Short answer: Spirulina (usually referring to Arthrospira species) has a biologically plausible atheroprotective pathway, but it has not been shown to prevent or treat atherosclerosis in people. Human studies mainly report changes in cholesterol and triglycerides—surrogate risk markers—not fewer heart attacks, strokes, deaths, or slower plaque growth.
Reviews propose antioxidant, anti-inflammatory, metabolic and endothelial effects. Those mechanisms are useful for explaining why Spirulina is being studied, yet they do not establish that a particular powder, tablet or capsule produces a clinically meaningful cardiovascular benefit.

What “atheroprotective” means in the case of Spirulina
Atherosclerosis is the long-term buildup and remodeling of plaque in artery walls. Plaque formation involves apoB-containing lipoproteins such as LDL, immune and inflammatory activity, oxidative stress, endothelial dysfunction and, later, processes that can make a plaque unstable. “Atheroprotective” therefore means something that might reduce one or more of those processes or lower the resulting cardiovascular risk.

A mechanism can be plausible in cells or animals and still fail to produce fewer cardiovascular events in people. The relevant human evidence in the available literature measures blood lipids and, in some studies, inflammatory or metabolic markers. It does not demonstrate regression of plaque or prevention of myocardial infarction, stroke or cardiovascular death.

Spirulina is a cyanobacterial biomass sold as powder, tablets and capsules. Products can differ in species identity, cultivation conditions, processing and contaminant control. That variability matters when trying to connect a laboratory mechanism with a consumer product.
What human trials show about cholesterol and triglycerides
The strongest quantitative evidence is a 2023 systematic review and dose-response meta-analysis by Rahnama and colleagues. It pooled 20 studies, 23 trial arms and 1,076 participants, with database searches completed through January 2023. The analysis evaluated serum lipid outcomes rather than plaque or cardiovascular events.
| Outcome | Pooled result | How to interpret it |
|---|---|---|
| LDL cholesterol (LDL-C) | Standardized mean difference (SMD) −0.6; 95% confidence interval −0.9 to −0.2 | A statistically significant difference across studies, expressed in standardized units rather than a fixed mg/dL reduction. |
| Total cholesterol | SMD −0.6; 95% confidence interval −0.9 to −0.2 | Consistent direction of change in the pooled analysis, but not a direct estimate of plaque or event reduction. |
| Triglycerides | SMD −0.6; 95% confidence interval −0.9 to −0.2 | Lower values were reported in the pooled trials; the size cannot be converted to a universal dose or mg/dL effect. |
| HDL cholesterol (HDL-C) | SMD 0.3; 95% confidence interval 0.0 to 0.6 | A modest standardized increase in the meta-analysis, with the confidence interval beginning at zero. |
The authors concluded that Spirulina supplementation was useful for improving serum total cholesterol, triglycerides, LDL-C and HDL-C. In context, “improving” refers to those measured laboratory outcomes. The review did not establish that the changes prevent cardiovascular events, eliminate plaque or remain after supplementation stops.
Why standardized mean differences need caution
An SMD expresses the difference between intervention and comparison groups in standard-deviation units. It is useful when trials use different lipid measurement scales or have different variability, but it is not the same as saying that every person will lower LDL-C by a particular number of milligrams per deciliter. The confidence interval describes statistical uncertainty around the pooled estimate; it does not answer whether an individual will respond or whether the change is large enough to alter clinical outcomes.

What the meta-analysis cannot answer
- Whether Spirulina lowers apoB or the number of atherogenic particles in a consistent way.
- Whether any lipid change persists over years rather than the shorter durations used in individual trials.
- Whether one species, extract, powder or tablet is responsible for the pooled signal.
- Whether the changes translate into fewer heart attacks, strokes, cardiovascular deaths or slower plaque progression.
How the proposed pathway could work
Mechanistic reviews published in 2024 and 2026 describe several connected hypotheses. They are best read as a map of biological possibilities, not as proof that a supplement has a therapeutic effect in patients.
Antioxidant response and oxidative stress
Oxidative stress can modify lipoproteins and impair vascular cells, creating conditions that favor inflammatory recruitment. Spirulina contains phycocyanin-related compounds and other constituents discussed for antioxidant activity. Reviews associate these constituents with activation of antioxidant-response systems such as nuclear factor erythroid 2–related factor 2 (Nrf2).
Nrf2 regulates genes involved in cellular defenses against reactive oxygen species. If a preparation meaningfully increased this response in human vascular tissue, it could theoretically reduce oxidative damage. The available evidence does not show that eating Spirulina produces a clinically important Nrf2-driven reduction in atherosclerosis, and it does not identify a single active constituent or dose that would do so.

Inflammatory signaling and NF-κB
Atherosclerosis is also an inflammatory disease. Endothelial cells, macrophages and other immune cells communicate through signaling networks that include nuclear factor kappa B (NF-κB). The 2024 and 2026 reviews describe studies in which Spirulina or its constituents appear to attenuate NF-κB-related signaling or alter inflammatory markers.

Those findings may explain why researchers are interested in Spirulina, but an inflammatory-marker change is not equivalent to a reduction in plaque rupture. Trials vary in participants, preparations, doses, duration and laboratory methods, so a result in one study cannot automatically be generalized to all products.
Metabolic signaling: AMPK and AKT
Reviews also discuss pathways such as AMP-activated protein kinase (AMPK) and protein kinase B (AKT). These networks influence energy metabolism, glucose handling, lipid processing and cell survival. A theoretical benefit would be improved metabolic conditions that indirectly reduce vascular stress.
At present, the evidence does not establish which of these pathways is necessary for the lipid findings in humans. It also does not show that changing a signaling marker with Spirulina produces the same cardiovascular protection as established therapies that have been tested in outcome trials.

Endothelial function and nitric oxide
The endothelium regulates vessel tone, permeability and interactions with circulating blood cells. Oxidative and inflammatory activation can reduce nitric-oxide signaling and make the vessel wall more adhesive to immune cells. The 2026 review discusses possible endothelial protection from lower oxidative and inflammatory activation, with a potential benefit to nitric-oxide signaling.

This is a proposed mechanism from a narrative review. It is not evidence that Spirulina restores endothelial function in people with atherosclerotic disease, nor does it show fewer clinical events.
How strong is the evidence?
| Evidence source | What it contributes | Main limitation |
|---|---|---|
| 2023 systematic review and meta-analysis | Pooled human trial results for LDL-C, total cholesterol, triglycerides and HDL-C. | Surrogate lipid endpoints; no verified reduction in plaque or cardiovascular events. |
| 2024 narrative review | Summary of cardiometabolic findings and the wide range of doses and study durations. | Heterogeneous interventions and a call for more randomized trials. |
| 2026 signaling-pathway review | Discussion of Nrf2, NF-κB, AMPK/AKT and possible endothelial effects. | Mechanistic and clinical evidence remains heterogeneous; product standardization is limited. |
| Preclinical and laboratory research discussed in reviews | Biological hypotheses about antioxidant and inflammatory activity. | Results in cells or animals do not establish benefit from commercial supplements in humans. |
The key gap is the link between a marker and an outcome. Lower LDL-C can be relevant to atherosclerotic risk, but the Spirulina studies do not show that their observed changes have the magnitude, durability or causal significance needed to claim event prevention. No verified statistic in the cited evidence shows fewer heart attacks, strokes, cardiovascular deaths or cases of plaque progression.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why dose and formulation make comparisons difficult
The 2024 review describes substantial variation in intervention doses and durations. Participants may have received different quantities, schedules and forms of Spirulina, and studies may have used products with different composition and manufacturing controls. A dose that appeared in a trial is therefore a description of that experiment, not a personal recommendation.

Safety, contamination and supplement oversight
The U.S. Food and Drug Administration (FDA) warns that some blue-green algae products can be contaminated with microcystins, toxins produced by certain cyanobacteria. Its consumer information discusses risks associated with products such as Aphanizomenon flos-aquae (AFA), including harvest conditions and past recalls. This is a product-quality warning—not a claim that all Spirulina is contaminated.
Because contamination is batch- and product-dependent, inspect the label and follow the directions rather than assuming that a product is safe because it is marketed as “natural.” People who are pregnant, breastfeeding, taking medicines, managing liver disease or dealing with another medical condition should ask a qualified health professional before using an algae supplement. Seek medical advice promptly if a suspected supplement causes illness.
FDA also states that dietary supplements are not approved for safety and effectiveness before they are marketed in the way medicines are. “FDA approved” should not appear on a Spirulina product as proof of cardiovascular benefit.

A practical quality checklist
- Confirm the ingredient identity: Look for the stated species or ingredient rather than a vague “blue-green algae” label.
- Check serving information: Record the amount per serving and the suggested use; do not infer a therapeutic dose from a research paper.
- Look for manufacturing transparency: Prefer companies that explain testing, lot identification and quality controls.
- Ask about a certificate of analysis: Batch-specific documentation should address identity and relevant contaminants, including microcystins where applicable.
- Check recalls and warnings: Review current FDA notices and the manufacturer’s lot information before use.
- Keep the product separate from medical treatment decisions: Do not stop a prescribed statin, blood-pressure medicine, diabetes treatment or antiplatelet drug because of a supplement.
Does Spirulina replace proven cardiovascular prevention?
No. The evidence summarized here does not support using Spirulina to treat diagnosed atherosclerosis, replace lipid-lowering medication, or substitute for blood-pressure control, smoking cessation, physical activity, a heart-healthy eating pattern or other clinician-guided care. A person with high LDL-C, diabetes, hypertension, kidney disease or prior cardiovascular disease should use the prevention plan recommended by a health professional.
If someone chooses to try Spirulina as a food or supplement, the sensible approach is to tell the treating clinician, use a product with clear identity and contaminant information, and monitor established risk factors through routine care. Any decision should account for possible interactions, allergies, underlying disease and the fact that trial doses and products were not uniform.
What researchers still need to establish
- Large, preregistered randomized trials using well-characterized, contaminant-tested Spirulina preparations.
- Standardized reporting of species, active constituents, dose, duration and adherence.
- Direct measurements of apoB, non-HDL cholesterol, endothelial function and inflammatory markers alongside conventional lipids.
- Long-term follow-up for plaque imaging and clinical outcomes such as heart attack, stroke and cardiovascular death.
- Analysis of whether any benefit differs by baseline lipid level, diabetes status, medication use or other risk factors.
- Evidence showing whether effects persist after supplementation ends and whether they are clinically large enough to matter.
The evidence-based takeaway
Spirulina has a credible biological hypothesis: its constituents may influence antioxidant defenses, inflammatory signaling, metabolism and endothelial function, while human trials have reported favorable shifts in several serum lipids. The best pooled evidence, however, concerns laboratory risk markers in 1,076 participants—not a reduction in atherosclerotic plaque or cardiovascular events. Treat the “atheroprotective pathway” as an area of research, not as a proven claim for a supplement.
