An online medical visit is a clinical encounter delivered remotely through electronic communication, most often a live appointment with a health professional by video or audio. The term is sometimes used more broadly, so the exact meaning depends on whether you are talking about clinical care in general or a specific insurance program.
The short definition
An online medical visit is a healthcare appointment in which the patient and the clinician are not in the same room. The information moves through electronic communication instead. The familiar version is a real-time video or phone call with a doctor, nurse practitioner, or other licensed professional. Broader usage also covers messages that are exchanged at different times, which is why the wording in official sources is not identical.

Where the definition comes from
The Health Resources and Services Administration (HRSA) describes telehealth as the use of electronic information and telecommunications technologies to support long-distance clinical health care. The HHS Office for Civil Rights reproduces that definition in its FAQ “What is telehealth?”, which was last reviewed March 27, 2020. The same page says telehealth services may be provided through audio, text messaging, or video communication technology, including videoconferencing software.
That definition is deliberately wide. It covers clinical care, patient and professional health education, public health, and health administration. An online medical visit is the clinical slice of that wider category.

Live and asynchronous: the two timing models
The most useful distinction is timing. Telehealth guidance from HHS, published on Telehealth.HHS.gov, sorts remote care into two groups.
- Synchronous care is a live interaction. HHS describes it as “a live interaction between a provider and a patient,” with examples including video calls, audio-only calls when video is not an option, and secure text messaging.
- Asynchronous care, also called “store and forward,” means information is shared at different points in time. The clinician reviews it later rather than in the moment.
The table below places the common formats side by side. The timing column reflects the source wording; where a source does not specify timing, the cell says so.

Telehealth or telemedicine?
The AMA’s telehealth resource center notes that the two words are used differently. Telemedicine has historically referred to remote clinical services. Telehealth is the broader term and can include synchronous and asynchronous services, online digital visits, brief check-ins, and interprofessional consultations between clinicians. In everyday search and marketing language, the words are often used interchangeably, but a provider’s website or insurance document may use one term with a narrower meaning.

What typically happens during an online medical visit
HHS’s direct-to-consumer guidance describes synchronous telehealth as “real-time, virtual care.” Its examples include acute care assessments, primary care, and behavioral health. The same guidance says the live conversation may lead to a diagnosis, a treatment plan, or a prescription.

Those outcomes are possibilities, not promises. A clinician may decide that an issue needs an in-person exam, lab work, imaging, or emergency care. The format itself does not decide whether a particular concern can be handled remotely.

Readers often find a different definition when they look at insurance material. The Centers for Medicare & Medicaid Services (CMS) tracks Medicare telehealth use on its Medicare Telehealth Use Trends Data page, which was last modified June 18, 2026 and reports data through Q4 2025. For that measurement, telehealth means routine office visits provided through synchronous, real-time audio and/or video communication.

That is a data definition for Medicare, not a universal definition of an online medical visit. Medicare.gov’s “Telehealth Insurance Coverage” page describes what Medicare pays for in its own program context. It also covers separate formats such as e-visits and check-ins, which fall outside the live-visit model. Private insurers and state programs set their own rules, so a visit that counts as telehealth in one plan may be handled differently in another.

What an online medical visit cannot establish on its own
- Coverage. Being remote does not make a visit covered, and it does not reduce the cost of care.
- Suitability. Not every symptom or condition is appropriate for a remote assessment.
- Specific equipment. The definition concerns the communication method. A phone or computer can be used, and HHS includes audio-only and text-based modes. No dedicated device is required by the definition itself.
How to tell which kind of online care you are getting
- Check whether the appointment is scheduled for a specific time. If it is, it is most likely synchronous care.
- Look at how the visit is delivered. Video or audio means a live encounter. A message in a patient portal usually means asynchronous exchange.
- Read the provider’s or plan’s own description. For Medicare, rely on Medicare.gov and CMS rather than general definitions.
- If you are unsure whether the care is covered, ask the provider or your insurer before the appointment.
