No available evidence shows that wearing a bra—including an underwire bra—causes breast cancer. But the main study cited in support of that conclusion did not measure tightness as a separate exposure. Its findings are reassuring about the bra habits it examined, not a direct test of every degree of tightness.
What the study found—and what it measured
A 2014 population-based case-control study compared postmenopausal women aged 55–74 in the Seattle–Puget Sound area: 454 women diagnosed with invasive ductal carcinoma and 590 with invasive lobular carcinoma between 2000 and 2004, alongside 469 controls. The researchers found no association between breast-cancer risk and the bra practices they assessed: cup size, how recently a woman wore a bra, average hours worn per day, underwire use, or age when regular bra use began. The study is indexed by PubMed.

The study did not list tightness as a distinct measured exposure. Its case-control design, age range, postmenopausal participants, and single geographic setting also limit how broadly its results can be applied. So the accurate takeaway is not that every possible bra fit has been tested; it is that this study found no link across the bra habits it measured in the women studied.

Does an underwire or going braless change risk?
The American Cancer Society says, “There is no scientific or clinical data that shows that wearing a bra changes the risk of having breast cancer.” Its summary includes the 2014 study, which found no association for underwire use or hours worn. The available evidence therefore does not support switching to a different bra style, avoiding underwire, or going braless as a way to lower breast-cancer risk.

What about the claim that bras block lymph flow?
The idea that a bra obstructs lymph flow and causes toxins to build up is not supported by scientific evidence, according to the American Cancer Society’s overview of unclear, disproven, and controversial breast-cancer risk factors. The cited evidence does not establish bra fit as a cancer-prevention issue.
Can a tight or poorly fitting bra cause symptoms?
A bra that fits poorly can cause discomfort and swelling, according to the National Breast Cancer Foundation. Those are fit and comfort concerns, not evidence that the bra has caused cancer. If a bra feels painful or uncomfortably restrictive, choosing a more comfortable fit is reasonable for comfort; it should not be treated as a cancer-risk intervention.
