That does not mean the first week determines a child’s future, or that changing those measurements prevents eczema or food allergy. The evidence concerns different outcomes and populations, so it is not a single, settled rule about newborn care.
What first-week studies have found
The evidence includes observational studies of biological markers and a randomized trial of skin care. These designs answer different questions: an association can identify a pattern, while a trial tests an intervention in a defined group.

Gut-microbe diversity and later eczema
Ismail and colleagues followed 98 infants considered at high risk of allergic disease. Stool samples collected at one week showed lower microbial diversity among infants who later developed eczema by 12 months than among those who did not. The study found no differences between infants classified as atopic and non-atopic. This is an association in a high-risk group, not evidence that changing an infant’s microbiome would prevent eczema. Ismail et al., 2012.

Skin-barrier water loss
A study analyzing 116 infants reported that forehead transepidermal water loss (TEWL)—an instrument-measured indicator of water passing through the skin barrier—within the first week was associated with subsequent atopic dermatitis. This finding is about a measurement in a study cohort; it does not establish a routine newborn screening test or a universal clinical cutoff. Study indexed in 2015.

What an emollient trial can—and cannot—tell parents
The STOP-AD randomized trial recruited term infants within four days of birth if a parent had atopic dermatitis, asthma, or allergic rhinitis. It compared emollient application twice daily for the first eight weeks with standard routine skin care, assessing atopic dermatitis at 12 months. Its results apply to that selected, higher-risk population and that eczema outcome; they do not establish that every newborn benefits or that emollients prevent food allergy. Ní Chaoimh et al., 2022.
A separate PEBBLES publication is a trial protocol for a ceramide-dominant emollient strategy in infants with a family history of allergic disease. A protocol lays out planned methods and outcomes; it is not evidence that the approach worked. PEBBLES trial protocol.

How guidelines frame newborn skin care and food-allergy prevention
WHO advice on skin conditions
WHO states: “Routine application of topical emollients in term healthy newborns for the prevention of skin conditions is not recommended.” The guidance says evidence did not establish benefits or harms, and studies in high-risk, preterm, and small-for-gestational-age newborns were not considered for that recommendation. It therefore should not be read as resolving whether a particular intervention may help a separately defined high-risk group. WHO newborn skin-care guidance.

The EAACI 2020 food-allergy prevention guideline suggests avoiding regular cow’s-milk formula as supplementary feed for breastfed infants during the first week; certainty of evidence is low. Separately, it makes no recommendation for or against emollients to prevent food allergy. These statements address food-allergy prevention, not proof that the first week controls eczema risk. How to interpret the evidence for an individual baby
- Keep outcomes distinct: eczema or atopic dermatitis, food allergy, allergic sensitization, and broader atopy are not interchangeable endpoints.
- Keep populations distinct: findings in infants with family risk do not automatically apply to all healthy term newborns or to preterm infants.
- Keep association separate from prevention: a marker linked with later eczema does not prove that testing for it or changing it will alter the outcome.
- Read recommendations within their scope: WHO’s statement concerns routine emollient use to prevent skin conditions in term healthy newborns; EAACI’s first-week formula advice concerns food-allergy prevention.
A Cochrane review examines infant skin-care interventions intended to prevent eczema and food allergy, including interventions begun in the first week. Its scope helps place individual trials in a broader evidence base, but the specific findings should be read from the review rather than inferred from a search summary.
