However, the strongest pooled evidence does not show that EVOO consistently lowers long-term blood glucose, HbA1c, insulin, or HOMA-IR more than other olive oils.
The practical distinction is between a short post-meal glucose curve and diabetes control measured over weeks or months. The studies also tested different oils, doses, meals and participants, so a result from one type of EVOO cannot automatically be applied to every bottle or to every person with diabetes.

What “better for blood sugar” can mean
When people ask whether EVOO is better than refined or ordinary olive oil, they may be asking about two different outcomes:
The Tool Desk
Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →- Post-meal glucose: the rise during the first few hours after eating a specific meal.
- Longer-term control: fasting glucose, insulin resistance, HbA1c or related measures collected after several weeks or months.
An oil can change digestion and the shape of one meal’s glucose curve without producing a measurable change in HbA1c. HbA1c reflects average glucose over roughly the preceding two to three months, while a single post-meal experiment captures only one meal under controlled conditions. Keeping those outcomes separate prevents the strongest claim the evidence can support from being overstated.

The clearest short-term signal came from one small type 1 diabetes study
What the trial did
Bozzetto and colleagues reported a randomized crossover study in Diabetes Care in 2016 involving 13 adults with type 1 diabetes who used insulin pumps. Participants ate meals with different glycemic indices and different fat sources, including EVOO, butter and a low-fat version. Because the same participants experienced the different conditions, the design could compare meal responses within individuals, but it was still a very small experiment.
What happened after the high-glycemic meal
For the high-glycemic-index meal, the 0–3-hour glucose incremental area under the curve was:
| Meal fat condition | Glucose incremental area under the curve, 0–3 hours |
|---|---|
| Extra-virgin olive oil | 198 ± 274 mmol/L × 180 min |
| Low fat | 416 ± 329 mmol/L × 180 min |
| Butter | 398 ± 355 mmol/L × 180 min |
The differences were statistically significant for that high-glycemic meal. In plain language, the glucose excursion was lower during the early post-meal window when EVOO accompanied the meal.

Why the result is not a general diabetes claim
The fat type did not significantly change glucose response when the meals had a low glycemic index. The study therefore points to an interaction among the oil, the carbohydrate quality and the meal—not to a universal glucose-lowering property of EVOO. It also involved only 13 people with type 1 diabetes and measured an acute response, not HbA1c or a change in insulin requirements over months. The investigators’ findings cannot be used to calculate an individual insulin dose or to justify changing prescribed meal or medication plans.
A 2025 trial found lower insulin resistance with a high-phenolic EVOO
The OILDIABET protocol
The OILDIABET randomized trial, published in Food & Function in 2025, enrolled 116 adults with type 2 diabetes. For 24 weeks, the intervention group consumed 30 mL per day of a phenolic-rich EVOO made from native Galician olives. The control group received advice to minimize EVOO and to prefer refined olive-oil blends. This was a comparison between a defined high-phenolic intervention oil and a control eating pattern, not a test of every extra-virgin and refined oil sold in stores.
The primary reported result
After 24 weeks, the trial authors wrote: “After 24 weeks, the Interventional group demonstrated significant reductions in insulin resistance (HOMA IR).” HOMA-IR is a calculated marker based mainly on fasting glucose and fasting insulin; it is not the same thing as a direct measure of average daily glucose or a guarantee that a person will need less medication.

- Graza Sizzle Extra Virgin Olive Oil: A cooking oil made from mature, mid-season olives that yield a more mellow flavor. Sizzle was born for the heat. 1 bottle / 750mL.
- Use It Every Day, In Every Way: Because of its mellow flavor and high smoke point, Sizzle can be used in a lot of different ways. Sizzle is great for everyday cooking like roasting, searing, poaching, pan frying, baking, and marinating.
- Fun Ways to Use Sizzle: If you would put it in or on top of an oven, use Sizzle. Chicken cutlets, sheet pan veggies, fried rice, or even chocolate chip cookies! You name it, Sizzle Sizzles it.
- Mellow Flavor: Just like (most) humans, olives get more chill with age. The oil produced from these olives is milder in flavor and makes for a more flexible cooking oil.
- Single Origin, Single Varietal: We use single origin olives that are harvested and then cold-pressed ASAP.
The authors also reported favorable findings for fasting glucose and HbA1c in subgroups. Those subgroup analyses did not have enough statistical power to establish a dependable benefit for everyone with type 2 diabetes. They should be treated as signals for further study rather than settled clinical effects.
What the trial does—and does not—show
The study provides a plausible, longer-term signal for one high-phenolic Galician EVOO at 30 mL per day under a specific protocol. It does not establish 30 mL as a recommended dose, prove that phenolic content is the reason for the result, or show that a particular retail brand will reproduce it. The intervention amount is a research protocol, not a substitute for diabetes treatment.
What larger evidence summaries find
The 2026 meta-analysis
A 2026 systematic review and dose-response meta-analysis in Nutrition Reviews pooled 51 randomized controlled trials involving 4,334 participants. Across the included olive-oil interventions, the analysis found no statistically significant overall changes in blood glucose, insulin, HbA1c or HOMA-IR.

- Pressed and bottled in Italy
- Use for cooking, grilling, and in finishing dishes
- One 16.9-ounce plastic bottle of Extra Virgin Olive Oil
This is the broadest and newest synthesis described here, and it weighs more heavily than a single small trial when asking whether olive oil reliably improves long-term glycemic markers. A nonsignificant pooled result does not prove that every oil, dose or subgroup has identical effects. It means that, taken together, the available randomized evidence has not demonstrated a consistent average improvement.
The 2021 EVOO-only review
Dehghani and colleagues’ 2021 systematic review and meta-analysis included 13 trials and 633 participants specifically examining EVOO. It found no significant pooled effect on fasting blood sugar, insulin or HOMA-IR. The result reinforces the uncertainty around a durable glucose-lowering effect, even when the analysis is restricted to extra-virgin oil.

Why EVOO and refined olive oil might behave differently
Phenolic compounds are one proposed explanation
EVOO is produced with less processing than refined olive oil and can retain phenolic compounds. The OILDIABET trial deliberately selected a phenolic-rich oil, so its result cannot be generalized to EVOO with a different phenolic profile. Phenolic content varies with olive variety, growing conditions, harvest timing, processing and storage. “Extra virgin” identifies a regulated quality category; it does not guarantee the same phenolic concentration as the Galician oil used in that trial.

Meals can outweigh the oil distinction
The type and amount of carbohydrate in the meal strongly affect the glucose curve. In the type 1 diabetes crossover study, the apparent EVOO advantage appeared with a high-glycemic meal but not with low-glycemic meals. That pattern is a reminder that replacing one oil while leaving the rest of the meal unchanged may not produce the same response observed in a study.
Study design changes the level of confidence
A 13-person crossover experiment, an 11-person sequential study and a 116-person 24-week randomized trial answer different questions. Crossover designs can detect a short-term difference but cannot establish a chronic effect. Sequential studies are vulnerable to order effects and changes over time. A larger randomized trial is stronger, but subgroup findings can still be unreliable when there are too few participants for the statistical comparisons.
What to do if you want to use EVOO in a diabetes-friendly eating pattern
Current evidence does not require people with diabetes to replace refined olive oil with EVOO for the purpose of lowering HbA1c. If you prefer EVOO for taste or cooking, it can be used as a food ingredient, but the decision should fit your overall eating pattern, energy needs and clinician’s advice.

- Think about the whole meal, especially the carbohydrate portion and its glycemic impact. The only clear acute signal in the cited evidence was meal-specific.
- Do not assume a premium price, “high polyphenol” wording or a particular bottle has been proven to lower glucose. No cited study establishes a retail brand benefit.
- If you monitor glucose, observe your own response to a consistent meal rather than drawing conclusions from a single reading. Record changes over several occasions and discuss unexpected patterns with your diabetes team.
- Do not change insulin timing, medication doses or prescribed carbohydrate targets because of these studies. The type 1 diabetes experiment was not an individual dosing guide, and the 2025 trial did not replace standard diabetes care.
Common conclusions the evidence does not support
- “EVOO controls diabetes.” The pooled evidence does not establish that claim.
- “EVOO always gives a lower post-meal rise.” The result depended on a high-glycemic meal in a very small type 1 diabetes study.
- “High-phenolic EVOO lowers HbA1c for everyone.” The favorable HbA1c findings in OILDIABET were subgroup results that lacked sufficient statistical power.
- “Refined olive oil is harmful for blood sugar.” The available comparisons do not establish that conclusion; the 2026 pooled analysis found no significant overall advantage for olive-oil interventions on major glycemic markers.
- “Thirty milliliters is the correct daily dose.” That amount belonged to one 24-week research protocol and is not an established prescription.
Bottom line
EVOO may modestly change the early glucose response to some meals, and a 2025 randomized trial makes high-phenolic EVOO worth studying for insulin resistance. But the 2021 EVOO review and the larger 2026 meta-analysis do not show a reliable average improvement in fasting glucose, HbA1c, insulin or HOMA-IR. For now, choose EVOO or another olive oil according to taste, cooking needs and your established diabetes plan—not because one has been proven to control blood sugar better in every person.
