In Diablo IV, World Boss gear rewards, Essence of Hellfire, and Heavenly Forge Sanctification are separate systems. Blizzard says the Urn of Calamity grants “additional gear rewards” from World Bosses, while Essence of Hellfire grants Lair Boss Keys for surviving World Boss encounters. Heavenly Forge Sanctification is a separate way to modify eligible items; the official description does not say that World Boss rewards or Hellfire keys are forged there.
What “+1 Loot” means—and what it does not establish
“+1 Loot” is not confirmed as an official feature name. Blizzard describes the Urn of Calamity as: “Earn additional gear rewards from World Bosses.” That wording does not promise exactly one extra item, specify a drop rate, or establish a fixed quantity. The phrase “+1 Loot To Forge” also appears in a secondary guide, but it is not confirmation of an official mechanic.

Blizzard’s description does not connect the Urn of Calamity or Essence of Hellfire rewards to the Heavenly Forge. In particular, the Hellfire reward described is keys, not equipment to forge.

How to find a World Boss
Blizzard says Azmodan is a permanent World Boss across Seasonal and Eternal realms, appearing in rotation alongside other World Bosses. The announcement does not give a live spawn timetable. Check the current in-game event and reward information for the schedule that applies to your character, realm, and patch rather than relying on a fixed time from an older guide.
The permanent-boss statement is from Blizzard’s seasonal announcement and should be read in that season’s context; it does not by itself confirm that every described seasonal mechanic remains available in the current live client.

What to check before you farm
- Use the in-game event UI to confirm the next World Boss event and its rewards for your character.
- Check the relevant Season Blessing and Essence of Hellfire details in game; do not assume “additional” means exactly one item.
- If your goal is item modification, check the Heavenly Forge requirements separately, including the Heavenly Sigil and item-quality requirements described by Blizzard.
- Confirm current season and patch availability in the client. The official announcement documents its described systems, but does not establish their live status for every later patch.
