Fraud prevention in federal relief programs depends on layered controls applied before money moves: verify identity and eligibility against authoritative data, compare applications across programs, document risk decisions, and route suspicious cases to investigators. During COVID-19, agencies had to deliver aid at unprecedented speed, and later reviews found that screening, data sharing and accountability were not consistently strong enough. The result was a system that recovered and prosecuted many cases but too often operated in the “pay and chase model” that the U.S. Government Accountability Office (GAO) says agencies should avoid.
This distinction matters when reading the numbers. Convictions, civil judgments, improper-payment estimates and calculated fraud losses are different measures with different dates and coverage; none is a complete total for all federal relief fraud.

Which federal relief programs were exposed?
Four emergency-relief laws appropriated $2.6 trillion, according to GAO in 2020. The money flowed through several systems rather than one program, including the Small Business Administration’s Paycheck Protection Program (PPP) and COVID-19 Economic Injury Disaster Loan (EIDL) program, unemployment insurance, and Treasury and Internal Revenue Service economic-impact payments.

The scale alone created a control problem. As of May 31, 2020, Treasury and the IRS had issued 160.4 million payments worth $269.3 billion. Processing that volume quickly required agencies to decide how much verification could occur before payment and how much would be investigated afterward.

How fraud prevention is supposed to work
Prepayment eligibility checks
Front-end controls test whether an applicant, business or payment recipient meets the program’s rules before funds are disbursed. They can check required fields, entity status, dates, loan or benefit limits and whether the stated purpose matches the program. GAO’s central prevention lesson is to build these checks before payment instead of relying primarily on recovery later.
Identity and authoritative-data verification
Agencies compare names, addresses, tax information, Social Security numbers and other identifiers with authoritative government records. The goal is to detect invented or stolen identities, deceased or nonexistent recipients, questionable Social Security numbers and entities that are barred or otherwise ineligible. A match is a risk signal, not automatically proof of fraud; legitimate applicants need a documented way to resolve false positives.
Cross-program analytics
Applicants can appear in multiple relief systems, so agencies need to compare records across programs. Duplicate applications, inconsistent payroll or income figures, simultaneous claims that cannot all be true, and repeated use of the same bank account or identity are examples of cross-program anomalies. GAO recommended stronger cross-program analytics and access to external data sources after reviewing pandemic-relief controls.

Risk assessment, accountability and audit trails
Regular risk assessments identify which rules, populations and payment channels are most vulnerable as conditions change. Agencies also need named owners for screening, clear referral thresholds, records showing why a payment was approved or stopped, and inspector-general access to those records. These controls make it possible to improve a process during an emergency rather than discover its gaps years later.
An exception path for urgent, legitimate cases
Emergency programs cannot treat every data mismatch as a denial. A safer design separates high-confidence approvals from cases requiring human review, allows time-limited exceptions when the public need is urgent, and records the evidence and approving official. That preserves speed without making verification optional for everyone.
What SBA did in PPP and COVID-19 EIDL
GAO reviewed SBA documentation, inspector-general reports, Pandemic Response Accountability Committee (PRAC) material and the independent financial-statement auditor’s work. SBA used a four-step process to detect and refer likely fraud in PPP and COVID-19 EIDL. The control points were:

- Screen the application and borrower information. Basic eligibility and identity information was checked against the data available to SBA.
- Compare records with government and external sources. This included checks such as the federal Do Not Pay system and other data that could reveal prohibited or inconsistent participants.
- Assess anomalies and supporting evidence. Suspicious identities, questionable Social Security numbers, duplicate or cross-program activity and other indicators were evaluated to determine whether the record required escalation.
- Refer likely fraud for investigation. Cases meeting referral criteria were sent to the appropriate investigative or enforcement body, with documentation intended to support follow-up.
GAO found weaknesses in both screening and referral controls. In some cases, potentially ineligible entities listed in Do Not Pay received funds without sufficient supporting evidence that the warning had been resolved. The finding illustrates why a database match must trigger a documented decision, not merely appear in a screen that no one acts on.
Signals investigators looked for
- Identities that could not be verified or appeared to be fabricated or stolen.
- Questionable or mismatched Social Security numbers and other identifier anomalies.
- Entities that were ineligible under a program’s rules or appeared on exclusion and payment-screening lists.
- Duplicate applications, repeated use of contact or banking details, and implausible application patterns.
- Claims or loans spanning programs in ways that conflicted with eligibility, payroll, income or timing rules.
- Data-matching results that connected an applicant to prior suspicious activity or authoritative records inconsistent with the application.
Who investigated and enforced pandemic-relief fraud?
No single office handled every case. Oversight and enforcement operated as a network:
| Body | Primary role |
|---|---|
| Agency inspectors general | Audit programs, investigate suspected misconduct and report control failures within agencies such as SBA, Treasury, Labor and Health and Human Services. |
| GAO | Review government-wide implementation, quantify documented problems and recommend improvements to Congress and agencies. |
| PRAC | Coordinate oversight and data analysis across pandemic-response programs. |
| Department of Justice COVID-19 Fraud Enforcement Task Force | Coordinate criminal and civil investigations and prosecutions across federal and other law-enforcement partners. |
| SBA, Treasury, Labor, HHS and other agencies | Operate payment programs, apply controls, suspend or refer suspicious transactions and support investigations. |
DOJ’s 2024 task-force report describes coordinated investigations and prosecutions across these bodies. Enforcement can include criminal charges, civil False Claims Act matters, asset recovery and administrative actions such as suspension or debarment.

What the published figures do—and do not—show
| Measure | Figure and date | How to interpret it |
|---|---|---|
| Federal criminal cases | At least 1,044 people pleaded guilty to or were convicted at trial of federal charges for defrauding COVID-19 relief programs through January 13, 2023 (GAO, 2023). | A count of people with documented guilty pleas or trial convictions by that date, not a dollar estimate of all fraud. |
| Criminal and civil outcomes | At least 1,399 individuals or entities were found guilty or liable in COVID-19 relief fraud cases from March 2020 through June 2023 (GAO, 2024). | Combines criminal guilt and civil liability and therefore is not directly comparable with the criminal-only figure. |
| DOJ civil enforcement | More than 650 civil settlements and judgments totaling over $500 million through December 31, 2024, according to DOJ figures reported by GAO in 2025. | An enforcement-recovery total for civil matters, not a comprehensive estimate of pandemic-relief fraud losses. |
| Improper payments | About $162 billion estimated by federal agencies for fiscal year 2024 (GAO, 2025). | An estimate of payments that may have been incorrect or unsupported under federal improper-payment definitions; it is not a COVID-19 fraud total. |
These measures answer different questions. A conviction shows a case reached a particular legal outcome; a civil judgment or settlement resolves a civil claim; an improper-payment estimate covers error and noncompliance as well as potential fraud. Treating them as one grand loss number would overstate what the evidence establishes.
Where the emergency model broke down
Speed versus verification depth
Relaxed or abbreviated checks helped legitimate applicants receive aid quickly, but every skipped check increased the need for reliable data and later review. Programs that launched before scalable screening was ready often had to build controls while payments were already moving.

Agencies did not always have timely access to one another’s records or to external sources. Without a shared view, a person or entity could look eligible in one program while appearing contradictory or duplicative across several programs.
Best Value
Unclear referral ownership
A warning is useful only when someone must resolve it. Weak referral thresholds, incomplete documentation or unclear responsibility can allow a flagged application to proceed without a defensible explanation.
Recovery is slower than prevention
Investigations, litigation and asset recovery can take years, and some funds cannot be recovered. Postpayment work is essential, but it cannot substitute for controls that stop high-risk transactions before disbursement.
A practical framework for the next emergency
- Map the fraud risk before launch. Define the affected program, participants, likely fraud activities, mechanisms used to execute them and potential impacts. GAO describes those five elements as the structure of a federal fraud scheme.
- Build tiered prepayment controls. Automate low-risk approvals, pause high-risk matches and require documented human review for exceptions.
- Secure authoritative data access. Establish agreements and technical connections to identity, tax, exclusion, death, business-registration and payment data before an emergency, not after suspicious claims appear.
- Run cross-program matching. Use consistent identifiers and analytics to detect duplicates, incompatible claims and repeated banking or contact details.
- Assign accountability. Set owners, referral thresholds, escalation deadlines and audit-log requirements, with inspectors general and oversight bodies able to inspect the trail.
- Measure and adapt continuously. Track false positives, prevented payments, referrals, investigative outcomes, recoveries and unresolved control gaps. Update rules as fraud patterns change.
- Prepare the recovery pipeline. Preserve evidence, coordinate with DOJ and inspectors general, and make suspension, recoupment and civil recovery procedures ready before the first suspected case.
The enduring lesson is not to choose between fast aid and strong controls. It is to design both: scalable checks that operate before payment, a fair path for legitimate exceptions, and an oversight system capable of acting when the data shows risk.
