask the facility to identify the responsible party, and obtain local legal advice before agreeing to payment guarantees.

If the person still has decision-making capacity, discuss documents early rather than during an emergency. If capacity is uncertain or documents conflict, a state legal-aid organization or bar referral service can explain the options. This is general information, not individualized legal advice.

How should siblings and relatives divide responsibilities?

Equal hours are rarely realistic. A fair plan matches tasks to skills, interests, location, work schedules, health, transportation, and money, while giving the care recipient a voice whenever possible.

  1. Hold a calm meeting. Include the care recipient, available relatives, close friends, and—when useful—a social worker or care manager. Agree on the immediate safety issue before debating old family grievances.
  2. List present and anticipated needs. Separate daily personal care, medical coordination, transportation, meals, household work, finances, legal paperwork, respite, and emotional support. Estimate frequency and time for each.
  3. Name a coordinator. One person can maintain the calendar and shared record, but coordination is a role—not ownership of every task. Set a backup for illness or travel.
  4. Assign specific, measurable jobs. “Handle Tuesday transportation” is clearer than “help more.” Record who does the task, when, how handoff occurs, and what happens if it is missed.
  5. Set boundaries and a budget. Each participant should state limits around work, travel, health, overnight care, and spending. Decide how approved costs are shared or reimbursed.
  6. Schedule review dates. Revisit the plan regularly and after a crisis, because a new diagnosis, fall, or hospitalization can change the workload.

Example responsibility map

Need Primary assignment Backup or remote support Record to maintain
Medication and appointments Local relative confirms schedule and attends priority visits Sibling joins by phone and tracks questions Medication list, calendar, clinician contacts
Transportation Relative or contracted service books rides Remote sibling confirms arrival and payment Ride details, cancellations, receipts
Meals and household tasks Rotate visits, grocery delivery, or paid homemaker Remote relative orders supplies Meal plan, delivery log, expense record
Respite Schedule adult day care, in-home aide, or substitute family shift Remote relative funds or researches options Approved hours, emergency contacts

What can a long-distance caregiver do?

Distance does not eliminate useful work; it changes the work toward coordination, administration, and reliable check-ins. With the right authorization, a remote caregiver can schedule calls with clinicians, maintain the shared record, manage benefits paperwork, compare agencies, arrange transportation, order groceries, coordinate respite, and help pay approved expenses. Ask the nearby caregiver what would actually reduce their load before adding another app, call, or reporting requirement.

  • Set a predictable video or phone schedule and a separate emergency escalation plan.
  • Keep a current list of local contacts: neighbors, paid aides, clinicians, pharmacy, transportation, and the person with keys.
  • Use written handoffs after appointments so medication or care changes are not lost in text messages.
  • Arrange backup coverage for holidays, illness, and delayed travel; do not make one nearby person the only emergency option.
  • Offer emotional support without treating the local caregiver as an employee who must be available constantly.

Digital tools can help, but protect sensitive health and financial information with strong, unique passwords, multi-factor authentication where available, device locks, and carefully limited sharing permissions. Confirm that every person receiving updates has the legal authorization required by the clinician, insurer, bank, or agency.

How to compare home care, adult day care, assisted living, and nursing homes

Price alone does not identify the right setting. Compare each option on the same eight dimensions:

Decision factor Questions to answer
Medical versus custodial scope Which tasks are permitted, and what licensed staff are available?
Hours and intensity Is coverage daytime only, overnight, intermittent, or continuous?
Total cost What is the monthly and annual total after transportation, supplies, deposits, and extra services?
Who pays Is payment private, Medicaid-funded, insurance-funded, veterans’ assistance, or mixed, and what eligibility limits apply?
Location and travel How far must family travel, and is reliable transportation available?
Training and respite Who trains family members, and what relief is available when a caregiver is sick or away?
Safety and oversight How are incidents, staffing, medication handling, complaints, and quality monitored?
Ability to scale Can the arrangement add hours or higher-level care without another disruptive move?

When the plan is failing

Rework the arrangement promptly if medication errors increase, appointments are missed, the caregiver is losing sleep or work, bills go unpaid, the care recipient is unsafe alone, or no one can cover an emergency. Escalate to a clinician, social worker, Area Agency on Aging, Medicaid caseworker, or Veterans Affairs contact as appropriate. A crisis is evidence that the current task list, staffing, authority, or funding no longer matches the need—not a reason to blame one family member.
