Fever, aches, and rash overlap; where and when you were exposed, clinical assessment, and laboratory testing all matter. The most important immediate distinction is that dengue can worsen quickly around the time a fever resolves, while chikungunya often causes especially prominent joint pain, Zika may be mild or symptom-free, and West Nile can affect the nervous system.
How the symptoms compare
The patterns below are clues, not a way to diagnose yourself. CDC guidance notes that certain clinical and laboratory findings can help clinicians distinguish these infections, but overlapping symptoms make confirmation important.

| Illness | Typical pattern | Clue that may help distinguish it | Important context |
|---|---|---|---|
| Dengue | Fever, pain behind the eyes, muscle, joint, or bone aches, headache, nausea or vomiting, and rash. | Eye pain and widespread aches can occur; clinicians may also consider findings such as low platelet count, rising hematocrit, or low white blood cell count. | Watch for warning signs, especially after the fever goes away. Symptoms commonly last 2–7 days. (CDC, “Symptoms of Dengue and Testing,” updated August 7, 2025; CDC Dengue Case Management Pocket Guide, 2024.) |
| Chikungunya | Acute fever with prominent, sometimes severe joint pain; rash may occur. | Severe joint pain and arthritis are more associated with chikungunya than dengue in CDC guidance. Joint pain can persist for months or years. | Joint symptoms may outlast the initial fever and other acute symptoms. (CDC, “Clinical Signs and Symptoms of Chikungunya Virus Disease,” May 15, 2024.) |
| Zika | Often no symptoms or a mild illness; possible rash, joint pain, fever, and nonpurulent conjunctivitis (red eyes without pus). | Conjunctivitis and itch can be useful clinical clues, but neither proves Zika. | Infection during pregnancy can cause serious brain and eye birth defects, and the pregnant person may have no symptoms. (CDC, “Clinical Signs and Symptoms of Zika Virus Disease,” updated January 30, 2025; CDC Dengue Case Management Pocket Guide, 2024.) |
| West Nile | Illness can be flu-like or severe. | Possible involvement of the central nervous system is an important distinction from an uncomplicated fever-and-rash illness. | Symptoms alone do not establish West Nile; assessment and testing depend on the clinical picture and exposure history. (CDC, “Symptoms, Diagnosis, and Treatment | West Nile Virus,” page dated November 19, 2025.) |
CDC estimates that about one in four people infected with dengue become sick, while approximately 80% of people infected with Zika have no symptoms. These figures describe infections overall, not the odds that a particular person’s symptoms are caused by either virus. (CDC, 2025.)
Which differences are most useful?
Joint pain: intensity and persistence
Prominent, severe joint pain during an acute fever is a pattern more associated with chikungunya. Pain may continue for months or years after the initial illness. Dengue can also cause joint and bone pain, so the presence of aches alone does not separate the two.

- PUMP SPRAY: Perfect for outdoor travel – the compact size stores perfectly with whatever adventure gear you're taking that day.
Rash, itch, and red eyes
Rash can occur with dengue, chikungunya, and Zika. Zika is particularly associated with a rash and nonpurulent conjunctivitis; itch may also be a clinical clue. These features are not definitive, and a red eye can have causes unrelated to these viruses.
Neurologic symptoms
West Nile deserves particular attention when an illness involves the central nervous system. A change from a flu-like illness to neurologic involvement needs prompt medical evaluation; the symptom pattern cannot identify West Nile on its own.

- Outdoor bug repellent is formulated with 25% DEET and provides long lasting protection from mosquitoes, making this DEET mosquito repellent an ideal bug spray for camping, hiking, running and your favorite outdoor activities

Pregnancy and possible Zika exposure
Possible Zika exposure during pregnancy warrants medical advice even if there are no symptoms. CDC warns that infection during pregnancy can cause serious congenital brain and eye effects; clinicians weigh travel location and dates, activities, symptoms, and appropriate laboratory testing.

Recognize dengue warning signs after fever
Severe dengue can become life-threatening within hours. Go to a clinic or emergency room immediately if someone with possible dengue develops any of these warning signs:
Rank #3
- Non-greasy, lightweight bug repellent spray dries quickly, not sticky
- Long-lasting protection from this OFF! mosquito repellent spray lasts up to 8 hours to keep you protected from mosquito bites day or night
- Odorless formula that helps you avoid mosquito bites during mosquito season
- OFF insect repellent spray is proven to repel mosquitoes, and ticks
- Easy-to-apply aerosol bug spray makes it simple to spray, spread and go
- Abdominal pain or tenderness.
- Vomiting at least three times in 24 hours.
- Bleeding from the nose or gums.
- Blood in vomit or stool.
- Extreme tiredness or restlessness.
These signs often begin 24–48 hours after the fever goes away, so improvement in temperature does not by itself mean the danger has passed. CDC estimates that about one in 20 people who become sick with dengue develop severe dengue. (CDC, “Symptoms of Dengue and Testing,” updated August 7, 2025.)
Why symptoms cannot confirm which virus you have
CDC states, “A blood test is the only way to confirm the diagnosis” of dengue. A clinician combines symptoms with travel or residence history, exposure dates, and testing; the same approach is important when considering Zika, chikungunya, or West Nile.

For dengue, the appropriate test depends partly on how many days the person has been ill:

- After day 7: IgM is the primary dengue test.
Results need clinical interpretation. Dengue IgM can cross-react with antibodies to other flaviviruses, including Zika and West Nile, which can complicate the interpretation. CDC says commercial rapid dengue NS1 tests are not FDA-approved and their performance is not well established; an online rapid test is not a reliable substitute for medical testing. For suspected Zika, CDC says preliminary assessment also considers clinical features, places and dates of travel, and activities, with laboratory methods used for diagnosis.

What to do if dengue is possible
If you have fever after being in a place where dengue occurs, contact a healthcare professional and mention where you were and when. Until dengue has been ruled out, ask a clinician or pharmacist before taking pain or fever medicines: CDC West Nile guidance advises avoiding NSAIDs if dengue is possible in an area where dengue occurs. Seek emergency care immediately for any dengue warning sign listed above.
