Do Semaglutide and Tirzepatide Have Different Nail Side Effects?

Based on the current U.S. prescribing information reviewed for 2026, neither semaglutide (Wegovy) nor tirzepatide (Zepbound) lists nail disorder as an adverse reaction. The labels therefore do not show that one medicine causes more nail problems than the other. They also do not prove that nail changes are impossible, because label silence cannot exclude rare, unrecognized, or separately reported events.

No head-to-head study or nail-specific comparative rate was identified in the reviewed sources. Both labels discuss hair loss associated with weight reduction, but hair loss is a different symptom and those figures cannot answer whether either medicine affects nails.

What the current labels say about nails

The narrow regulatory answer is the same for both medicines: nail disorder does not appear as a listed adverse reaction in the U.S. prescribing information reviewed for 2026. You can read the DailyMed prescribing information for WEGOVY (semaglutide) and the DailyMed prescribing information for ZEPBOUND (tirzepatide).

Semaglutide (Wegovy)

The reviewed Wegovy label does not identify nail disorder as an adverse reaction in its clinical-trial safety information. It does identify hair loss in adults receiving the 2.4-mg dose in pooled weight-reduction studies, and the label associates those hair-loss events with weight reduction. That statement concerns hair, not nails.

Tirzepatide (Zepbound)

The reviewed Zepbound label likewise does not list nail disorder. It reports hair-loss events in pooled studies and states that “Hair loss adverse reactions in ZEPBOUND-treated patients were associated with weight reduction.” This is a labeled observation about hair and does not establish a nail effect or a nail-specific mechanism.

Side-by-side comparison

Question Semaglutide (Wegovy) Tirzepatide (Zepbound)
Is nail disorder listed in the reviewed U.S. label? No No
Is there a nail-specific incidence rate? Not stated in the reviewed label Not stated in the reviewed label
Is there a direct head-to-head nail comparison? No comparison was identified in the reviewed sources
What related finding is labeled? Hair loss associated with weight reduction Hair loss associated with weight reduction

This table supports a limited conclusion: the labels are silent on nail disorder for both drugs. It does not support ranking one drug as safer or riskier for nails.

Why the hair-loss percentages cannot answer a nail question

Hair and nails are different tissues and different clinical outcomes. A trial count for hair loss cannot be converted into a probability of brittle nails, ridges, discoloration, separation, or another nail change. Treating the hair figures as a proxy for nails would overstate what the labels show.

Semaglutide hair-loss figure

In pooled adult weight-reduction studies, hair loss occurred in 3.3% of people receiving Wegovy 2.4 mg and 1.0% receiving placebo. The label describes these reactions as associated with weight reduction. These are hair-loss percentages, not nail-event percentages.

Tirzepatide hair-loss figures

In pooled Zepbound Studies 1 and 2, hair loss was reported by 7.1% of treated women and 0.5% of treated men, compared with 1.3% of women and 0% of men receiving placebo. The label again associates the reactions with weight reduction. These sex-specific figures describe hair loss only; they do not estimate nail risk.

Why cross-trial arithmetic is misleading

The semaglutide and tirzepatide numbers come from different clinical-trial programs, with potentially different populations, follow-up periods, definitions, background treatments, and reporting practices. The Wegovy labeling cautions that adverse-reaction rates from separate trials should not be directly compared. Consequently, even the hair-loss percentages should not be used to claim that tirzepatide has more or less hair risk than semaglutide, much less to infer a nail difference.

What “not listed” means—and what it does not mean

Prescribing information summarizes adverse reactions detected and considered relevant in the studies and safety monitoring used for that product. When a nail disorder is not listed, the label has not established it as a recognized adverse reaction at a frequency or level that appears in that section. That is useful evidence, but it is not proof that no patient can ever experience a nail change while taking the medicine.

Label silence cannot rule out an uncommon event, an event that was not recognized in the available data, or a change caused by something other than the medicine. The reviewed sources do not establish a nail-specific causal pathway for either drug, do not provide an incidence estimate for nail changes, and do not determine whether a particular person’s nails changed because of treatment.

Timing alone cannot prove causation

If a nail change begins after starting or increasing a medicine, the timing is worth reporting but does not by itself demonstrate that the drug caused it. The same period may include rapid weight change, altered food intake, an underlying condition, another medicine, or an unrelated nail problem. The available evidence does not separate those possibilities for an individual patient.

Weight reduction is a separate clinical context

Both labels connect their hair-loss observations with weight reduction. That association should not be silently expanded to nails. A person losing weight may notice several changes at once, but the reviewed labeling does not show that either drug directly causes nail disorder or that one produces more nail changes than the other.

What to do if your nails change during treatment

A nail change deserves a clinical conversation even though neither label lists nail disorder. The American Academy of Dermatology advises people with GLP-1-related skin, hair, or nail concerns to communicate with their medical team and partner with a board-certified dermatologist. Its guidance is available in “How can GLP-1 drugs affect my skin, hair, and nails?”

  1. Record what changed. Note which nails are involved, what you see (for example, a change in texture, color, shape, or attachment), when it started, and whether it is spreading or improving. Clear photographs taken under similar lighting can help a clinician compare changes over time.
  2. List treatment timing. Write down when you began semaglutide or tirzepatide, any dose changes, and when weight change or other symptoms occurred. Include prescription medicines, over-the-counter products, and supplements so the clinician can consider the full context.
  3. Contact the prescriber. Tell the clinician who manages the medicine rather than assuming the nail change is an expected side effect. Ask whether an examination, laboratory work, or review of other causes is appropriate for your situation.
  4. Consider dermatology evaluation. A board-certified dermatologist can examine the nails and look for causes that cannot be distinguished from a description alone. This is especially useful when changes persist, involve several nails, or are accompanied by other skin or hair concerns.
  5. Do not change the prescription on your own. Do not stop, reduce, or switch semaglutide or tirzepatide solely because a nail change appeared. Make medication decisions with the prescriber, who can weigh the nail concern against the reason the medicine was prescribed and your overall health.

Information to bring to the appointment

  • A timeline of medication starts, dose changes, weight changes, and the first nail symptoms.
  • Photographs showing the nails before and after the change, if available.
  • A complete list of medicines, supplements, and relevant medical conditions.
  • Details about pain, tenderness, swelling, drainage, injury, or changes in more than one nail.
  • Any simultaneous hair or skin changes, while keeping those symptoms distinct from the nail complaint.

How clinicians can interpret the complaint

An evaluation is needed because “nail side effect” can describe many different observations. A clinician may first verify that the change is truly in the nail and then consider its distribution, timing, severity, and other exposures. The reviewed sources do not identify a single nail pattern that points specifically to semaglutide or tirzepatide.

Medication review

The prescriber can compare the timing of the nail change with treatment initiation and dose changes, review other medicines, and decide whether the benefit-risk balance requires any adjustment. A temporal association is a reason to investigate, not a diagnosis.

Broader health and nutrition context

The evidence limits identified for this question include rapid weight change, nutrition, an underlying condition, or another cause as possible explanations that have not been resolved by the labels. Discussing eating patterns, medical history, and concurrent symptoms helps the clinician avoid attributing every change to the injectable medicine.

Dermatologic examination

A dermatologist can inspect the nail plates and surrounding skin directly and determine whether testing or treatment is needed. The American Academy of Dermatology specifically recommends partnering with a board-certified dermatologist for GLP-1-related skin, hair, and nail concerns.

What evidence would be needed to show a difference

To establish that semaglutide and tirzepatide have different nail side-effect profiles, researchers would need a direct comparison using the same nail definitions, examination methods, follow-up period, and reporting rules. Ideally, participants would be assigned to the two treatments and nail outcomes would be collected systematically rather than inferred from spontaneous reports.

Useful evidence could also come from well-documented postmarketing reports that describe the nail finding, timing, competing causes, dose, outcome, and whether the change recurred after re-exposure. A revised prescribing label could add a nail reaction if accumulating evidence supports it. Neither a direct nail comparison nor a nail-specific regulatory statement was identified in the sources reviewed.

Answers to common questions

Can semaglutide or tirzepatide affect your nails?

The reviewed U.S. labels do not list nail disorder for either medicine, so they do not establish a known nail side effect. They also cannot exclude rare or unrecognized events. A new or worsening nail change should be discussed with the prescriber and, when appropriate, a board-certified dermatologist.

Are nail changes more likely with one drug?

There is no nail-specific comparative statistic in the reviewed sources, and no head-to-head nail-outcome study was identified. The available information does not support saying that semaglutide is safer or riskier than tirzepatide for nails.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can the hair-loss percentages be used to estimate nail risk?

No. Hair loss and nail changes are separate outcomes. The reported percentages—3.3% with Wegovy 2.4 mg versus 1.0% with placebo, and sex-specific Zepbound figures from pooled Studies 1 and 2—describe hair loss associated with weight reduction, not nail events. Because the trial programs differ, their hair-loss rates should not be directly compared either.

Should you stop treatment if your nails change?

Do not stop, reduce, or switch a prescription medicine without speaking with the prescriber. Report the change, provide its timeline, and ask whether a dermatology assessment or evaluation for other causes is needed.

Bottom line for the nail comparison

As of the 2026 U.S. labeling reviewed here, semaglutide and tirzepatide look the same on the narrow nail question: neither label lists nail disorder, and no direct comparison shows a difference. Both labels contain hair-loss information associated with weight reduction, but that evidence cannot be used to infer nail effects. If your nails change during treatment, document the pattern, contact your medical team, and consider a board-certified dermatologist rather than assuming the medicine is—or is not—the cause.
