Usually, no. You can receive Baldur’s Gate 3 updates on your current save; starting over is not generally required just to access patched content. But an update does not rewind your campaign or guarantee access to a scene whose trigger you have already passed or whose story conditions your choices did not meet. If you have missed a particular scene, an earlier save—or, if none can reach it, a new playthrough—may be necessary.
Patch access and scene eligibility are different
Updates can add scenes, dialogue reactivity, and fixes, but they do not automatically replay scenes in a campaign already underway. Whether a particular Karlach or Astarion moment can occur depends on where your save is in the story and on relevant choices. Larian’s patch notes describe specific additions and fixes; they do not promise that every missed scene will appear retroactively in every existing save.

If your save is still before the relevant scene, update the game and continue that campaign. If you are already past its trigger, load an earlier save if you have one. A fresh campaign is the fallback only when your current campaign cannot reach the scene and no suitable earlier save is available.
What the updates added or changed
Karlach’s optional ending
In August 2023, Larian said it was expanding the epilogues, with Patch 2 adding an optional ending with Karlach. The studio described the change as “the addition of a new optional ending with Karlach.” Larian’s Community Update #24 explains that context.

Patch 5: epilogue and scene changes
Patch 5 added a new section after the Netherbrain’s defeat, intended to give the ending more closure with companions. It also restored cinematic portions of Astarion’s Act III Ascendant scene and fixed a black-screen issue during his endgame romance scene. The notes say Karlach’s camp reflection about leaving Act II can also trigger during the transition into Act III if needed. Read Larian’s Patch 5 notes.
Patch 6: epilogue reactivity and an Astarion kiss scene
Patch 6 added Karlach and Astarion reactions in God Gale’s epilogue dialogue, where they can request a cure for their conditions. It also added a version of the kiss scene with Astarion after he ascends, along with relationship-related epilogue reactivity and fixes to some epilogue behavior after loading a save made there. See Larian’s Patch 6 notes.

Patch 7: targeted fixes
Patch 7 addressed particular issues, including a Karlach Avernus scene-flow problem and an Astarion romance edge case at the camp celebration, alongside other dialogue and save fixes. These are examples of targeted repairs, not a general guarantee that a missed scene will be restored in every campaign. Larian’s Patch 7 notes list the changes.

What to do if a scene is not appearing
- Update the game, then continue your current save. You do not need to restart merely to download and receive updates.
- Check your campaign position. If you have not passed the point where the scene could occur, keep playing and allow the relevant story events to unfold.
- Load an earlier save if you have passed that point. Returning to a save from before the trigger is the most direct way to revisit it.
- Start a new playthrough only if needed. If no available save can reach the scene, a new campaign may be required for that particular content.
The official notes identify individual scene changes and fixes, but do not provide a complete checklist of romance flags or trigger conditions for every Karlach and Astarion scene. Avoid assuming a scene is owed on every save: campaign progress and choices still matter.
