Oracle Health and MEDITECH are the clearest enterprise hospital alternatives to include on an Epic shortlist. Neither should be chosen on name recognition or broad capability claims alone: fit depends on your facilities, care settings, workflows, interfaces, migration capacity, operating model and total cost. Compare vendors through the same scripted scenarios, integration requirements and reference checks.
Which Epic alternatives belong on a hospital shortlist?
For a hospital or health system seeking an enterprise EHR, start by evaluating Oracle Health and MEDITECH alongside Epic. ONC’s 2024 hospital certified health IT reporting identifies all three among the most reported developers by participating hospitals. That establishes meaningful hospital use, not that any platform is best for a particular organization.

ONC counts hospitals reporting certified products from developers; it is not an exclusive measure of primary-EHR market share, and a hospital may report more than one developer. The available evidence does not support a quality ranking or a claim that one vendor will be less expensive, faster to implement or more satisfying to clinicians.

Consider Oracle Health as an enterprise-scale candidate, then test the specific clinical modules, integration routes, transition approach and service model your organization needs. Oracle describes FHIR APIs, developer resources and health information exchange capabilities. Those are vendor-provided capability statements, not confirmation that a particular interface is available, included, affordable or ready for your workflow. Ask Oracle to demonstrate the exact configuration and identify terms, partner dependencies and implementation requirements.

MEDITECH
MEDITECH is another enterprise hospital candidate with demonstrated use in ONC’s hospital developer reporting. Confirm which product edition and modules are proposed, how they cover your clinical and administrative workflows, what integrations are supported, and how implementation and ongoing support will work. Ask for references from organizations with comparable size, care settings and complexity.
Other vendors require a scope check
CMS lists athenahealth, eClinicalWorks, Epic, MEDITECH and Oracle on its EHR pledge page. Its electronic prior authorization early-adopter list includes athenahealth, eClinicalWorks, Epic, MEDITECH, Modernizing Medicine, Oracle and TruBridge. These lists show participation in initiatives; they do not establish equivalent suitability for a large acute-care health system. Consider other vendors when their product scope and customer references match your facilities and care settings.

How to compare vendors on your organization’s needs
Build a requirements matrix before demonstrations. Include every hospital and care setting in scope, identify the people who own each workflow, and distinguish essential requirements from preferences. Compare proposed product editions and modules rather than vendor names in the abstract.

| Organization and care settings | Fit for a single hospital or multi-hospital system, acute and ambulatory care, academic or community settings, and required specialties. | A site- and service-line-specific module map; references from organizations with a similar footprint. |
| Clinical and administrative workflows | Coverage of inpatient documentation, orders, medication management, ancillary systems, revenue cycle, scheduling, prior authorization, quality reporting and patient engagement. | End-to-end demonstrations using your roles, scenarios and exceptions, not a generic product tour. |
| Interoperability | Whether each required exchange works for the partner, data, direction and workflow involved. | An interface inventory naming the method, data elements, production status, dependencies, charges and support owner. |
| Migration and operating model | Conversion scope, historical-record access, implementation sequence, downtime planning, training, staffing, hosting, upgrades and optimization. | A transition plan with assumptions and contingencies, plus references from comparable implementations. |
| Economics and governance | Full ownership costs, decision rights, contract terms and responsibilities across vendor and customer teams. | A line-item five-to-ten-year model stating inclusions, exclusions, escalation terms, interface charges and internal labor assumptions. |
| Regulatory readiness | Certification status and the particular interoperability and electronic prior authorization workflows applicable to your organization. | Written confirmation of the proposed product’s status, required modules and implementation responsibilities; review applicability with counsel. |
Prove workflow-level interoperability, not just API availability
ONC’s analysis of 2024 AHA IT Supplement data found that about nine in ten hospitals enabled patient access through an API, while seven in ten reported using a standards-based API for that patient access. Those figures concern patient access; they do not show that every clinical or administrative exchange uses a standards-based API.

For third-party clinical and administrative use cases, ONC found that data exchange often relies on proprietary APIs or non-API approaches as well. A product’s FHIR support or general interoperability statement therefore cannot establish that a specific connection will meet your requirements. For each required exchange, ask the vendor to state whether it uses a standards-based API, proprietary API, HL7 interface or another method, and what it takes to deploy and maintain it.
Use an interface-by-interface inventory
List each required partner and workflow, including the direction of exchange and the data needed. Ask for the interface type, production status, partner dependencies, implementation effort, fees and named support owner. Verify the answer with the relevant partner where possible; an available standard does not guarantee that both systems implement the same data or workflow.

Run comparable demonstrations and reference checks
Give every shortlisted vendor the same scenarios and scoring criteria. Include an ordinary workflow and a realistic exception—for example, a handoff between inpatient and ambulatory care, an ancillary result, a revenue-cycle task or a prior-authorization case. Require demonstrations in the proposed product edition and configuration, and record any capability that depends on a separate module, partner or future development.

- Set the scope. Name facilities, service lines, user roles, required modules, partner systems and must-have exchanges.
- Script the scenarios. Use the same end-to-end inpatient, ambulatory, ancillary, revenue-cycle and prior-authorization tasks for each vendor.
- Inspect the proposed solution. For each step, ask what is generally available now, what requires another module or partner, and what is only a roadmap statement.
- Validate implementation claims. Request conversion scope, historical-record access, test and cutover plans, downtime contingencies, training assumptions and post-go-live staffing.
- Call comparable customers. Ask references about the same kinds of facilities, complexity and workflows, and discuss both implementation and ongoing operation.
- Compare the economics. Require a line-item five-to-ten-year cost model and reconcile its assumptions against your own staffing and interface inventory.
Account for migration and long-term operating work
Changing an EHR is not just a software selection. The organization must decide what data to convert, how staff will access records that remain in an archive, which sites go live in what order, and how to manage downtime, training and post-launch support. Ask vendors to make those decisions visible in their proposed plan instead of treating them as later implementation details.
Best Value
Compare the operating model as well as the initial transition: hosting, upgrades, support ownership, internal staffing and ongoing optimization can affect whether the system remains workable after go-live. Obtain references from systems similar to yours and ask what the proposed plan assumes the customer will supply.
Check regulatory applicability and dates
CMS’s electronic prior authorization materials point to FHIR-based API exchange requirements for impacted payers and January 1, 2027 readiness. That date and the stated payer requirements do not by themselves determine which obligations apply to a particular hospital or how its EHR must be configured. Confirm applicability and implementation details with counsel and the vendors, including the relevant product edition, modules and responsibilities.
CMS initiative participation is not an endorsement or proof of readiness for your specific workflows. Ask vendors to identify the applicable requirements, the capabilities available in the proposed configuration, and any remaining work or dependencies.
