Epic says the feature is planned for release in summer 2027 and is not yet a generally available consumer product.
Amazon Pharmacy, Mark Cuban Cost Plus Drugs, Costco and Eli Lilly’s LillyDirect are the named collaborators. Epic also describes a broader network in which any pharmacy can provide cash prices to participating Epic health systems.

What Epic announced
Prescription Marketplace is intended to put price information into the prescribing workflow rather than leaving patients to discover the final cost at the pharmacy counter. For a selected medication and dose, the system is expected to present the patient’s insurance price alongside available cash prices from participating pharmacies and programs.

Epic has not published a final consumer-facing screen, a universal list of participating pharmacies, the number of health systems that will support the feature, or its initial geographic coverage.
How the prescription price comparison is expected to work
- Select the medication and dose. The clinician starts a prescription in an Epic health system, using the same drug, strength and quantity that would otherwise be sent to a pharmacy.
- Check insurance pricing. Epic’s existing prescription-benefit tools can return an estimate of the patient’s copay or coinsurance under the active plan.
- Check cash options. Prescription Marketplace is planned to retrieve cash prices from participating pharmacies and programs, including any applicable member pricing and delivery methods.
- Compare fulfillment choices. The patient and clinician can weigh the insurance price against cash payment, delivery or pickup, pharmacy availability and other terms.
- Send the prescription. After choosing an option, the clinician sends the prescription through the appropriate fulfillment channel. The displayed amount should be treated as an estimate until the pharmacy confirms the final price and eligibility.
Epic’s Open documentation describes real-time cash-price checking through its Nebula cloud platform. Pharmacies and vendors that want to join the Pharmacy Prices network can request information through Epic’s Technology Guidance Form. The documentation indicates that the network is intended to support more than a fixed group of launch partners.

Amazon Pharmacy vice president and general manager Tanvi Patel said customers should be able to choose whether to use insurance or pay cash and select delivery or pickup when available. Costco assistant vice president Ricky Li said clearer pricing could reduce prescription abandonment. LillyDirect general manager Jen Mazur said patients should not have to navigate the health system alone to understand filling options.

Why cash-versus-insurance comparisons matter
Epic says more than 40% of American adults report not taking medicines as prescribed because of cost. A lower price found before the prescription is transmitted could prevent a patient from declining treatment, delaying a refill or abandoning a prescription at the counter.

Insurance is not automatically the least expensive route. Depending on the medication, plan design and pharmacy, a cash price can be below a copay or coinsurance amount. The reverse is also possible, particularly when an insurance benefit or manufacturer assistance reduces the patient’s out-of-pocket cost.
Epic says its existing Real-Time Prescription Benefit feature helped patients save more than $250 million during 2025. That figure refers to the existing benefit tool, not savings already produced by Prescription Marketplace, which has not launched.

How to judge the options shown
A useful comparison needs more than one headline number. For the same medication, strength, quantity and formulation, check:

| Insurance cost | Is the figure a copay, coinsurance estimate or deductible-season price? |
| Discounts | Does the price depend on a manufacturer offer, pharmacy membership or other eligibility rule? |
| Availability | Can the pharmacy fill the prescription in the patient’s state and currently stock it? |
| Speed and method | When will the medicine be ready, and is delivery or pickup available? |
| Support | Can a pharmacist answer questions, transfer the prescription or coordinate refills? |
| Prescription rules | Is a valid prescription required, and are controlled, refrigerated or specialty medicines handled differently? |
The lowest displayed amount may not be the best choice if it cannot be filled locally, arrives later than treatment allows or excludes a service the patient needs. Patients should confirm the final price with the pharmacy before payment.
What remains unknown before release
- Epic has announced a summer 2027 target, but not a specific launch date.
- The company has not stated how many Epic health systems will enable the feature at launch.
- Initial state-by-state coverage and the number of pharmacies in the network have not been published.
- No final patient interface, notification method or consent flow has been shown.
- There is no guarantee that every medication, insurance plan or pharmacy will return a comparison.
- Prices, inventory, delivery windows and eligibility can change after a result is displayed.
Where LillyDirect fits in the broader picture
LillyDirect is one of the named collaborators, but its reported reach should not be confused with Prescription Marketplace usage. Eli Lilly says LillyDirect reached more than one million patients in its first two years. The company also reported that LillyDirect accounted for approximately 49% of new-to-brand Zepbound prescriptions in the fourth quarter of 2025. Those figures describe LillyDirect’s own reported scale and are not measurements of Epic’s marketplace.
What patients can do now
- Ask the prescriber to check the insurance benefit and whether a cash price is available for the exact prescription.
- Request the pharmacy’s complete out-of-pocket amount, including delivery or membership charges.
- Confirm that the pharmacy can fill the prescription in the patient’s location and has the medication in stock.
- Ask whether a discount or assistance program changes eligibility, refill timing or the ability to use insurance.
- Have the pharmacy verify the final amount before paying, because estimates can change with plan status, inventory and local rules.
Prescription Marketplace could make those checks visible during a clinical visit, but its availability and coverage will depend on the Epic health system, pharmacy and prescription involved.
