Double Emulsions for Co-Delivering Drugs: What We Know

Double emulsions could carry two otherwise incompatible active ingredients in one microscopic structure, but they do not by themselves prove that a drug combination will be synergistic, safe, effective, approved or ready for routine treatment. In the most studied water-in-oil-in-water (W/O/W) design, a small inner water droplet sits inside an oil droplet, and that oil droplet is dispersed through a second, external water phase. Hydrophilic cargo can occupy the inner water, while a hydrophobic compound can reside in the oil layer.

That compartmental arrangement makes co-encapsulation physically plausible and may enable staged or sustained release. It also creates difficult engineering problems: water can move between aqueous compartments, droplets can swell, shrink, merge or separate, and the process must be reproduced reliably at useful scale. The concept is therefore a formulation strategy under active study, not established combination therapy.

What a double emulsion is

An ordinary emulsion disperses one liquid as droplets in another immiscible liquid, such as oil in water. A double emulsion contains an emulsion inside another emulsion. The nested interfaces create at least three compartments and two boundaries that must be stabilized.

W/O/W: the architecture relevant to combination delivery

In a water-in-oil-in-water system, the innermost phase is water, the middle phase is oil, and the continuous outer phase is water. The structure can be represented as:

inner water droplets → oil droplets → external water

A water-soluble molecule can be dissolved in the inner water. A poorly water-soluble, oil-compatible molecule can be dissolved in the middle oil. The outer water phase may contain buffers, salts, polymers or other excipients, depending on the formulation. The compartments are physically separated, so the two actives do not have to share the same solvent during processing.

O/W/O is a different system

Oil-in-water-in-oil (O/W/O) reverses the arrangement: oil droplets are enclosed in water droplets, which are dispersed in an external oil phase. Its partitioning behavior, emulsifier choices and release pathways differ from W/O/W. A claim about one architecture should not be generalized to the other.

How the structure could carry a combination

The key idea is selective partitioning rather than an automatic pharmacological interaction. Hydrophilic and hydrophobic compounds often require different solvents and can destabilize one another if forced into a single phase. In W/O/W, the hydrophilic ingredient can remain in the internal water while the hydrophobic ingredient is dissolved in the oil shell. Both can then be transported in one population of composite droplets.

Two complementary emulsifiers are normally needed. One stabilizes the inner water–oil interface; another stabilizes the oil–outer-water interface. Their chemical balance, concentration, order of addition and compatibility with the active ingredients determine whether droplets survive processing and storage. The carrier architecture can improve physical co-location and protect a compound from its immediate environment, but it cannot demonstrate that the ingredients act better together in a patient.

How manufacturers and laboratories make double emulsions

Different fabrication routes trade off droplet-size control, energy input, leakage, throughput and ease of scale-up. The following methods are described in the technical literature; a specific drug product would require formulation-specific optimization.

Two-step emulsification

  1. Prepare the inner aqueous phase with the water-soluble model compound or drug and an emulsifier selected for the inner water–oil boundary.
  2. Disperse that phase into the oil phase to create a primary water-in-oil emulsion. Mixing intensity and residence time influence droplet disruption and size distribution.
  3. Add the primary emulsion to the external aqueous phase containing the second emulsifier. Apply controlled mixing to form oil droplets containing the inner water droplets.
  4. Remove entrained air, adjust pH or osmotic conditions if required, and test droplet size, phase composition, loading and leakage before storage.

This route is conceptually simple and widely used in laboratory work, but the second emulsification step can rupture internal droplets or drive active ingredients into the wrong phase.

Phase inversion

In phase-inversion approaches, composition or processing conditions are changed so that the continuous phase switches. The method can generate nested structures without the same sequence as conventional two-step mixing, but the inversion window is sensitive to formulation composition, temperature, surfactant balance and mixing history. No universal operating recipe or performance ranking for pharmaceutical products is established.

Membrane emulsification

A dispersed phase is pressed through membrane pores into a continuous phase, producing droplets as they detach. Membranes can reduce the violent shear associated with some high-energy mixers and may improve uniformity. Throughput, membrane fouling, pore-size distribution and the need for a second emulsification stage remain practical constraints. Large-scale nondestructive equipment for the second stage has been identified as a barrier in food systems; pharmaceutical equipment and acceptance criteria will differ.

Ultrasonic emulsification

Ultrasound supplies intense local energy that breaks one phase into fine droplets. A probe sonicator or ultrasonic homogenizer is a possible laboratory tool, not a required or validated recommendation. Excessive energy can heat the batch, damage sensitive molecules or rupture the internal water droplets, so amplitude, pulse pattern, temperature and exposure time must be controlled.

How cargo may be released

A double emulsion does not have one guaranteed release profile. The 2026 review by Cao, Liu and Du describes several possible mechanisms:

  • Structural rupture: swelling, shrinkage or shear can break an oil shell or destabilize an interface, releasing the contents.
  • Interphase coalescence: internal and external aqueous droplets can merge through transient contact or interface failure, changing the route and rate of release.
  • Permeation or diffusion: a molecule can migrate across the middle oil phase or through an interface, depending on its partition coefficient, emulsifier film and shell thickness.

Real products may combine these pathways. Temperature, agitation, osmotic pressure, droplet size, oil composition, emulsifier films and the chemical properties of each active all affect the observed curve. A sustained-release result in one model system cannot be transferred to another drug pair without new measurements.

What the evidence actually shows

Model-substance co-release in 2016

E. Dluska, A. Markowska-Radomska, A. Metera and W. Tomaszewski reported “Drug-Core Double Emulsions for Co-release of Active Ingredients” in the International Journal of Chemical Engineering and Applications, 7(6), December 2016 (DOI 10.18178/ijcea.2016.7.6.619). Their work used a Couette–Taylor flow (CTF) bioreactor and model substances to examine simultaneous encapsulation and release behavior. The authors state:

“The presented work aimed to study the effect of simultaneous encapsulation of two different model substances (drugs) in double emulsions formed in a Couette- Taylor Flow (CTF) bioreactor on their release kinetics and mechanisms.”

That experiment is evidence about formulation behavior under specified laboratory conditions. It is not evidence of clinical efficacy, a therapeutic synergy claim or regulatory approval.

Earlier drug-pair example

The 2016 paper cites earlier work investigating metronidazole and ornidazole in internal and external phases of a W/O/W emulsion for vaginal delivery. This example should be attributed to the earlier study as reported by Dluska and colleagues. It does not mean the 2016 CTF study tested that pair, nor that the formulation became an approved therapy.

Broader application reviews

A 2006 review, “Multiple emulsions: an overview,” discussed possible uses including controlled or sustained delivery, targeted delivery, taste masking and bioavailability enhancement. These are research applications and historical context, not proof of current clinical adoption. The 2026 Cao, Liu and Du review focuses on bioactive delivery and sports nutrition, so its observations should not be read as pharmaceutical approval or patient-outcome evidence.

Why double emulsions are hard to stabilize

Nested droplets are thermodynamically unstable and must be kept intact by kinetic stabilization. The 2026 review identifies several failure modes:

  • Gravity separation: density differences can make droplets cream upward or sediment downward.
  • Flocculation: droplets cluster without immediately merging, changing flow and apparent concentration.
  • Coalescence: protective films fail and neighboring droplets merge into larger droplets.
  • Water diffusion: water can move between inner and outer aqueous compartments through the oil phase or defects in the interfacial films.
  • Osmotic swelling or shrinkage: an imbalance in solute concentration draws water into or out of inner droplets, stressing the oil shell.

These effects can cause leakage, dose non-uniformity, altered release timing or visible phase separation. A formulation that looks stable immediately after mixing may behave differently after transport, temperature cycling, dilution or mechanical agitation.

Comparing preparation routes

Method Inner-phase retention and leakage Release control Energy and droplet disruption Scale-up and repeatability
Two-step emulsification Depends strongly on second-stage shear and interfacial films; leakage must be measured for each formulation. Adjustable through droplet size, shell composition and osmotic balance; no universal profile. Moderate to high mixing can rupture internal droplets. Familiar laboratory workflow, but second-stage scale-up can be difficult.
Phase inversion Sensitive to composition and inversion conditions; formulation-specific data required. Potentially tunable during the inversion window. Depends on the inversion path and mixing history. Repeatability depends on tightly controlled composition, temperature and processing.
Membrane emulsification Gentler droplet formation may help retention, but membrane and second-stage losses remain concerns. More uniform primary droplets may aid control. Generally lower shear at the membrane than aggressive homogenization. Throughput, fouling and large-scale second-stage equipment are barriers noted in food-system reviews.
Ultrasonic emulsification High local shear can increase disruption or leakage if over-processed. Fine droplets can change diffusion distances, but release still requires testing. High local energy; heat and molecule damage must be managed. Batch-to-batch control requires calibrated amplitude, pulse, temperature and time.

The comparison criteria that matter for a pharmaceutical decision are inner-phase retention, encapsulation efficiency, release control, storage stability, processing energy, scalability, repeatability and material compatibility. Food-grade emulsifier limitations reported in food reviews should not be generalized to every pharmaceutical formulation; pharmaceutical excipients follow a different regulatory and compatibility assessment.

What a pharmaceutical development program would have to prove

  1. Define the therapeutic rationale. Specify why the two actives need co-delivery, what exposure relationship is intended and whether each compound remains chemically stable in its assigned compartment.
  2. Map partitioning. Measure how much of each active is in the inner water, oil and external water phases after manufacture and during storage.
  3. Quantify encapsulation and leakage. Report loading, encapsulation efficiency, initial burst and leakage after defined temperature, agitation and dilution challenges.
  4. Control structure. Characterize droplet-size distribution, internal-water fraction, shell thickness where measurable, viscosity and visible separation.
  5. Stress the product. Test osmotic changes, freeze–thaw or temperature cycling where relevant, light, oxygen, transport vibration and intended-use shear.
  6. Establish release methods. Use validated analytical assays for both actives and distinguish diffusion, rupture and coalescence contributions rather than reporting a single combined signal.
  7. Demonstrate manufacturing repeatability. Show that independent batches meet the same critical quality attributes and that scale-up does not change droplet structure or dose distribution.
  8. Complete safety and clinical work. Evaluate excipient tolerability, impurities, biodistribution, pharmacokinetics and clinical outcomes for the actual formulation. None of those conclusions can be inferred from the carrier architecture alone.

Where the field stands

Double emulsions offer a credible physical strategy for placing hydrophilic and hydrophobic compounds in one carrier and for exploring controlled release. The strongest evidence cited here is experimental formulation work and broad reviews, not a demonstrated clinical combination product. Translation depends on solving water transport, osmotic balance, coalescence, leakage, storage stability, process repeatability and scale-up with a specific drug pair and a defined regulatory route.
