If you are a young adult newly diagnosed with cancer, ask your oncology team about fertility, hormones, sexual function, and future pregnancy before treatment starts whenever medically feasible. Request an urgent referral to a reproductive endocrinologist or urologist if you are interested, unsure, or ambivalent. At the same time, ask for a social worker or financial navigator and a psychosocial professional. These conversations can protect options that become unavailable after treatment begins.
Your diagnosis, anatomy, age, treatment urgency, location, and insurance determine which preservation methods are possible. Do not postpone cancer treatment on your own; have the oncology and reproductive teams coordinate timing.

What to do in your first oncology discussions
- Ask about risk in plain language. Say: “Could my planned surgery, chemotherapy, radiation, immunotherapy, or endocrine treatment affect fertility, hormones, sexual function, or a future pregnancy?” Ask the clinician to document the answer and identify which part of the plan creates the risk.
- Request a prompt referral. Ask for reproductive endocrinology, a fertility clinic, or urology as appropriate to your reproductive organs. The American Society of Clinical Oncology (ASCO) strongly recommends discussing infertility as early as possible and referring people who are interested or uncertain.
- Explain your goals without needing a final decision. You can want children later, have no plans for children, be undecided, or need time to consider family-building. “I do not know yet” is a valid reason for referral.
- Ask how much time is medically available. The oncology team should state whether a preservation procedure can occur before treatment, during a short window, or only after treatment. Never create a delay independently.
- Ask for coordinated support. Request a social worker, financial counselor or patient navigator, and mental-health professional familiar with cancer and reproductive decisions.
ASCO’s guideline summarizes the urgency: “Clinicians caring for adult and pediatric patients with cancer should discuss the possibility of infertility as early as possible before treatment starts to preserve the full range of options.”

How cancer treatment can affect fertility and sexual health
Fertility risk is not a single yes-or-no outcome. It depends on the cancer, the planned dose and field of treatment, the operation, age, reproductive organs, baseline fertility, and whether treatment changes hormone production or sexual function. Surgery can remove or injure reproductive organs. Chemotherapy can reduce sperm production or damage eggs. Radiation can affect the ovaries, testes, uterus, or hormone-control systems, depending on where it is directed. Immunotherapy and endocrine (hormone) treatment can also affect reproductive function or the timing of pregnancy, although the impact differs by drug and diagnosis.

Ask for a treatment-specific explanation rather than relying on a generic internet risk list. The team should distinguish between temporary and potentially permanent effects, whether periods or sperm production may return, possible effects on sexual function, and any recommended waiting period before attempting pregnancy. If you may become pregnant or cause a pregnancy, ask how treatment affects contraception and pregnancy safety as well.
Established fertility-preservation choices
The appropriate method depends on anatomy, diagnosis, treatment urgency, age, personal goals, and whether a partner or donor is involved. The options below are recognized in ASCO guidance for adults, but availability and eligibility differ by clinic.

| Method | What happens | Timing and practical issues | Status and questions to ask |
|---|---|---|---|
| Sperm cryopreservation | Sperm samples are collected and frozen for possible future use. | Usually requires collection before treatment; ask about the number of samples recommended and storage arrangements. | Established option. Ask whether illness, medication, or difficulty producing a sample requires an alternative collection approach. |
| Oocyte (egg) cryopreservation | Medication stimulates the ovaries; eggs are retrieved and frozen unfertilized. | Requires monitoring, stimulation, and a retrieval procedure. Ask how many treatment cycles are feasible before cancer therapy. | Established option. Individual chance of future use varies with age, egg number and quality, clinic expertise, and future circumstances; no single success rate applies to everyone. |
| Embryo cryopreservation | Retrieved eggs are fertilized and resulting embryos are frozen. | Requires sperm from a partner or donor and the same stimulation and retrieval process as egg freezing. | Established option. Discuss consent, storage decisions, and what happens if a relationship or family-building plan changes. |
| Ovarian-tissue cryopreservation | A portion of ovarian tissue is surgically removed and frozen for possible later reimplantation or other use. | Can be considered when there is little time for stimulation or when ovarian stimulation is unsuitable; it requires surgery and storage. | Established option in current ASCO guidance. Ask about surgical risk, the chance of returning tissue function, and whether it fits your diagnosis. |
| Ovarian transposition | The ovaries are surgically moved away from a planned radiation field. | Must be coordinated with radiation planning and may not protect against all treatment effects. | Established option for selected situations. Ask whether the ovaries can be repositioned safely and what protection is realistic. |
| Selected conservative surgery | When medically appropriate, surgery preserves reproductive organs or function instead of removing them entirely. | Depends heavily on cancer type, stage, anatomy, and oncologic safety; requires agreement between the cancer surgeon and fertility specialists. | Established only for selected diagnoses. Cancer control takes priority, and not everyone is a candidate. |
| In-vitro maturation | Immature eggs are matured in the laboratory rather than after a conventional stimulation cycle. | May be considered when standard stimulation is not practical. | Emerging option. Ask whether it is offered, what evidence applies to your situation, and whether it could affect treatment timing. |
ASCO does not consider gonadotropin-releasing hormone agonists a replacement for established preservation methods. If a clinician proposes one, ask what benefit is expected and whether a proven option can also be used.

When there is a short window
Tell both teams the planned treatment start date, whether a hospital admission is expected, and whether you need surgery first. The fertility specialist can explain the fastest feasible pathway, while the oncologist decides whether that pathway is medically safe. A procedure that is technically possible may still be inappropriate if it would compromise cancer care.

When treatment cannot wait
Ask whether ovarian-tissue cryopreservation, sperm collection, ovarian transposition, or a conservative operation can fit without a dangerous delay. If no preservation procedure is safe, ask about future family-building options and survivorship follow-up instead. The absence of a pre-treatment option is not a failure on your part.
What to document
- The treatment name, expected start date, and stated fertility risks.
- The referral date, clinic contact, and any tests or appointments needed.
- Instructions about contraception, pregnancy, sexual activity, and when attempts at pregnancy may be considered safe.
- Who will make decisions if the cancer plan changes.
Paying for preservation, storage, and later treatment
Coverage in the United States is uneven. ASCO reported that in 2024, 17 states and the District of Columbia had mandates applying to some state-regulated plans for medically indicated fertility preservation. A mandate does not cover every policy. Self-funded employer plans are generally outside state mandates, Medicaid coverage differs by state, and deductibles, coinsurance, exclusions, and annual limits still determine what you pay.

Build a written payment plan
- Ask the fertility clinic for an itemized estimate. Request separate amounts for consultation, laboratory tests, medications, monitoring, anesthesia, retrieval or surgery, laboratory processing, initial storage, and continuing storage.
- Call the insurer using the exact procedure and diagnosis information. Ask which services, medications, monitoring, and storage are covered; whether prior authorization is required; which providers are in network; and what appeal route applies.
- Ask whether a state mandate applies. Give the insurer your state, plan type, and employer information. Ask specifically whether the plan is state-regulated or self-funded.
- Get decisions in writing. Save reference numbers, names, dates, benefit summaries, denials, and appeal deadlines. Ask the clinic’s billing office to review estimates before you authorize treatment.
- Request financial navigation. A cancer-center social worker or financial counselor may identify grants, nonprofit assistance, payment plans, transportation help, or legal resources. Eligibility and available funds change, so ask about current requirements.
The National Cancer Institute identifies LIVESTRONG Fertility for financial support and clinic finding, the Alliance for Fertility Preservation for insurance-law information, and the Oncofertility Consortium for navigation and community resources. These organizations may have different eligibility rules and availability; confirm details directly with them.
Support for relationships, identity, work, and daily life
A cancer diagnosis in young adulthood can disrupt dating, sex, gender identity, plans for parenthood, school, work, housing, and friendships. Tell the team which problems are most urgent instead of waiting for a crisis. NCI says many cancer centers offer some combination of young-adult groups, counseling, fertility services, art or music therapy, adventure-based programs, and practical help with appointments, finances, legal matters, transportation, and social support.

Emotional and mental-health care
Ask for a psychosocial professional who understands cancer and reproductive decision-making. You can request individual counseling, a peer group, couples counseling, or support for a partner or family member. NCI’s patient guidance puts the need plainly: “It’s important to talk about how you are feeling emotionally—so you can get the psychosocial support that you need and deserve.” Seek urgent help if you feel unable to stay safe or care for yourself; tell the oncology team immediately or use local emergency services.
Practical assistance
- Ask a navigator to help combine oncology, fertility, imaging, and laboratory appointments.
- Tell the social worker about work or school deadlines, disability paperwork, transportation, childcare, food, or housing problems.
- Ask for legal or benefits guidance if treatment affects employment, insurance, privacy, or debt.
- Invite a trusted person to appointments, with your permission, and keep a shared list of questions.
Revisit fertility during survivorship
Fertility conversations should not end when active treatment ends. ASCO’s 2025 survivorship recommendation formally includes fertility preservation in survivorship care. Ask for a follow-up assessment when the treatment plan changes, when periods or sperm production do not recover as expected, or when you begin considering pregnancy or other family-building routes.
A survivorship visit can review hormone symptoms, sexual health, stored sperm, eggs, embryos, or tissue, storage fees and consent instructions, contraception, pregnancy timing, and referrals for assisted reproduction or other family-building options. Keep copies of preservation records and clarify who manages storage and how the clinic will contact you if policies change.
A concise appointment checklist
- What parts of my proposed treatment could affect fertility, hormones, sexual function, or pregnancy?
- How urgent is treatment, and what safe time—if any—is available for preservation?
- Which options fit my anatomy, diagnosis, age, and goals?
- What are the procedures, medications, monitoring, anesthesia, and storage requirements?
- What is established versus emerging for my situation?
- What costs are expected now and each year for storage?
- Which services does my insurance cover, and what are the authorization and appeal steps?
- Can I meet a social worker, financial counselor, navigator, and psychosocial professional?
- What contraception, pregnancy, or sexual-health precautions apply during and after treatment?
- When will we revisit fertility and family-building in survivorship?
About 70,000 people ages 15–39 in the United States were diagnosed with cancer in 2020, according to ASCO. Your own next step is personal: alert the oncology team now, obtain a coordinated fertility consultation when medically feasible, and ask for financial and psychosocial support before decisions become time-critical.
