Does Exercise Promote Neurogenesis? What Human Studies Can—and Can’t—Tell Us

Short answer: Voluntary running reliably increases adult hippocampal neurogenesis in animal models. In people, exercise studies have found changes in hippocampal volume, blood flow, fitness and memory, but those measurements do not directly count newborn neurons. Adult human hippocampal neurogenesis is itself still debated, so no human exercise type or dose has been proven to create new hippocampal neurons.

What neurogenesis means in this question

Neurogenesis is the production of new neurons from neural stem and progenitor cells. The hippocampus, a brain structure involved in learning and memory, is the region most often discussed in adult-neurogenesis studies. A complete neurogenesis process is more than a stem cell dividing once. It can involve activation of a previously quiet stem cell, proliferation of progenitor cells, differentiation into a neuronal lineage, survival, maturation and integration into existing circuits.

That sequence matters because a larger pool of precursor cells, a temporary rise in immature cells or a change in a brain scan is not equivalent to a durable, functioning neuron added to a human circuit. A claim that exercise “promotes neurogenesis” therefore needs to specify which stage was measured and in which species.

Why running is considered a strong stimulus in animal studies

A 2023 review of voluntary-running experiments describes running as one of the strongest known stimuli for adult hippocampal neurogenesis in experimental animals. Across those models, running has been associated with several stages of the process:

  • Activation of quiescent neural stem cells.
  • Proliferation of stem and progenitor cells.
  • Greater differentiation toward neuronal cells.
  • Improved survival of newborn cells.
  • Maturation and integration into hippocampal circuitry.

These findings provide biological plausibility for asking whether exercise could influence human neurogenesis. They do not establish that the same chain occurs in people. The review also notes that the detailed mechanisms behind the animal effects remain unclear. A running wheel in a controlled animal experiment is not a direct substitute for a human exercise program, and the cellular measurements available in animals are generally not available in living participants.

Is adult human hippocampal neurogenesis proven?

No. Evidence from multiple studies supports the possibility that neural stem cells persist and generate some new neurons in the adult and aging human hippocampus. However, a 2023 review concludes that robust neurogenesis in the adult and aged human hippocampus remains unconfirmed.

The unresolved questions are specific:

  • How many new neurons are produced at different adult ages?
  • How long do those cells survive?
  • Do they integrate into working hippocampal circuits?
  • Do they make a measurable contribution to memory or other behavior?
  • Would deliberately increasing neurogenesis improve a human outcome?

Human tissue studies are difficult to standardize. Differences in tissue preservation, markers, sampling, age, health and analysis can change the apparent result. Consequently, a study that reports evidence consistent with adult neurogenesis does not settle how much neurogenesis occurs or whether exercise increases it.

What human exercise studies can actually measure

Most exercise trials cannot count newly generated neurons in a living person. They instead measure outcomes that may be related to brain health:

  • Perfusion or blood flow: measured with specialized imaging methods.
  • Fitness: assessed with exercise-performance or cardiorespiratory measures.
  • Cognition: assessed with memory or other behavioral tests.
  • Biomarkers: blood or other indirect measures that may reflect physiological pathways.

These outcomes can answer whether an intervention is associated with a measurable human change. They cannot, by themselves, identify newly born neurons, show that those neurons survived and integrated, or prove that neurogenesis caused an improvement in memory.

What the hippocampal-volume meta-analysis found—and what it did not

A meta-analysis of controlled human exercise interventions combined 22 studies comprising 23 trials and 1,204 participants. The underlying studies were published from 2010 through 2020. It reported a significant overall positive effect of exercise on hippocampal volume, but the result describes an imaging outcome, not a direct neurogenesis count.

Analysis or subgroup Reported result How to interpret it
All included trials Significant overall positive exercise effect on hippocampal volume Exercise can affect this MRI measure in the included populations; the cellular cause is not identified.
Older participants Significant effect The association was detectable in older samples, not proof of new-neuron formation.
Interventions longer than 24 weeks Significant effect Longer programs showed an effect in this analysis; duration is not a proven neurogenesis threshold.
Younger participants Not significant The analysis did not find a statistically significant volume effect in these samples.
Healthy participants considered alone Not significant Results varied by participant health status.
Less than 150 minutes of exercise per week Significant subgroup effect This exploratory pattern is not a recommended dose and may reflect compliance or intensity differences.
150 minutes or more per week Not significant subgroup effect The contrast does not show that more exercise fails, nor does it define a neurogenesis dose.

The authors suggested that compliance or intensity might help explain the unexpected weekly-minutes pattern, but the analysis could not distinguish exercise intensity as a moderator. It would be a mistake to turn these subgroup findings into a prescription such as “exercise for less than 150 minutes to grow neurons.” They are observations about hippocampal-volume results across heterogeneous trials, not a controlled test of human neurogenesis.

Why a larger hippocampus is not proof of new neurons

Hippocampal volume is a composite anatomical measure. Several biological changes could alter it without adding new neurons. The meta-analysis discusses possible contributions from:

  • Neurogenesis.
  • Angiogenesis, or growth and remodeling of blood vessels.
  • Changes in dendrites and synaptic structure.
  • Neurotrophic factors that support neuronal maintenance and plasticity.
  • Reduced vascular, stress-related or inflammatory damage.

These mechanisms are not mutually exclusive. Exercise could influence blood supply, tissue water, synaptic architecture or vascular health while an MRI scanner reports a volume difference. A volume increase can therefore be compatible with neurogenesis without being specific for it. The same logic applies to better memory: improved performance may result from vascular, metabolic, attentional or learning effects rather than newly generated neurons.

Can MRI show neurogenesis?

MRI can show structural or physiological differences in a living brain, but routine human MRI does not directly count newborn neurons. An imaging review on adult human hippocampal neurogenesis describes a promising but indirect approach: some imaging signals have been related to neurogenesis in animals, and researchers have considered whether analogous signals in people might be informative.

Cross-species correspondence is not the same as a human cell count. An imaging signal can be influenced by multiple tissues and processes, and the relationship established in an animal experiment may not hold quantitatively in humans. Unless a specific study directly measures a cellular endpoint, the accurate wording is that imaging is consistent with or associated with a possible mechanism—not that MRI “shows new neurons.”

Question Why it matters
Who was studied? Age, health status and medication can alter hippocampal measures and the response to exercise.
What exercise was used? Modality, frequency, intensity and duration determine what intervention was actually tested; “exercise” is not one uniform exposure.
Was the design randomized or observational? Randomized interventions provide stronger evidence about effects than associations observed in people who choose to exercise.
What was measured? A direct cellular measure, if available, answers a different question from MRI volume, perfusion, a blood marker or a memory score.
Could other mechanisms explain the outcome? Volume, blood flow and cognition can change through vascular, synaptic, hormonal, inflammatory or stress-related pathways.
Were newborn cells shown to survive and integrate? Generation alone would not establish a lasting functional contribution.

This framework prevents a common chain of overstatement: “exercise improved memory, memory depends on the hippocampus, therefore exercise created new hippocampal neurons.” Each step contains an assumption that the measured study may not test.

Does exercise create new brain cells in people?

Human exercise studies can establish that a program is associated with changes in measured outcomes such as hippocampal volume, perfusion, fitness or cognition. They can also show whether those changes differ between age groups, health groups or intervention durations. Such findings are useful for generating mechanistic hypotheses.

They cannot establish, from those outcomes alone, that new neurons were generated, that the cells survived and connected to existing circuits, or that neurogenesis caused the observed structural or cognitive change. The uncertainty is not a technical footnote; it is the central limitation of answering this question in living humans.

Can running grow new neurons?

Running is the clearest positive example in animal work, especially voluntary-running models. That evidence makes running a plausible candidate for influencing related biology in humans, but it does not prove that running grows new human hippocampal neurons. No exercise type, running speed, weekly duration or training schedule has been established as a human neurogenesis dose by the evidence summarized here.

The 150-minute subgroup result from the volume meta-analysis should not be used to choose a target. The analysis did not show that a lower amount is biologically superior, and it could not separate intensity from adherence or other trial differences.

What evidence would provide a stronger answer?

A more decisive human case would require converging evidence rather than a single indirect endpoint. Studies would need reliable cellular or tissue measures, standardized methods, clearly characterized participants and exercise exposure, and evidence that any newly generated cells persist, mature and integrate. They would also need to separate neurogenesis from vascular, synaptic, hormonal, inflammatory and stress-related effects.

Candidate endocrine pathways are discussed in a 2023 review of peripheral hormone responses to exercise, but proposed pathways are not the same as demonstrated human neurogenesis. Likewise, a statistically significant MRI result is valuable evidence about an anatomical measure, not a substitute for cellular confirmation.

How to phrase the evidence accurately

  • Supported: Voluntary running strongly promotes adult hippocampal neurogenesis in animal models.
  • Reasonable but unproven: Exercise may influence biological pathways related to neurogenesis in humans.
  • Supported with qualification: Some controlled human interventions are associated with changes in hippocampal volume, particularly in certain older or longer-duration subgroups.
  • Not established: That exercise-induced volume, blood-flow or memory changes were caused by newly generated human neurons.
  • Not established: A specific exercise mode or dose that reliably increases human hippocampal neurogenesis.

The careful conclusion is therefore narrower than many headlines: exercise is a biologically plausible influence on neurogenesis, with strong support from animal experiments, but current human exercise studies do not establish that exercise promotes new-neuron formation in the human hippocampus.
